Program of All-Inclusive Care for the Elderly (PACE)

The Program of All-Inclusive Care for the Elderly (PACE) is a managed care program for older adults. This program features a comprehensive service delivery system and integrated Medicare and Medicaid financing.

Enrollment:

  • Enrollment in PACE is voluntary.
  • The PACE organization’s Interdisciplinary Team (IDT) completes a comprehensive needs assessment.
  • If an individual elects PACE, an Enrollment Agreement is signed that contains the participant’s:
    • demographic data
    • description of benefits
    • effective date
    • explanation of policy regarding premiums
    • emergency care protocol
    • rights and responsibilities
  • PACE enrollment will continue until the participant's death regardless of changes in health status.
  • PACE enrollment will end if the:
    • Participant voluntarily disenrolls, or
    • PACE organization involuntarily disenrolls the participants for strictly defined reasons.

Eligibility and Enrollment:

To participate in PACE, an individual must:

  • Be enrolled in Medicaid only
  • Be enrolled in Medicare only
  • Be enrolled in Medicare and Medicaid (dually eligible)
  • Have the ability to privately pay

To receive PACE services, an individual must:

  • Be 55 years of age or older
  • Be determined to need the level of care required under the state Medicaid plan for coverage of nursing facility services
  • Reside in the PACE organization's service area
  • Be able to live in a community setting when enrolled without jeopardizing health or safety
  • Meet program-specific eligibility conditions imposed under the respective PACE agreement

PACE Services:

PACE Centers include a primary care clinic, an adult day health program, areas for therapeutic recreation, restorative therapies, socialization, personal care, and dining that serves as the focal point for coordination and provision of most PACE services.

A PACE organization can be a public or private entity primarily engaged in providing PACE services. A PACE organization must:

  • Have a governing board that includes community representation
  • Be able to provide the complete service package regardless of frequency or duration of services
  • Have a physical site to provide adult day services
  • Have a defined service area
  • Have safeguards against conflict of interest
  • Have demonstrated fiscal soundness
  • Have a formal Participant Bill of Rights

All PACE organizations are approved by Centers for Medicare and Medicaid Services (CMS) and NC Medicaid. Federal and state agencies are required to consistently monitor PACE organizations and ensure that they comply with state and federal regulations and provide quality care and services.

Each PACE organization must define its service area, which must be approved by NC Medicaid. The final approval is granted by CMS.

Personal Care Services (PCS) and Home and Community Based Service (HCBS) Waivers

Personal Care Services (PCS) provides personal care services to individuals residing in various living arrangements such as private homes, residential facilities, combination homes, and group homes. These services benefit individuals who require assistance with activities of daily living (ADLs), including eating, dressing, bathing, toileting, and mobility. Eligibility for the PCS program is determined by an independent assessment conducted by NC Medicaid, and services are provided according to an individualized service plan.

Home and Community Based Service (HCBS) Waivers pay for long-term care goods and services that help North Carolina Medicaid recipients who require a Nursing Facility Level of Care to remain living "in the community" instead of moving to a nursing home. These waivers cover some long-term care services but do not cover mortgage payments, rent, utilities, or food expenses. Unlike Nursing Home Medicaid, HCBS Waivers are not an entitlement, meaning that eligible applicants are not guaranteed to receive benefits if enrollment spots are full. The term assisted living residences (ALRs) in North Carolina covers two types of long-term residential care settings: (1) adult care homes (ACHs); and (2) multi-unit assisted housing with services facilities. The North Carolina Division of Health Service Regulation licenses ACHs based on size, with family care homes for 2-6 residents and ACHs for seven or more residents. A license indicates that this is the population to be served. Facilities may provide respite services, but provision of this service is not a condition for licensure. Multi-unit assisted housing with services settings are not licensed but are required to register with the Division of Health Service Regulation and provide a disclosure statement.

ACHs must provide specific information in writing to a resident upon move-in, including house rules and facility policies, discharge criteria, residents’ rights, and grievance procedures; and specific information in the resident contract, including accommodation and service rates, and rate change and refund policies. They may not admit or retain residents who meet the state’s eligibility criteria for nursing home care, or individuals with certain conditions or treatment needs.

Services provided by ACHs include 24-hour staff monitoring and supervision, and assistance with scheduling personal needs, as well as transportation, activities, and housekeeping services. ACHs must assess each resident within72 hours of admission using a state-provided or approved form and complete a functional assessment of each resident within 30 days after admission, as well as at least annually thereafter, using a state-provided or approved assessment instrument.

Multi-unit assisted housing with services facilities must screen prospective residents to determine if they need an in-depth assessment by a licensed home care agency, and if the facility has the capacity and legal authority to meet the prospective resident’s needs.

Medication provisions for adult care homes include orientation to medication policies and procedures for staff responsible for medication administration prior to their administering or supervising the administration of medications. Residents are permitted to self-administer medications as long as they are competent, physically able to do so, and have a physician’s order to do so. In addition, specific instructions for administering medications must be written on the medication label.

Apartment-style units and private rooms are not required in ACHs. In ACHs licensed after July 1, 2004, a bedroom may not be occupied by more than two residents. If licensed prior to that date, a bedroom may not be occupied by more than four residents. Bathroom and toilet facilities are shared with a minimum of one toilet and sink for every five residents and a tub or shower for every ten residents.

Special training and staffing requirements apply to dementia care staff. Prior to establishing an SCU, the administrator must document receipt of at least 20 hours of training specific to the population to be served for each SCU to be operated and must have in place a plan to train other staff assigned to the unit. Facility requirements for SCUs include providing direct access to a secured outside area and avoiding or minimizing the use of potentially distracting mechanical noises.

Inspection and monitoring of all licensed ACH facilities are subject to inspections at all times and are inspected on an annual basis. County Departments of Social Services monitor ACHs at least quarterly, investigate complaints, and accompany the state inspection teams for annual surveys. The Medicaid State Plan pays for personal care provided to eligible residents in ACHs.
