It further holds its stature above the state average. Besides, it also takes pride in a 4-star health inspection rating, the second highest of all rankings, positioning it in the top 30% of all facilities in terms of the health inspection rating, and remarkably, it stands higher than the state average on that aspect too. The institution excels in the quality of service, too, bearing a 5-star quality rating, the epitome of excellence, and finds its place in the top 20% of facilities based on quality ratings, again exceeding the state average.Staff Ratings
Trellis Centennial Nursing Home, despite having a 2-star staff rating which is the second lowest possible, exhibits commendable staffing metrics. While falling in the bottom 20% of facilities for nursing aid staffing, providing just 1.77 hours per resident per day as opposed to the state average of 2.29 hours, it compensates through other staffing areas. It is among the top 10% of facilities in terms of LPN staffing hours per resident per day, setting an impressive score of 1.46 hours, notably higher than the state average of 1.01 hours. It ranks in the top half of facilities for RN staffing hours, providing 0.6 hours per resident per day, just shy of the state average of 0.83 hours. The nursing home excels notably in the area of licensed staffing hours per resident per day, ranking in the top 10% of facilities with a score of 2.06 hours, surpassing the state average of 1.84. Similarly, in terms of physical therapist staffing hours per resident per day, it again ranks within the top 10%, providing a remarkable 0.23 hours, more than double the state average of 0.11. Thus, Trellis Centennial maintains its commitment to patient care through robust staffing measures, despite low staff rating.Covid Data

Only a paltry 6.9% of the residents have received their latest vaccinations, a figure that is markedly lower than the state average, which stands at a significantly higher 51.3%. Furthermore, a critical health concern is the fact that Trellis Centennial holds a 0.0% staff vaccination rate, surprisingly less than the state staff's average vaccination rate of 27.9%. This situation places the nursing home among the lowest 10% of facilities in this respect, highlighting urgent matters that require immediate attention.Quality Measures
The Trellis Centennial Nursing Home, boasting a rare 5-star quality measure rating, positions itself in the top echelon of nursing homes. Among its achievements, it surpasses 60% of facilities with a commendable record of only 14.29% of its residents requiring an antianxiety or hypnotic medication, a figure that is notably lower than the state's average of 20.0%. However, while Trellis Centennial excels in many areas, it falls within the bottom 20% of facilities regarding the administration of antipsychotic medication. The nursing home reports 23.81% of its residents receive this type of medication, which is significantly higher than the state's average of 13.64%. Despite this, the nursing home remains committed to providing the highest quality care to its residents.Emergency and Hospitalization Quality Measures
The Trellis Centennial is a well-regarded nursing home known for its high-quality care and standards. It ranks in the top 30% of facilities with regards to the percentage of short-stay residents who necessitate outpatient emergency department visits, maintaining a rate of 8.36%, which is notably less than the state's average of 9.95%. However, in terms of its short-stay residents who require rehospitalization post-nursing home admission, the Trellis Centennial finds itself in the bottom 30% of facilities. It has a rate of 27.39%, which is relatively higher than the state's average of 20.68%. Trellis was the third rehab my mother had been to in a year and was by far the best! They were so accommodating with great attitudes, have terrific PT people (Tab is the main guy), and a lot of activities that all the patients can partake in.My Mother is not a sociable person, but looked forward to the activities like Wii Bowling and the gingerbread house building. She talks about the rehab all the time as says how great it was to heal there!Thank you Trellis humans!!
Wish I could leave 0 stars but you have to give at least one to be able to comment. This place is negligent, the whole "due to covid...please excuse " is BS!!!! Covid doesn't prevent them from being able to properly bath and clean after diaper changes, etc. My mother came home with the worst bleeding diaper rash and peeling skin all over her body because of neglect NOT COVID!!!!!Also, just because the facility looks good doesn't mean the staff is good. They treated my mother horribly
Terrible communication and follow up. Due to covid my mother in law can't have visitors. I will leave politics out of this. My wife has asked 5 times to get on the approved communication list. Hopefully the fifth time works. Over a week there we still don't have information regarding her surgery or pathology reports. Nobody there seems to care about the families. We have no idea what's happening. One time, a staff member said "Why don't you ask your mother what's happening?" This was just days after major surgery and she is 81. She was very confused and didn't know any details. I will not recommend this facility especially during the lockdown.
The entire staff is the most incompetent people I know. The health care ln las vegas is horrendous anyway. My mother was ignored, received no answer to her call light. We got her out of there yesterday.A nutless monkey could have taken better care of her.It took them 30 minutes just to open the window so we could talk to her.All the staff are useless!
I myself was not at Trellis Centennial, but right now I have a family member at the facility. What I can attest to is the telephone system at Trellis leaves much to be desired. Why there are not telephones in every room is baffling to me. I live out of town and the first time I called and was told I would be transferred to the nurses station I questioned "why there was not a phone in the room", the answer was "because these days most people have cell phones". On that particular call I was disconnected twice, and had to call back. On two other occasions when I was transferred to the nurses station, I was told "all the patient phones were in use", and if could I call back in 30 or 40 minutes. Which tells me that if the patient phones are all in use that disputes the statement of "most people have cell phones".
My dad was just in Trellis Centennial for many many weeks and all I can say is if you love your family member, you will bring them to Trellis. From the therapists like Rain to the food to the caring nurses like Nicole the sweetheart and Gladys and Karen and Shelly and Courtney. . The list goes on and on.even to the wonderful lady Julie who answers the phone and Jamie the social worker who helped arrange things throughout his stay oh and the great caring doctors like Chandra and even sweet Jan the Activities director. . I literally could go on and on. My point is if you need a rehab, i highly recommend Trellis Centennial. I promise, you'll thank me :-)
Kristy, director of marketing, Annabelle, case manager, Lanie, director of nursing, nurse Jessica, cna’s Mykel and Sandra was excellent. If there was a concern, they took care of it. Physical and Occupational Therapy was excellent. This is why I choose this facility for my husband. The P/T and O/T people kicked butt. It was just what my husband needed before coming home. The one thing they need to do is add more CNAs. There is not enough of them and the ones they have work their butts off. No place is perfect but compared to other Rehabilitation Facilities Trellis was by far the better choice for us. I would use them again.
