Consulate Healthcare of Woodstock, VA is the dirtiest facility I have ever been in. People were overdosed this past spring 2020 on medication that they weren't even prescribed. When the Director and Director of Nursing were made aware of this the residents weren't sent to the hospital to receive treatment for the overdose. Two of them died within a couple of days. CNAs have routinely done wound care because the nurses refused to do it. Medication is found on the floor that residents should have taken. A resident was moved into a room with a woman who had c-dif without being told what she was being exposed to. The nursing staff wasn't given the proper personal protection equipment when caring for this resident and others with Mrsa. The staff could have, and possibly did, spread these illnesses to other residents. Several residents have unexplained black eyes and bruises. Please do not take your loved one there for rehab or long term care. The residents living there need to be taken to a better, safer facility then this place should be closed for good!
This is not a safe place to be a patient. I feel they are responsible for my mother's death. As a patient she suffered with COPD. At home her oxygen machine was routinely set to 4, the maximum! On the day of her death, the nursing staff had changed the level on her oxygen machine to 2.5. They were NOT giving her enough oxygen, and this hastened her death. I feel this was intensional on the part of the staff. They had no right to play God. My mother was old and very sick, but she still brought happiness to my father and me. No purpose would be served in suing Consulate for malpractice. My purpose in writing this review is to warn others about this facility and the staff. Rest in peace Mom.
If you have a loved one please take them somewhere else. This place sucks staff is not bad just overwork and need help, management is the problem this place needs HELP. as far as stars it would be a 0000000000000000000000000000000000000000000000000--------GOD help this place.
I am POA of a long term resident. We have experienced more issues than good care. The communication is lacking then excuses are made as to why or they try to place blame on the resident or POA. Finances not properly managed causing phone disconnects and needed items not able to be purchased. I have had to contact the state ombudsmen numerous times because of these issues. They have difficulty keeping staff with administration changing several times in the last year. Avoid at all costs and try to find another location for your loved one. I have a history in healthcare, including as an administrator and am shocked they still have a license
On several occasions asked for assistance to the bathroom with one nurse coming in several times. Eventually they came. Most often they appeared to be under staff. When they weren't they would completely ignore and not address the needs. Was to have physical therapy daily. No therapy on several days. Many days left to sit in chair. Was not given medications daily that was prescribed and needed per doctor. Asked for assistance in evening and wouldn't always get it or told that they would return and sometimes wouldn't. Didn't see the doctor while there for the duration of stay. Financial personnel came in to speak about what benefits being received and was insistent on having benefits to come to the facility. I fear for those that cannot speak for themselves or that does not have family to look out for them. This would not be a place I would recommend for anyone. I gave a one because there is not an option for less than that.
My mom's stay there was very short and we will do whatever we can to keep her from going back. She cut way back on her fluid intake so she didn't have to ask for assistance to go to the bathroom because it seemed to be a bother. Part of her hospital emergency room trip this morning was pnumonia, afib, very low bp and dehydration. No one from Consulate called me to say she was taken to the emergency room. Thats completely unacceptable. At least the nurse recognized that something was wrong, grateful for that.The hospital called after she had been there a couple hours. Thats how I found out.This is the final issue I have as the others will be addressed in a letter to corporate and state.As much as I had with the other issues I would have thought they'd called me for sure when she was taken to er. I did request to speak to a supervisor and administrator. They were busy I was told..
One of my relatives was in this facility and like most nursing homes/rehab places they are way too understaffed and your loved one will not receive the attention they need if just coming from a hospital setting . For 80 some beds at night there is, what I hear, only one person on staff. Probably not much better during the day, but I do know my relative had to wait hours to get attended to! This is unacceptable and would be better to go anywhere else....But check how many staff they have on hand at night before you go anywhere. ...
When my dad was at Consulate, the staff was very caring and attentive. We were surprised at how good the food was as well. The biggest surprise was how clean the facility was on a daily basis. It is the only facility I've been in that does not smell like urine! If you're in need of rehabilitation, the rehab department is top notch and the best in the area, without a doubt.

****update**** corporate did forward my message to the regional team that can help with my concerns patiently waiting for them to call. Rebecca was very nice in her emails and so far helpfulTo whom ever it may concern,I was happy to get a call today stating one of my residents was able to come home to our facility today, I gladly took report and waited on his arrival. I was told during the report he may or may not be on antibiotics and after they decided to check the chart I was told he wasn’t on anything. I was told he is swollen but the don’t know where the fluid was coming from, and also that he is on isolation alone and just stays in his room yelling: at first I was taken back when I learned he was yelling, this is not like him. I’m sure it was because he was alone. I took the information down and that was the end of that call. When he came home to my facility he stated he was so happy to be home to be out of your facility. He told me that during his stay he never felt more alone and my heart broke for him. I was shocked to notice he had a catheter from the hospital stay before because I was never informed of that during report and that is something all of our staff needs to be aware of. I was also appalled with the sore he had on his bottom. I checked his bottom after he complained about it burning and hurting, when I saw it I was taken back, I was truly heart broken from his pain. He never left our facility like that and I am disgusted he came back like that. With proper care and turning that could have been prevented even if it started forming at the hospital it could have been better cared for. He also looked like he hasn’t been bathed or shaved in weeks.You can choose what you do with my opinion take it and be productive with it or just let it roll of your shoulder which obviously that’s what your staff does. In my facility everyone is treated like family and we were all saddened to see him in that condition. The staff is very inconsiderate and rude. My mother is staying there, very briefly now, she asked for ice and was told that they don't provide ice. Then when she rang her call button, several times, because she had to use the restroom, was told by the staff that she was going to have to learn to hold it. Also, when I called to speak to her, I was told that they couldn't transfer my call. I'm very upset and disappointed with this facility!
