Its quality rating stands at 2-stars, the second lowest possible, and is in the bottom 20% when looking at similar facilities' ratings. Unfortunately, this rating is also lower than the state average. This broad trend of low ratings suggests a need for significant improvements in various aspects of the facility.Staff Ratings

However, with regard to RN staffing hours, it makes an improvement and is seen in the top 40% with 0.68 hours, although it still falls beneath the state's average of 0.72 hours. The troubling figure of 11.88% of residents experiencing significant weight loss surpasses the state's average of 7.02%, highlighting an area in need of focused improvement.Emergency and Hospitalization Quality Measures
At Apple Rehab Middletown, certain performance indicators associated with the quality of their patient care appear to be less favorable compared to the state averages. First, I am sorry for the negative post, but I need to share this...I want to warn you all that if you have any elderly loved ones in an APPLE CARE facility, it is in your best interest to find a safer place for them to be cared for.I taught yoga at two of their facilities from November - March and was SHOCKED.In the short time that I was there, I witnessed a number of staff changes. A putrid odor of urine, feces, and bleach (so much so that I had difficulty breathing). Patients left out alone in the middle of the hallways having panic attacks. Some of the staff that I interacted with were extremely rude, not just to me, but to the patients as well. Telling them to "shut up."After sharing my experience with a few people, I have also heard other's horror stories about their negative experiences with Apple Rehab (Like staff refusing to change patients after they wet themselves). I also have a friend who works there and has shared with me how cheap and understaffed they are.I want everyone to be aware of what kind of care is provided.I do not want to blame all the staff because some of them are good and actually do care, but it seems that the people at the top do not have the best character. Apple Rehab hired me to teach Chair Yoga and has not paid me a dime. The staff kept telling me that it takes a while for them to process my invoice, but after 4 months of no pay it looks like they just took complete advantage of me.The residents there are so sweet and I worry for them.
I’d recommend taking a full inventory of your loved ones possessions if you unlucky enough to stay at this facility. My father was a short term resident who had to go back to hospital after a fall. When I called about picking up his things I was told they were all lost. I wonder how many people who have stayed there had their glasses, cell phone, shoes, clothes etc. “lost”?
My grandmother was there back in August she's 94 and hearing impaired. They took forever to bring her to the bathroom when my mom told them she can't hear them very well the girl laughed and said no one here can hear very well my grandmother didn't know what was going on she didn't know how long she was going to be there or what the plan was my mom said they wanted to keep her as long as the insurance company would pay for it we had to fight to have her discharged home in a timely manner I would not Recommend this place
Horrible! My mom went in for rehab, not to sit in her chair all day staring into space. Mom had brain surgery & needed medical REHAB, instead of inexperienced aids with very little oversight from the nurses. Lets see... numerous times not getting her out of bed until we came in after 11am, finding her still in bed, shivering because she peed, closing her door leaving her alone with no way to call for help, I pressed nurse button while helping her in bathroom & no one came after 15 min so I did it myself, not making sure she drank enough to stay hydrated, not using straps to move her, not protecting her head while moving her in bed (after brain surgery!!), giving her the wrong medicine which set her back, leaving her in her room for hours with no one checking in, not taking her to any activities or social dining, told that they only had staff for 30 min a day of PT. Really? This is a "REHAB"?! Either the staff is overworked, under trained or lazy. Not sure which, but don't really care. Therapists and activities directors were great but spread thin. Nurses & aids were sub-par with the exception of Ruby (why she stays there is beyond me). Numerous patients wheelchaired next to nurse station for hours on end with no attempt to stimulate them (big red flag there). The grounds were unkept, no music. We transferred my mom to another place & I am realizing more and more how horrible and unsafe Apple Rehab was. After two days, she is already using her walker, using the bathroom (instead of the commode), dining & talking with others, doing activities. Shame on you administrators.
This place is wonderful! The staff is professional and caring. The physical plant is clean, open, and visitor friendly. The occupational and physical therapists are amazing and helped my mom heal better than she was before she came in! Of course there are normal staffing problems like any other place so it is not perfect but close!
