1. Waitlist Availability
I have a short waitlist, but I will be having a room come open soon.
2. Pricing and General Information
At Southern Plaza, we offer private suites with three floor plans to choose from, each equipped with a kitchenette, a full-sized refrigerator, microwave, and an individual central heat and air unit. Services include medication management, a pendant for calling our 24-hour licensed care staff, three full meals a day, weekly housekeeping and laundry service, transportation to local medical appointments, and many daily social engagement opportunities. Our pricing currently starts at $3850.
3. Professional Staff and Training
We have an RN as our full-time employee in the building M-F daytime hours as our DON, an LPN M-T daytime hours, and these two nurses have 2 assistants that are ACMAs. The Executive Director is licensed by the state of Oklahoma. Our care staff are either CNAs, CMAs, ACMAs, and staff assigned to give medications have their MAT. We conduct monthly in-services and evaluate practice skills throughout the year.
4. Resident Safety and Well-being
Our organization follows extensive state guidelines for safety and well-being, featuring an extensive fire safety system, 3 FEMA approved saferooms, and we opened our doors in October of 2017.
5. Transition Support Services
Yes, we provide numerous resources to help residents transition into the community.
6. Transportation Services
We offer transportation to doctor’s appointments through reservations with our front desk, along with shopping, dining out, and other outside ventures through our activities coordinator.
7. Dining Experience
We offer restaurant-style dining with a choice of 2 main entrees at lunch and supper, and a variety of breakfast options. Special treats like homemade cinnamon rolls are also offered.
8. Addressing Complaints and Issues
Our executive director has an open door policy to resolve issues promptly.
9. Communication with Residents' Families
We maintain communication with families regarding any incidents, concerns, or health issues, and provide updates on community health situations via email.
10. Short-term or Respite Stays
Our community usually stays full and does not have the empty rooms for respite stays.
11. Pet Policy
We allow one pet that weighs 20 pounds or less in the assisted living, with certain conditions and a $500 non-refundable deposit.
12. Covid and Flu Vaccinations
We have held many COVID and flu vaccine clinics for residents.
13. Aging in Place
We allow aging in place with a plan of accommodation to satisfy state requirements and ensure the best care for our residents.
14. Activities and Socialization
At Southern Plaza, we offer a broad spectrum of activities to support creativity and socialization in both Assisted Living and Memory Care, which is a separate community in our same building.
