Our Team and Services
We have RNs, CNAs, a Certified Wound Nurse, Physical Therapist, Speech Therapist, and Occupational Therapist.
Quality Assurance and Management
The administrator, along with Nurse Managers and Managers, conduct QAPI on a monthly basis to check on daily quality care, etc. Our Social Worker works with families and insurance to assist with the transition from facility to home. We also provide transportation for appointments, etc.
Dining and Nutrition
Our Dietary aides and CNAs assist with dining either in the dining room or resident rooms.
Grievance Procedure
We have a grievance procedure in place for families and residents if they have any complaints.
Financial Information
We accept insurance for skilled services and Institutional Medicaid for Long Term. Private Pay is $260 per day or $8,060 per month.
Communication
We communicate by phone or email to all families.
Facility Offerings
We are a hybrid facility: we offer respite, hospice, skilled, and long-term services. We allow pets to visit residents as long as they are vaccinated. We currently have availability for all services and do not have a waiting list.
Admittance Process
There is a vetting process for clinical and financial prior to admittance.
Health and Safety
We provide vaccinations for flu, COVID, etc. We do have residents that age in place.
