    Overall Ratings

Salem Home, a nursing facility, ranks within the top 50% for its overall rating, which slightly exceeds the state average, revealing a standard of care significant across myriad nursing homes. Despite this, the nursing home does have its shortcomings, positioned in the lower percentile for specific ratings. A slightly disconcerting consideration is its 2-star health inspection rating, a grade only a notch above the lowest possible and which positions it in the bottom 40% of comparable facilities. Further, this rating falls below the state's average. Similarly concerning is the 2-star quality rating, the second lowest achievable score, placing the nursing home among the bottom 20% in terms of facility-wide quality. This score is, disappointingly, below the state average.

    Staff Ratings

Salem Home is a highly-rated nursing home, boasting a top-tier 5-star staff rating. Ranking in the top 10% of facilities, it provides a remarkable 3.08 nursing aid staffing hours per resident each day, significantly higher than the state's average of 2.53. However, with regards to LPN staffing hours, Salem Home finds itself in the lower 10%, contributing only 0.29 hours per resident per day, compared to the state's higher average of 0.66. In terms of RN staffing hours, Salem Home fares better, finding itself in the top 40% of facilities, providing 0.71 hours, although this is equal to the state's average of 0.71. Licensed staffing hours per resident per day at Salem Home is lower than the state average of 1.38, making it fall within the bottom 10% of facilities at 1.01 hours. Finally, physical therapist staffing hours at Salem Home is less than ideal as well, ranking in the bottom 20% of facilities with 0.01 hours, falling short of the state's average of 0.05.

    Covid Data

The Salem Home nursing facility is distinguished in its commitment to vaccinating its residents and staff. Notably, this distinguished care home ranks in the top 40% of facilities when it comes to the rate of its residents who are up to date on their vaccinations. Specifically, Salem Home boasts a resident vaccination rate of 76.2%, significantly surpassing the state's average of 62.4%. It’s not only the residents who are keeping pace with their immunizations though, the staff at Salem Home also rank in the top 40% of facilities for vaccination uptake. Despite the modest difference, the Salem Home's staff vaccination rate of 22.5% is still notably higher than the state's average staff vaccination rate of 20.4%.

    Quality Measures

The Salem Home is a nursing facility with only a 2-star quality measure rating, showing a considerable need for improvement. Despite the need for progress, it performs better than the state average in regards to the percentage of residents that have experienced one or more falls with severe injury, standing at 3.95% as opposed to the state's 4.88%, though it is still in the bottom 40% of facilities in this respect. However, there is greater concern in other areas, as the home is among the bottom 20% of facilities concerning the percent of residents with a catheter inserted and left in their bladder, with a proportion of 3.46% compared to the state's lower rate of 2.35%. Additionally, it falls into the same percentile regarding residents who lose too much weight, with an alarming 8.89% compared to the state's 5.68%. The Salem Home is also in the bottom half of facilities when it comes to residents who received an antipsychotic medication, although its percentage (14.49%) is less than the state average (18.52%). While it stands below the state average in residents' falls with severe injury, it is nevertheless among the bottom 40% of facilities in this category. These figures demonstrate substantial areas for the Salem Home to address to meet and exceed the standards for nursing home care.

    Emergency and Hospitalization Quality Measures

Salem Home, a nursing facility, demonstrates a mixed standing regarding its hospitalization rate, outpatient emergency department visits, and rehospitalization of short-stay residents compared to state averages. Falling in the bottom 30% of facilities, Salem Home's hospitalizations per 1000 days is higher at 2.38, as opposed to the state's rate of 1.85. The facility's outpatient emergency department visits are also greater than the state's, with Salem Home at 2.02 per 1000 days, putting it in the lowest 20% of facilities. This is compared to the state's rate of 1.36. On the other hand, it ranks in the top 40% for the percentage of short-stay residents who had such visits, with a lesser figure of 9.4% compared to the state's 12.03%. However, Salem Home's percentage of short-stay residents who were rehospitalized after a nursing home admission is 30.9, notably higher than the state's 21.86%, putting it in the bottom 20% compared to other facilities.

My husband was at Salem Home for 5 1/2 years and I am so thankful to all the staff for his wonderful care. I usually just dropped in on him and never caught him in a distressful situation in all that time. I’d ask him is they were good to him and the answer was always yes and he wasn’t an easy man to please. When he passed away they were there to comfort us like family. I cannot thank you enough. What a blessing you were to us. Mary Ann

HORRIBLE staff. Treat the residents like animals. Literally had my family member peeing in a plastic jug because they wouldn't give him a urinal. Then tried to intimidate us into not reporting them to the state. Also would not transport my family member to required doctor appointments. The DON Kelly Kidd was hateful and rude when I tried to talk to her about what was going on.Honestly, Salem Home should either be shut down or have everyone canned and start over. I will NEVER put another family member in there.
