    PLEASE do not have your loved one go to this facility!! St. Rose Sienna sent my brother here as the only place that would accept him and the Medicare timeline for inpatient was up. He had a brain tumor, and after three inpatients, two rehabs, and one discharge to home in two months WITHOUT DYING.....he died here in less than TWO DAYS!!! We were going to take him home on hospice in a few days. He had four nieces and nephew coming the next day to see him from out of state.  They only allow you to visit for ONE HOUR a DAY, so you can't help monitor the patient. My brother was sent there AT 2:30 pm. I ARRIVED AT ABOUT 5:00 AND FOUND HIM CHOKING ON A PILL!  the NURSE JUST SAID "WELL I'll GET HIM ANOTHER one"!! HE HAD PROBLEMS SWALLOWING AND TALKING, SO NEEDED MONITORING AND GIVEN FOOD WITH HIS MEDS! The next day I went for my ONE HOUR and his head was lower than it should have been. The next day I got a call in the AM that my brother had died, THEN within 15 minutes got a call from the case manager that she had just seen him! She checked, and yes, he was deceased!!! When i went to the facility the board in his room still had the nurse from the day before listed. MY brother choked to DEATH!!! I spoke with  admin who was nonchalant. I had read the bad reviews before he went, but we were froced into it(with pink forms if we wanted to contest his discharge) DONT send your family here!!!!!!!!

    . NEEEDS TO GO OUT OF BUSINESS!!! THIS PLACE WILL PUT YOUR LOVED ONES IN DANGER!!!!My father was sent here from Mountain View hospital  for his post acute therapy!! He did not even last 14 hours in this trash sage creek facility. When we arrived they did not have his special bed he needed for his bedsores and I was argued with by the staff that it was to be approved by the dr before they could order one! I was told 10am the morning of him being admitted that by 4pm his bed would be there! We arrived @6:30pm still no bed!!! they made speak to my Father through the side window in the rocks of the buildings landscapingMy father that has suffered a massive stroke and has severe brain damage and memory loss so it is crucial For him to be around familiar faces and voices!!! this facility stated that it's cdc guidelines to quarantine 2 weeks for patients but I had contacted the cdc and health department and they said as long as he is COVID free that quarantine is not mandatory unless it's requested by facility! This facility took my father in and I was contacted by the staff prior to his arrival and so was a case worker From Mountain View and we thoroughly went through my fathers medical history his meds and precautions to follow. Sage creek agreed and understood the severity of my fathers condition! Tell me why...... my father face timed me @ 9am and said he feels like he's dying and nobody is responding!! I was on the phone with him for 10 minutes and not one staff member went to help my father!!!! come to find out they gave him fluid and pills by mouth when they were advised to NOT feed him by mouth because he can't swallow from the stroke and he has a feeding tube in his stomach that they were aware of.  I rushed to the facility with myFather on the phone I heard my father tell sage creek staff  he's choking and for help i them called 911 from my work phone while I had my father on FaceTime on the other phone. The drive from my house to the facility  was about 15 minutes plus the 15 minutes I was going n the phone prior to the drive and nobody came to help him!!!! I then arrived to facility and they tried to have me deal with my choking father through a window while they weren't helping him! I then demanded for someone to unlock the door so I could goto my fathers room and help him! The guy in charge told me I wasn't allowed in the back and completely disregarded the fact that my father was choking and screaming for help and his staff want helping!  All he cared about was telling me that my father is no longer allowed at the facility because I "an outsider" was not permitted in the building!!! Paramedics then showed up and all this facility cared about was having me sign a paper to release them of any lawsuits.

    My mother's nurse got COVID and this nurse helps almost every single person in this facility (they are severely understaffed). It's spread to 7 people throughout the building now. There is something wrong with how they handle COVID here.   Additionally, my mom was late to her doctor's appointment because no one told her that the person who was going to drive her got sick, even though management knew about it the day before. She was also scheduled to go home on a specific day, but she needed an RN to do something for her to leave, and no RN was on duty when she was supposed to leave. Now she has to stay longer than she was supposed to. No one in management here knows what they're doing.

    After a week of battling pneumonia at St. Rose Hospital, my father was sent to Sage Creek.  My father received good care there.  The staff was attentive to his needs.  He especially wants to thank: CeCe, Raul, Tina, the PT ladies and Alex, the van driver who stayed with my father for an extensive amount of time while my dad received a blood transfusion.  Alex was very comforting to my mother as she waited for the transfusion to end.  Alex was extremely kind and caring.  Our family is grateful to all of you who helped us so much during this difficult time.  My father would not be as far along in his recovery without your help.  The facility was bright and clean and COVID protocol was carefully followed.  The menu offered many choices.  We also want to thank the team that helped us devise a plan to utilize at home.  Thanks, again, the Jaeger family.

    I thought I was the only person unhappy with this facility, but after reading other reviews I see there are others.My husband has been in the facility since 02/09/21, and the care he received since he has been here is unethical to say the least.  From no one answering when he needed help to his feeding tube spilling all over him and his bed because a nurse did not cap the tube correctly.  My husband was suppose to be discharged on 03/20 to which I appealed with the insurance company and it was denied, I appealed it again because my husband was no where near ready to go home. instead of Sage Creek calling me to see if I wanted to wait for the second decision on the appeal or to go with the discharge, they took it  upon themselves to keep him there. My problem with this is what gives you the right to make that decision. When I spoke to Glen and ask this question he could not give me a feasible answer except we were waiting for the insurance companies decision. Wrong answer!Every time you need to speak with someone they are always in a meeting. If they concentrated more on the patients instead of those meetings maybe these negative reviews would stop.  Management should take the time to read the reviews and maybe just maybe the facility could get better,I don't like writing  negative reviews on anyone, but when it comes to my family  I don't have a problem doing it. This facility could be one of the best under new management and new staff. Hopefully they get better, I can't see them getting any worst.

    My mother had to come here because St Rose Acute couldn't keep her at their facility past 14 days. She was improving and thriving  so well from a hip replacement. My mother was here for 10 days and died July 3rd in this facility. I wouldn't place any of my relatives here. The only helpful person was Wendy. Everyone else was to busy in meetings. Because of COVD-19 I could not visit her and had entrust the health care workers to take care of her. The Facility missed her Doctors Appointment and displaced the blame amongst the staff.  I also received a 25,000 dollar bill for the month of July , my mother was only in the establishment for 3 Days in July ; but the bill was sent for the entire month, Why?  My family never received a sympathy card or phone call from this facility upon my mother's passing. This is just a short summary of the facts involving my mother.

    The care provided at Sage Creek was exceptional. Staff was very courteous and professional. Physical therapy really took care of me well.  They really made him work which made his recovery quicker than normal.  The facility was very accommodating to his needs. I would highly recommend Sage Creek for rehabilitate. I would like to thank the staff again for all their exceptional care they gave.

    This facility talks like they really do work for you.  My brother is suffering there.  First, they have not supplied him with a hospital bed and he fell from their inadequate bed.  Thankfully he was not hurt.Second, I have been trying to reach a health professional to discuss the limited physical therapy that they provide which is the keystone of my brother's therapy need.  Right now, he receives one session a day for half hour to an hour 5 or 6 days a week. The rest of the time he spends unhappily in bed with no way to help himself get better since he can't yet sit up on his own or walk. What kind of rehabilitation is that?Third, the health professional did not return my calls.  FIND A BETTER OPTION

    Place is not managed well. They are all unorganized. Staff is underwhelming and not knowledgeable. Looks like they all took a 3 month class to be in there position. The Doctor here doesn't know patients at all or the medicine he is distributing. All that schooling and all you can fix is a cold. Pathetic! Get yourself more Knowledgeable staff. I asked to see my mom and they didn't even know she was in the emergency room from there negligence! Save yourself sometime and go to Kindrid.

    Many months after our experience at Sage Creek, we learned more about how the rehab thing works with elderly post stroke patients. My father had been technically living in an elder group home (which is a wonderful place - True Loving Care - because my parents home is not equipped for his needs nor can my mother care for him in his current state. My father has been given many more opportunities for 2+ week rehab stays since July 2019. When he got stronger and was improving, he was transferred back to a rehab (a different one). After a few more brief hospitalizations, he went for more 2+ week rehab stays before returning to his adult home. He's had an up and down course, and looking back, the seeming premature discharge from Sage Creek was neither unusual nor inappropriate. In addition, I've consulted with PT/OT colleagues who have explained why they tried to get my dad standing right away, way before I thought he was ready. I was told that if his pre-stroke functioning was complete independence with walking, then they had to evaluate how close or far he was from his pre-stroke functioning. I gave 4 stars because the nursing and therapy staff was great. Communication by the administration and doctors to the family was terrible, however, as they seemed to do everything they could to avoid calling back. It would have been very easy for them to explain what I just explained above. We were a first time family dealing with a stroke. You would expect educating families would bd a big deal in the rehab process. Other facilities we have been to since, had much better and more helpful communication with families.

    Talking about being FRUSTRATED WITH THIS FACILITY. These people tell me twice about my mom being discharged and NO ONE returns my calls/emails to give me any discharge follow-up info (after leaving 3 phone messages/email. I will write another review when my mom is discharge!

    This is a beautiful facility with extremely friendly staff from front desk, nurses, doctors to OT & PT and a kitchen staff willing to make changes to meals, with a smile. The place smells great, is meticulously clean and couldn't be a better place to heal!! They treat the patient and their family members like "family". They keep patients and family members informed and actively engage family members in learning and  becoming active in the patient's treatment plan. They "hear" you and I highly recommend using Safe Creek as your place to recover and transition back home. Its been a fantastic experience watching our mother receive TLC every single day!

    The conditions of the staffing  at Sage Creek Acute is deplorable I don't know how the company can live with itself knowing that it's understaffed and not considering the patients let alone the nurses and the CNA nurses.All I can think of is that sage Creek must be in dire need of money therefore cutting staff at the sacrifice of the patients who are trying to get themselves better.It's pathetic, when the paitient calls for bathroom help and it takes close to 15 minutes for someone to acknowledge  and then left there for another 15 minutes to get help out of the litrene. How unfair it is again to the staff the nurses the assistant nurses running around taking care of two separate wings while trying to maintain.Hire more staff!!

    If I could give zero stars I would, this is one of the worse rehabilitation place I've ever seen. The nurse barely EVER respond to the patients room calls, the CNA's are doing most of the nurses work for them because they don't show up when called. The food is the absolute worse, my grandparent has a no salt diet and her dinner had soooo much salt on it, it was intolerable to eat. There is never anybody at the front desk, I watched a transport person sit outside with another elderly waiting for 10mins in the cold for someone to let them in. I highly suggest you don't allow your family to stay here.

    I’ve been in here less than a day and the staff doesn’t the call light. The care is terrible!This is the worse facility I have ever worked in or been a patient. The PT and OT departments do the nursing care and leaves hardly any time for therapy! It’s really sad. The CNAs are over worked. No staffing’

    Worse place I have ever been understaffed they treat you like thier doing you a big favor to take care of you gave me the first Covid shot and never gave me a card or had any documentation that I had one then sent me home at midnight with no care for the weekend took 4 days to get home healthcare would take 2 hours to answer call button left to lay in urine soaked bed make you wait 6 to 8 hours for pain medication that was ordered by doctor every 4 hours than sent me a bill for $1200.00 I wouldn’t recommend this place for my dying mother

    My wife just got out from Sage. The night care was horrible. No one came when you pushed the button for help. People were screaming for help for over two hours. It took 2 days for her laundry to get done.The room was clean. The day people were very nice and helpful. The food was good. The therapist was excellent.

    My Mother was there a year ago in January of 2020. The care was good and you could engage with the staff and patient.My Mother was there this year again after a femur break. She was in a back room, did not get the care she needed including bathing, had to see her outside a window with covid. Some nasty CNAs. Could not engage with nurses or staff. I then sent my mom to respite care across the driveway , same owners and within 48 hours the CNA was to the side of her and she fell backwards. She cut her head and no idea what else happened as she was sent to St. Rose and then to another rehab. I would be careful if you are considering either facility.

    This is a brand new facility so clean and sterilized! The staff really treats their patients with so much compassion and TLC! My husband was rehab-ed there for two weeks and they welcome food orders! So understanding!

    Do yourself a favor and stay away from this place! It is a terrible facility and it seems they don’t care for the patients well being. Some Asian lady who I believe is the director of nursing, has to be the rudest person I have ever dealt with. She was so rude and condescending when she spoke to me and wouldn’t even let me voice my concerns. It’s incredible to me how a facility would let such a nasty person like this work there. I will never go back there again and quite frankly nobody should take their loved ones there. Very unprofessional facility and it seems like no one there knows what they are doing, especially that rude Asian lady who wouldn’t even give me her name.STAY AWAY!!!!

    This is my third stay at Sage Creek.I have felt each time I was a patient there I was well cared for. Surroundings are comfortable.But I absolutely must say that the NURSES and CNA and other patient care professionals are where Sage Creek shines. That staff remembers your name and your basic information. They have almost all remembered me from visit to visit.This time I had a broken back. I needed gentle care and lots of physical assistance. I could not find a place that I feel would be so over the top in the care I have received . They have enough staff on hand to lift me and help me do all that I must to recover. I want to make a heartfelt thanks to each and every one. Thank you so very much.They are also very adherent to the regulations regarding Covid.

    My mother had to come here because St Rose Acute couldn't keep her at their facility past 14 days. She was improving and thriving so well from a hip replacement. My mother was here for 10 days and died July 3rd in this facility. I wouldn't place any of my relatives here. The only helpful person was Wendy, the case worker. Everyone else was to busy in meetings. Because of COVD-19 I could not visit her and had entrust the health care workers to take care of her. The Facility missed her Doctors Appointment and displaced the blame amongst the staff. I also received a 25,000 dollar bill for the month of July , my mother was only in the establishment for 3 Days in July ; but the bill was sent for the entire month, Why? My family never received a sympathy card or phone call from this facility upon my mother's passing. This is just a short summary of the facts involving my mother.

    The food is terrible 😭 the amount of food is barely for a child and they don't follow your diet plans. My dad has been here already a little over a week and he has had no therapy and the nurse doesn't come in the room unless they have to. It is terrible here and upper management should come in and check how this place really is!

    Nursing staff is very slack in there duties and got so bad that my mothers stay ended up in an ambulance ride. If you go there do not get sick or if your health diminishes get out of there before you end up in ER. My mother was there a short stay and every time I visited there was an ambulance taking someone to ER.The physical rehabilitation was great and I cannot stress this enough the nursing staff is horrible.

    I was in Sage Creek. It was like a snake pit. They took blood tests pm Me but never told Me that I had a Uti.The service was horrible by some of the nurses. One nurse made me sleep in a urine covered bed for the night. Now they are trying to bill me for $1500. Imagine that?It doesn't get any worse. Kindred was much better..I can"t give Sage creek 1 star but I guess I have to to post a comment.When friends called me i was so heavily medicated they thought i had a stroke.i was taking a medicine for pain in the stomach. i couldn;t even talk on the phone. I was grossly overmedicated i didn't know how long i was in there. I fell once in my room trying to use a walker.my Sister ddn't think i was coming home.A friend of mine called my in Cqnqdq to see if she could come down..

    My sister was transferred to Sage Creek from Henderson hospital at approx. 8:15 p.m. on 6/18 after experiencing a major illness that has impacted her health severely.We were here for 30 mins before someone came in the room. Staff did not identify themselves by name or role. It was apparent they didn't know the patient's health history. They seem to operate in silos focused on their job only and policy. Communication among the staff seems very poor. Late night interruptions with no care plan provided on what to expect. An attempt to just give her a TB shot without asking about her history. The information is in her health record that she has an autoimmune issue, and giving a TB shot after recovering from a bacteria and infection in her blood should have been something staff knew or were aware of. The environment is very sterile with no welcoming atmosphere. Had it not been for someone from the family being with my sister 24/7 I believe they would have given her that TB shot which could have caused complications.Administration might consider training staff on effective communication and great customer service. Doing a shift report and share information collectively to every staff that participates in patient care.While the facility is clean, no phone in the room makes it difficult for family to connect with loved ones. Improvement in the long intervals and wait time to explain what to expect and create some kind of schedule for care.

    I'm out of state and have had a difficult time reaching my father-in-law. They don't have phones in the rooms, and they pass a "universal cell phone" around between patients. I work in healthcare in another state, and this is an infection control issue, as well as extremely frustrating for the patient and family who need updates. My father-in-law is fully alert and oriented at baseline, but is now incoherent due to pain meds, not sure why he's there, and we are unable to get updates from staff due to HIPAA laws. I spent 5 hours trying to connect with him and was told "another patient is using the phone, can you call back in 10 minutes" about 5 times. Family is coming in from 3 other states to try to figure this out. A working phone is a minimum requirement.
