    Overall Ratings

Meadowbrook Rehabilitation Hospital is a nursing home that, unfortunately, ranks quite low in terms of quality. It has been given a 1-star overall rating, indicating that it is in the bottom 20% of facilities in terms of overall rating, which is lower than the state average. Notably, it has also secured a 1-star health inspection rating, considered the lowest possible, placing it in the bottom 10% of facilities in this category, once again, below the state average. Furthermore, its quality rating stands at 2 stars, which is the second lowest possible score and positions it in the bottom 20% of facilities per quality rating. This too is lower than the state average, indicating areas in need of significant improvement.

    Staff Ratings

Meadowbrook Rehabilitation Hospital is a prominent nursing home that boasts a respectable 4-star staff rating, situating itself amongst the superior facilities. When considering nursing aid staffing hours, Meadowbrook places within the top 30% of facilities, providing 2.47 hours per resident per day, albeit marginally less than the state's standard of 2.53 hours. However, the nursing home's LPN staffing hours fall into the bottom 20% of facilities with only 0.6 hours as compared to the state's 0.66 hours. Meadowbrook also champions exceptional RN staffing, finding itself in the top 30% of facilities with an impressive 0.83 hours per resident per day. This is ahead of the state's RN standard of 0.71 hours. In terms of licensed staffing hours, the facility falls slightly lower in the rankings, being positioned in the bottom half of facilities, despite offering 1.44 hours which is still greater than the state's expectations of 1.38 hours. Lastly, Meadowbrook's physical therapist staffing is placed in the bottom 40%, providing a mere 0.04 hours which falls slightly short of the state's average of 0.05 hours.

    Covid Data

The Meadowbrook Rehabilitation Hospital, a nursing home located in the state, is significantly behind in maintaining up-to-date vaccination rates for both its residents and staff members. The resident vaccination rate falls within the bottom 10% of facilities, marked at only 7.6% - a stark contrast to the state average of 62.4%. This figure puts the center severely beneath the state average. Similarly, the staff vaccination rate at this nursing home is disappointingly low at 4.4%, placing it in the bottom 20% of facilities in comparison to the state staff vaccination rate of 20.4%. These statistics clearly demonstrate that The Meadowbrook's vaccination rates are significantly wanting, putting it falling far behind most other facilities in the region.

    Quality Measures

The Meadowbrook Rehabilitation Hospital, a nursing home currently in operation, holds a concerning 2-star quality measure rating, making it the second lowest possible in classification. This ranking is potentially due to its relatively low performance metrics. Specifically, it falls into the bottom 40% of facilities statewide in relation to the percentage of residents whose ability to move independently has deteriorated, with a percentage of 18.54, notably higher than the state average of 16.88. The use of catheters also seems prevalent in the home, with 3.31% of its residents with a catheter inserted and unmoved from their bladder, placing it in the bottom 20% of statewide facilities and surpassing the state average of 2.35%. While 95.53% of its residents have received flu vaccination, surpassing the state's rate of 94.62%, Meadowbrook Rehabilitation Hospital still ranks in the bottom 40% in terms of vaccination. Unfortunately, the nursing home has a poor track record with falls, having 0.36% of its residents experience one or more falls with major injury, impressively lower than the statewide average of 4.88%, nevertheless, this places the nursing home in the top 10% for this negative statistic. Lastly, the home is in the bottom 10% statewide for the administration of antipsychotic medication, with 25.86% of residents having received it— a number considerably higher than the state average of 18.52%.

    Emergency and Hospitalization Quality Measures

Meadowbrook Rehabilitation Hospital, a nursing home facility, ranks in the lower 20% of facilities when it comes to hospitalizations per 1000 days, with their rate being 2.67, notably higher than the state average of 1.85. Similarly, it falls in the bottom 20% bracket concerning outpatient emergency department visits per 1000 days, registering 1.94, greater than the state's average of 1.36. Additionally, the home is in the bottom 30% for the percentage of short-stay residents who had outpatient emergency department visits, with 15.53% in contrast to the state's 12.03%. Also, when considering the percentage of short-stay residents who were rehospitalized after a nursing home admission, Meadowbrook rates higher at 29.22%, placing it once again in the lower 20% of facilities, compared to the state average of 21.86%.

I became a Patient at Meadowbrook Rehabilitation Hospital on Wednesday September 9, 2020 around 5:15pm.Today is Sunday September 20, at 7:05 am and I just finished arguing with a CNA named Tammy about my Shower.I am going home Tomorrow, Monday September 21, 2020 5:00pm.This has been the Extremely WORST CHALLENGE OF AN "ENTIRE  LIFETIME"!I wouldn't RECOMMEND MEADOWBROOK Rehabilitation HOSPITAL to A DOG.Please explain to me why I am arguing with a CNA NAMED TAMMY over "" A SHOWER""?Please NOTE:Why is my Neighbors BED PAN put in a SINK that I use to wash my hands and face? Why were my neighbors legs braces that go on her wheelchair put in front of the sink that I use to wash my face and hands? These are very, very heavy for me to try and move before I can access the sink. Why is her bedpan put in the "TRASH CAN that I have to USE"?This was put in the Trash Can for me to INHALE someone's else's URINATION!A plastic bag is never being UTILIZED!Why are the hand wash cloth wipes that I use to wash off with gone out of the RESTROOM and used for my NEIGHBOR AND NEVER REPLACED?June an LPN on SUNDAY SEPTEMBER 20, 2020 just informed me that my SHOWER days are,Wednesday and Saturday's!My Neighbor got a SHOWER On yesterday before Breakfast by Tammy and another person,Nobody said anything at all to me.Absolutely Nothing was Mentioned!LPN JUNE said that her PAPERWORK WAS RECORDED TO SHOW THAT I HAD REFUSED SERVICE!THIS IS A LIE.THIS PLACE IS NOT FIT FOR A DOG!My neighbor only gets CARE because her FAMILY is INVOLVED with her CARE!IF you have no one visiting you or seeing to your CARE PERSONALLY and VISITING,"YOU GET NO CARE AT ALL"!On Wednesday and Thursday I got absolutely no CARE because a CONFUSED MALE RESIDENT CAME into my room 3-Times.The fourth time he was trying to enter he was caught and told to go back to his own ROOM.My Bath-day/ Shower has always been on Wednesday's and Supposedly Now I hear Saturday's!So what happened to Saturday September 12, 2020. I was SKIPPED!On Wednesday September 16, I was promised a Shower from 6:00am until 3:00 at least 5 times.Because a RESIDENT DIED no other Resident's were rendered CARE ON the 100 side. I am literally astounded at some of the hate I see here for this place.   Don't believe it.   I CHOSE Meadowbrook after leaving another really bad SNF when recovering from brain surgery and re-learning how to walk again.   I wrote a $60k check out of pocket to come here specifically because I knew the moment I first wheeled into the place I was "home".   The staff were amazing.   Nurses were super attentive, pretty much NEVER had to wait more than 2-3 min for a call light to be answered.   NEVER got a bad attitude from any of them, even when I fell and peed myself and kept them there after their shift was over.   And the therapists are AMAZING.   Sylvia, Billy, Katy... all amazing people and I owe so much to them.   If you have a loved one that needs serious rehab (especially TBI cases) don't hesitate for a second to come here!

This has been a LONG time coming and I'm sorry for the delay.Meadowbrook Rehabilitation Hospital helped to save my lower leg ! ! !  On 30-Jan-2016, my 1993 Harley Davidson Classic Soft Tail Limited Edition Motorcycle was T-Boned by a PT-Cruiser; the driver was speeding (late for work) and texting (his boss) when the accident occurred. I received a fractured left fibula, compound fracture of the medial malleolus and the foot was dislocated from the leg; due to the nature of the injury (see picture) I was transported by ambulance as found with the boot on the foot. In the ER the boot was removed and they discovered 76klbs of force went through my heel and literally split my foot from heel to arch (not pictured, but I'll share it if you ask). I was encouraged to proceed with a below the knee amputation due to the long recovery time it might take "IF" I didn't lose the leg to gangrene - I was given a 5% chance of keeping the lower leg ! ! !After the top-notch care of the ER and surgeon staff, I was transported to Meadowbrook and for the next 3-months underwent daily care and therapy which I have no doubt is a reason that TODAY I HAVE MY LEG - I am still in recovery, but I owe everything to them for their diligence and care in the aid of my remaining whole. They were also key in helping to diagnose and begin care for the Traumatic Brain Injury I receive from the accidents impact.Thank You Dr. Meadowbrook Rehabilitation Center, YOU Are MY Hero ! ! !

I would not put my dog in this place. My daughter was in a bad accident last year. And she has been in this place for almost a year and I have had to call this place several times. She was getting bed sores, they were only giving her a bath twice a week, they were not cleaning her Colostomy bag. They were hanging her catheter bag so hight it was not working and she kept getting UTI'S. As far as I'm concerned this is not a very good place.

Do not send a family member to Meadowbrook rehab. Our 80 year old Mother was sent there for two weeks of physical therapy. The police were called to question patients because nurses were stealing medications from patients. The management there then tried to cover it up by falsely reporting that the patients refused to take medications home when checking out and that’s why the medication wasn’t sent home with them. When actually the medications were taken home by nurses there. There will be many lawsuits to hold Meadowbrook rehab accountable for their shady policies in my opinion. The one star I was forced to give is way to generous!!

My son Matt was transferred there after experiencing a traumatic brain injury and spent two months in NKC hospital and, the staff at Meadowbrook treated him terrible like an infant and, he's 26 years old and, they didn't even try to realize how serious his brain injury was they just kept him in a diaper and treated him badly and we're mostly very rude to us meaning his family when we came up too visit him and called to check in on him as well. They, has him in a dirty room which they barely cleaned up and had him shower not often and, scolded him every time he acted up too the point where we asked for him too finally be discharged from there because he didn't like it there and don't be fooled by the picture's on there website or on brochure because it's very deceiving and, most of the time was very unclean and, I just wouldn't recommend that rehabilitation center for traumatic brain injurinjuries too anyone because they're suppose too specialize in that field and, the most of the nurses, doctor's, and, staff don't do there expertise that they say that they are qualified for.

Huge Shout out to Greg and the Meadowbrook team!!! I have been a social worker in the field doing placements for 20 years. I need to say that this has been one of the easiest facilities to work with. Their communication and dedication to helping others is awe inspiring. They fought tooth and nail to make this placement happen knowing that he would not be a big money maker for their facility. It was clear from the beginning that they truly has the pt and families best interests in the forefront. If I get the opportunity to refer to their facility again, I know that it will be a great experience.
