    Overall Ratings

Cedar Valley Nsg & Rehab Ctr is an exceptional nursing home that boasts top rankings in several categories. With a 5-star overall rating, the pinnacle obtainable, the facility ranks in the top 10% of all nursing homes based on overall rating, surpassing the state average significantly. The prestigious ratings continue with a 5-star score in health inspections, once again placing the Cedar Valley Nsg & Rehab Ctr in the elite top 10% of all facilities in terms of the health inspection rating, far exceeding the state average. Not to be outdone, the home's quality rating is an impressive 4 stars, the second highest possible, securing the facility's spot in the top half of similar establishments. This quality rating is again notably higher than the state average, further illustrating Cedar Valley Nsg & Rehab Ctr's dedication to superior care.

    Staff Ratings

Cedar Valley Nursing and Rehabilitation Center is a nursing home that has some areas requiring significant improvement. It has been given a 2-star rating for staff performance, which is the second lowest rating possible. In terms of nursing aid staffing hours per resident per day, this facility is unfortunately placed in the bottom 10%, providing only 1.62 hours compared to the state average of 1.97 hours. The center is however within the top half of facilities for LPN staffing hours per resident per day, albeit still below the state average (0.91 against 1.03). Registered nurse staffing hours per resident per day are a considerable area of concern as the center sits in the bottom 20% of facilities, with a meager 0.33 hours as opposed to the state average of 0.45. Licensed staffing hours per resident per day are also considerably below average, falling within the bottom 30% facilities with an average of 1.24 hours against the state's 1.48. Notably, the center is in the lower half of facilities in terms of physical therapist staffing hours per resident per day, offering 0.05 hours which is on par with the state average.

    Covid Data

The Cedar Valley Nsg & Rehab Ctr ranks impressively in terms of the vaccination rate among its residents and staff. Being in the top 40% of all nursing home facilities, the resident vaccination rate is commendably higher than the state average, standing at 75.9% against the state's 59.9%. In parallel, the facility also boasts a superior rate of staff vaccinations. Ranking in the top half of facilities, Cedar Valley's staff vaccination stands at 20.6%, which although slight, still edges over the state average of 19.8%. The facility's commitment to the prevention of communicable diseases is evident in its high rate of vaccinations.

    Quality Measures

The Cedar Valley Nursing & Rehabilitation Center shines with a 4-star quality measure rating, placing it amongst the best in the healthcare sector. The facility is known for its exceptional care, evidenced by the fact that it falls within the top 40% of facilities with the least number of residents suffering from one or more falls leading to a major injury. Contrary to the state's rate of 3.4%, the center recorded a rate of a mere 2.18%, demonstrating its outstanding proactive measures. Furthermore, the Cedar Valley Nursing & Rehabilitation Center maintains an exceptional record in terms of vaccinations; it falls into the superior top 20% category of facilities delivering the pneumococcal vaccine, having successfully vaccinated 100% of its residents. This figure far surpasses the state's vaccination rate of 91.53%. However, the institution faces a challenge concerning residents' nutritional status: it remains in the bottom 20% of facilities grappling with residents losing too much weight, registering a 10.11% rate, which is higher than the state's rate of 6.52%. Regardless, Cedar Valley Nursing & Rehabilitation Center continues to strive for excellence in elderly care.

    Emergency and Hospitalization Quality Measures

Cedar Valley Nsg & Rehab Ctr proves itself to be a top-tier nursing home, providing superior care with metrics significantly better than state averages. Among facilities, it ranks in the top 10% for its low rate of hospitalizations per 1000 days, scoring 0.54, staggeringly lower than the state's average of 1.78. The facility also boasts being in the top 20% percentile for outpatient emergency department visits per 1000 days, at just 0.52 compared to the state's 1.16. Notably, Cedar Valley is in the top 30% in terms of its short-stay residents who've had outpatient emergency department visits, posting an 8.02 rate, lower than the state's 12.21. Furthermore, their rate of short-stay residents who were rehospitalized after a nursing home admission, which is only 21.19, places them in the top half of all facilities, undeniably an accomplishment considering the states average is 22.62. The Cedar Valley Nsg & Rehab Ctr indeed stands as a model of competent and high-quality nursing care.

Horrible. My Dad was admitted here for rehabilitation after he broke his leg. They refused him the meds the hospital prescribed him. He was also supposed to be on oxygen and they didn’t believe him when he told them there wasn’t any coming out until he put the tube in water and no bubbles came out. Several times he tried to get a nurse to help him move around while I was on the phone with him, the nurse button either got turned off or it didn’t work so he had to yell for help. The nurses finally came in after he yelled a few dozen times telling him not to yell or to be so hateful. Due to inadequate help, my Dad ended up going back to the ER. Either these were several unfortunate events that occurred within a couple of days, or this place is just horrible. I would not want any of my loved ones under “care” like this.

I have nothing good to say about this place. My mother was pretty much paralyzed in her arms & hands. She was sent to this nursing home for rehab. She had C3-5 Neck surgery but she had to be quarantined. They would not let anyone see her . Not even thru a window. I did have facetime video with her and she couldnt hold the tablet or feed herself. She was never checked in on. . Someone that cant move their arms need 24/7 care .
