Semi-Private Room Rates

Our semi-private room rates are $273.50/day and we would require 30 days in advance. This covers room and board only. Medications and supplies would be separate.

Overview of Professionals and Training

Our Medical Director is here every Tuesday. We have a Nurse Practitioner here every Mon, Wed, Thu and Fri. They are available after-hours and on weekends. We have Podiatry, Optometry and Dental services come in quarterly. We have Registered Nurses, Licensed Practical Nurses, Medication Aides and Certified Nursing Assistance. Physical Therapist, Occupational Therapist and Speech Therapist. All are trained according to state regulations and board certifications.

Safety and Well-being Measures

Safety and well-being of our residents is an individualized interdisciplinary process that varies from each resident.

Transition Resources and Support Services

Our Social Worker would provide resources and support services prior to discharge.

Transportation Services

Wheelchair transportation to local providers is offered.

Dining Experience

I highly recommend touring the facility to understand the experience fully. Residents are encouraged to dine in the dining room to be able to socialize with other residents but if chosen, may dine in their room.

Resident Complaints and Issues

We have a formal grievance process that is completed and given to our Administrator which is then investigated and replies to resident and/or family the outcome of the investigation.

Cost of Living

Semiprivate room and board is $273.50 per day.

Communication with Residents' Families

Initial care conference is scheduled within first 72 hours of admission and then conducted quarterly. If resident has a change of condition or any events family would be contacted at that time. Any additional requests regarding communication could be discussed during the initial care conference.

Short-term or Respite Stays

We do offer short-term and respite stay at the same rate as listed above.

Pet-Friendly Community

We are pet friendly as long as the pet is up to date on immunizations and is facility friendly. We do have a waiting list for long-term care beds.
