    Overall Ratings

The Westerly Health Center is a nursing home with a modest 2-star overall rating, placing it in the bottom 40% of facilities in terms of the overall rating. This score is notably lower than the state average, indicating a need for improvement in various aspects. Despite this, the Health Center has been able to excel in one area: health inspection. The facility finds itself sitting comfortably in the top half of nursing homes in terms of its health inspection rating, which is marginally higher than the state average. However, the Westerly Health Center faces a significant challenge with its quality rating. The facility once again only received a 2-star rating, which has placed it in the bottom 20% of facilities in that category and below the state average, highlighting a significant area that requires immediate attention and improvement.

    Staff Ratings

Westerly Health Center, a nursing home, has an overall staff rating of 1-star, indicating the lowest possible evaluation. In context, it ranks in the bottom 10% of facilities for both nursing aid and LPN staffing hours per resident per day, providing 1.42 and 0.39 hours respectively, both of which are lower than the state averages of 2.35 and 0.46 hours. On the bright side, Westerly Health Center ranks in the top 30% of facilities for RN staffing hours per resident per day, with a reported statistic of 0.85 hours, which exceeds the state average of 0.8 hours. However, in terms of licensed staffing hours per resident per day, the nursing home is in the bottom 30% of facilities, providing 1.23 hours that is somewhat less than the state average of 1.26 hours. Furthermore, it's within the bottom 40% of facilities for physical therapist staffing hours per resident per day, offering only 0.03 hours, which is less than the state's 0.06 hours.

    Covid Data

Westerly Health Center, a nursing home facility, currently ranks in the lower half of all establishments based on the percentage of residents and staff up-to-date with their vaccinations. With resident vaccination rates at a modest 61.5%, the center's averages fall below the state figures, which currently stand at a markedly higher 68%. This disappointing performance places the facility in the bottom 50% of comparable institutions. The staff vaccination rate is even more concerning. The 8.5% rate is alarmingly lower than the state's average staff vaccination rate of 36.7%, relegating Westerly Health Center to the lowest 40% of facilities. These figures emphasize an urgent need for improvement in Westerly Health Center's vaccination policies and processes.

    Quality Measures

The Westerly Health Center, a nursing home, unfortunately holds a below-average 2-star quality measure rating, making it a facility that requires improvements. This rating is particularly reflective in their record of patient health. They rank in the bottom 10% of facilities for major injury falls, with a percentage of 7.79% of residents experiencing one or more such falls, as compared to the state average of 3.88%. Additionally, health complications like ulcers are more prevalent among the residents with a rate of 7.91%, higher than the state's rate of 7.48%, placing the Center in the bottom half of facilities in this regard. However, it is not all bleak. Westerly Health Center ranks in the top half of facilities when it comes to addressing certain health concerns, such as weight loss, with only 4.87% of its residents losing too much weight — an outcome significantly better than the state average of 5.76%. Furthermore, they show a commendable commitment to preventative care as seen by their high flu vaccination rate. A impressive 98.14% of their residents have been vaccinated, surpassing the state average of 95.67%. Despite setbacks, it is clear that Westerly Health Center outperforms in certain areas and demonstrates potential for improvement overall.

    Emergency and Hospitalization Quality Measures

The Westerly Health Center, a nursing home, has some concerning metrics in relation to patient hospitalizations and visits to outpatient emergency departments. This facility ranks in the bottom 10% compared to other nursing homes in terms of hospitalizations per 1000 days, with a rate of 3.44. This is significantly higher than the state's average of 1.36. Similarly, Westerly Health Center is in the bottom 20% for outpatient emergency department visits per 1000 days, with a rate of 1.87, almost double compared to the state's average of 0.99. However, this nursing home displays a slightly better performance in the category of short-stay residents having outpatient emergency department visits. With a percentage of 10.83%, it is in the upper half of all facilities, but still falls short of the state average which is 14.58%. Furthermore, Westerly Health Center is in the bottom 10% in terms of the percentage of short stay residents who are rehospitalized after a nursing home admission. Its percentage is 38.94, which is a staggering 15% higher than the state's figure of 23.95%. This suggests a need for considerable improvement to match statewide standards.

I just finished a stay for a little more than two months at the Westerly Health Center. I must say this place was amazing well organized professional caring, communication was incredible follow up you name of this place has it going on. Everyone was happy every single moment everyone treat me with respect and compassion and they made me feel like I was part of the family. If you are in need of rehab or if you have an elderly person or parents that need to stay at a nursing facility this is the best place that I’ve seen ever. I’ve spent two other rehabs at other we have facilities and they don’t even come close to how great this place is . My room was kept very clean every day they wipe down every knob every handle swept mopped and disinfected everything the room was immaculate so wasn’t a place in the hallways in the cafeteria everywhere the food and beverage department did amazing job they were like clockwork delivering the beverages first and then some came a hot meal every single time the food was amazing the staff was very very accommodating so wasn’t the chef . Can’t say enough about the nurses every single nurse that I encountered took credible care of me and they were incredibly knowledgeable caring professional lots of experience couldn’t ask for a better staff administrative office often help me out with things paperwork printing mailing they were so accommodating and friendly they welcomed me in I wanna thank Sherry, Nancy, Devon, Alisa, Dominique, Carol, Rachel, Dave, Eileen, Crystal, Carrie, Christine, Jen, Shannon, Nicole, Prudence, Kathy, Eryka, Jordan, Kathleen, Donna, Stephany, Cathy, Eileen, Scott, Danielle, Tara, Talia, Doctor Sara,Rosemary, Chris, from maintenance was also amazing and his team was amazing, if there was a name that I missed I do apologize there’s so many great people there I couldn’t catch all the names or remember everyone. My experience with my sweet mom that had dementia. Entered this facility 2 1/2 years ago. From the day she enter the facility I found there was lack of care and compassion. Mom suffered broken teeth, broken nose, numerous bruises and hospital visits that could’ve been prevented if people were watching her...The infection that I brought to their attention two weeks ago was probably an ongoing UTI that was let go far too long. By the time she was taken to the hospital by my request she also had pneumonia. Uncompassionate administrators, social workers. The new floor nurses seemed to care , but the head staff not soThey cleared her room out without contacting me on Saturday , day she died. Obviouslyi was at the hospital with her. I went in the next day Sunday, ( yesterday) to start getting her possessions. Walked into an empty room. The nurses said the paperwork work noted the family gave permission to empty the room. I am the only family she had.I asked for the written consent. There is None. When interviewing with this faculty to get your loved one in you’ll feel very welcomed and be shown the best of environments. Upstairs in dementia unit was dirty, residents in their rooms were mostly unattended. I found my mom sitting in feces and unwashed many times in her room aloneGo somewhere else if you care for your loved one 😥I have attached pictures on this review of mom’s injuries

My mom was a resident there. She turned 95 and the next night she had an unsupervised fall. She was legally blind with dementia. A nurse called me and said she had a fall so they were sending her to the hospital. It didn't sound serious. I was without a car and living in Warwick. The next morning when I arrived I was shocked on how I found my mom. On top of all this she was on cumidine and her count was 7 times higher than normal. Neglect was what the nurse told me. She died from injuries from the fall a week later.
