We are proud to be members of three major post-acute care networks: Trinity Health of New England - SOHO Health Integrated Network, the Hartford HealthCare Integrated Health Network and the UConn Health Preferred Provider Network. By partnering with both systems and physician medical staff, we are able to provide high-quality care to our mutual patients.

We offer a variety of programs that are specially designed to meet the individual needs and goals of each of our short-term patients and long-term care residents. Our signature Congestive Heart Failure (CHF) Recovery Program includes collaboration with medical, physical, educational and psychosocial supports to return people to their homes and reduce and/or eliminate re-hospitalization. Our Touchpoints Rehabilitation Program is designed for those who may require a post-acute stay for orthopedic, surgical or medical recovery, intravenous (IV) therapy, wound care, hospice services and pain management.

I went to Touchpoints at Farmington for physical therapy on my upper spine after trying several other providers . I was immediately impressed with the knowledge of my therapist ( Jessica ) and her ability to zero in on my particular condition . She carefully reviewed my doctor's report and discussed her specialized approach to assist me . The resulting therapy has resulted in significant improvement in both my pain and mobility. Over my weeks of therapy she continuous and carefully assessed my improvement and communicated that to me . I am completely pleased with Touchpoints and it's P.T. unit as well as all of its staff that I experienced .  I strongly recommend this facility with hesitation   Al  L. , patient

Disgusting place.  Staff is very lazy. My father was there in 2013 and had bed sores, and was not properly cleaned. The rooms were not cleaned and on one occasion I noticed human feces on the wall in the bathroom. I believe that the staff keeps the residents heavily drugged in order to get away with doing minimal work.  The PT/OT staff is not very competent in my opinion. Do not send your loved one here.  Horrible!

My mom has been a resident since she suffered a brain aneurism in 2015 and as much as she complains about being “locked up “ as she calls it I really can’t complain about the care she’s been giving and in the end that’s all that matters in my opinion

DO NOT SEND YOUR LOVED ONE HERE!!!!! This place did not call us to let my family know my Aunt passed back in April from Covid!!!!! Anytime we tried to visit they said they were on lock down because of covid or she wasn’t in her room!!!!! They neglected her and left her to pass away!!! Shame on them!!! This is NOT over!!

This place is horrible do not send your loved ones here.My friend was there for rehab they ignored her crying for help I would call the front desk to try to get help very rude uncaring staff.She ended up falling broke her hip who knows how long she was on the floor they always kept the door to her room closed I have pictures to prove it staff never called to let us know She fell it wasn't until uconn called we knew she was hurt and needed surgery

Just returned from visiting a family friend. This place is filthy.Sat in one of the lounge rooms, and there was a large spot of dried blood on the floor.Staff, including nurses aids, and nursing staff were rude, and very unprofessional. Patients rooms were dirty, dark, and overcrowded.The client i was visiting needed to use the bathroom and there call bell just went unanswered. When i went to the nurses station to ask for help the nurse told me she would find someone to help them. , another 30 minutes went by and i finally saw the Director of Nursing (Janet H)spoke to her about my issue and her response was ;I dont know whose here so i dont know who can help you :Really you dont know whose here.!A grievience report will surely be sent to the state, and this place needs to be inspected by the Health department.On a good note, i did find the kitchen staff polite and very helpful when requesting a drink for my friend.

My father has been in this facility for a month or so. His care is pathetic. He waits over an hour to be changed. He is never hydrated. His call bell is always attached to the wall never in his bed. His dresser drawer that contains his toothpaste and personnel care items is used as a waste basket. The facility is dirty. Nursing staff does as little as possible and are rude. If this is a five star facility, I would hate to see s one star.

If care anything at all about your loved ones you will NOT send them here! This place is very dirty and smells like dirty diapers. It’s at least 90 degrees in that place and is a cesspool of germs. The staff has no bedside manner for the patients nor the family. They do NOT care about the patients. My father went over a week without having his hair washed. They did not give him anything to drink for over 24 hours. Also if your family member is bleeding don’t expect them to come with a bandaid. I’m guessing you have to bring your own. This place needs to be investigated by the state and the board of health!!

Upon arrival from the hospital the whole place was dirty and at least 80 degrees everywhere and patients were screaming every two minutes. The first thing we heard from one of my grandmother's TWO roommates was "I can't live here anymore! I hate it here." Also, the space was the size of a small school bus with three beds stuffed in.We immediately wanted to transfer out but it took six hours for somebody to come to the room and tell us she was starting on the process. At which point we had already solved it ourselves with an outside facility and were about to leave.I asked three separate people if there was a fan we could bring into the room so she wouldn't pass out from the heat but I never heard anything back.2/4 of the handicapped parking spaces were not plowed and the outside of the building was visibly neglected and dirty.Absolutely under no circumstances bring a loved one to this facility. It will be one of the most stressful and upsetting experiences you will ever have. I wish more than anything I could give zero stars.

I would never place another loved one here again. This facility is dirty. The nurses use the bathroom without washing their hands. Attempted to care for.my loved one after this YUCK just disgusting. The nurses would not help My mom get dressed she did it all on her own...no one assisted her with showering again she did that on her own... My mom asked for tissue to blow her noise, she got a role of toilet paper. No check ins....6 hours would pass before someone came in. Dirty laundry remained on the floor for 4 days. The cleaning ppl cleaned around it. It took for days for someone to say "welcome to touch point" here's your welcoming packet. We fought for 3 days to have my mom transferred which we did the work the facility would not...we did their job for them!!! Never ever again!!!!!
