Went to see a friend shortly before lunch meal time and I was appalled on seeing how he was presented to visitors. Hair was greasy looking, but combed, teeth needed cleaning. Had not been shaven for a few days, face needed washing, no bathroom in room, no call light seen nor any water available within reach for resident. Thin 1 inch mattress laying on floor by bed incase pt fell out of bed. Was brought out in wheelchair with no leg rests so patient had to lift his legs up when wheeled by staff. Other residents had greasy hair and one women was in sitting way slumped off to her right side. This gal clearly needed repositioning. When lunch came around, all residents had 1/2 juice glass, bread sandwich that looked like melted cheese with some type of meat on it, 2 thin sliced of pickles. Don't know what else they were given because I left as lunch was being served. Only 2 staff members present, barely spoke english and was difficult to understand. Only positive comment I can make was that facility was clean. I would never, ever put my loved one in a place like this. No way this can be a 5 star facility. Very disgusted with visit. I had a guest with me who is also an RN that witnessed this scene incase facility denies this.
