Our Nursing and Care Services
We have Licensed Practical Nurses (LPN’S) 24 hours a day, Registered Nurses (RN’s) at least 12 hours a day, and Certified Nursing Assistants (CNA’s) 24 hours a day.
Safety Measures
We have a system in place that we call the “wander guard system”. For residents that do wander, they have bracelets placed on them that will set off our doors in the event they try to exit. We also have keypads on the doors for entering/exiting, as well as cameras on the exits that are monitored at our nurses' stations.
Post-Discharge Support
We offer home health services when patients are discharging back into the community. I myself also follow up with each resident that discharges to make sure they have everything they need and the transition is going well.
Transportation Services
Our facility has a transportation/scheduling coordinator and a company wheelchair van to get our residents to/from appointments. In addition, we have a contract with an outside company for transportation of our residents that require stretcher transportation.
Dining Experience
In regards to dining, we have two dining areas (one on each side of our building). We have lunch and dinner dining programs (we are currently implementing our breakfast program). Residents are encouraged to come out of their rooms for meals (if physically able). They are assisted by our staff during these times.
Grievance Policy
It is the policy of this facility that resident or family concerns/grievances occurring during the resident’s stay in the facility be responded to by the designated Social Service worker or responsible Department Head closest to the cause of the concern. Responses to resident/family shall be made as soon as possible and preferably immediately. Actions taken to resolve the complaint shall be made within a reasonable time frame from the time the grievance form was received. Most importantly, actions taken must include contacting the resident and/or family with an explanation of the steps we are going to take to resolve the complaint and to ensure their satisfaction.
Costs and Charges
The daily rate for our facility is $433 a day. There is an additional charge of $7 for incontinence care and $7 if a resident requires oxygen.
Medical Updates and Consent
Residents/families will be updated any time there is a change in medical condition or medication change. If a resident is able to make their own medical decision, consent will be gotten from the resident for us to be able to release information to their designated family members/friends. If the resident is NOT able to make their own medical decisions, the person that has been designated as their “responsible party” or “decision maker” will be automatically notified of any concerns or changes. In addition, our Social Work department schedules “care plan” meetings that include dietary, activities, therapy, and nurse management. During these meetings, any concerns are addressed and family/resident are updated on the progress in each area of their care. Lastly, we are in the process of rolling out an electronic medical record platform in which residents/families will be able to view their/their loved one’s medical information during their stay with us. This can be opted into at the time of admission.
Short Term and Respite Stays
We offer both short term therapy stays as well as respite stays. The daily rate for that is also $433 a day.
Pet Policy
We are a pet friendly community 😊. We don’t allow pets to stay in our facility, but they are more than welcome to visit. The only thing that we do ask is that they are up-to-date with their vaccines.
Vaccination Services
Our facility offers both covid and flu vaccinations here. If for some reason we don’t have them available during the resident’s stay, they are more than welcome to get them elsewhere.
Aging in Place
Yes, residents can age in place. We do offer an in-house palliative care program as well as in-house hospice services through Hospice of the Panhandle. For long term placement, we are on a waiting list that is about 6 months or so long.
