    An incident happened to my underage daughter on the sidewalk near OPAC. I called them to see if they had cameras outside. The receptionist told me that she would have them look at the footage and call me back if they found anything. Sweet! They called back and said that I need an officer to request the video footage. Since we filed a police report prior that day, I had the officer request the footage. Turns out the cameras weren't on (that's what they told the officer)... OR maybe they are hiding something and an employee was involved in this incident with my underaged child? Hmm. Really makes you think when people change their stories.

    We just had to call 911, to return my cousin back to the hospital because she was so badly neglected it borders on criminal. For the longest time all she could hear was loud music playing from the staff while she was left with a call light that did not work. They only heard her screams in-between songs. Please do not allow your loved ones to be transferred to this shameful place. Their just collecting a check and will not care for your loved ones. We have notified the police.

    Made man appointment to see my mother. They forgot and sent me all around the place. They had no clue what was going on. After about  a half hour, they sent me to the wrong place again.  I wonder what happens when someone isn't watching over them. Very disappointed and scared for my mother

    If I could give 1 star I would. I don't normally write reviews and have never written a yelp review before but I feel that people need to know about this dreadful facility and the appalling level of care being provided. My mom is there for rehab and ended up contracting covid and is getting abysmal care.  As of 1/5/2021 there are 28 residents and 9 staff infected. Staff are going back and forth between covid and non-covid residents which they are not supposed to do and is probably why it is spreading. They are understaffed. It is nearly impossible to get through to the facility and no one calls back. My mother was left overnight for over 12 hours in a soiled depends! She is bedridden. Her food is late and cold. She is supposed to be having an abdominal wound cleaned and bandages changed and they are not doing this. She tells me that caregivers yell at residents who are in distress, telling them to shut up. I've contacted the county Ombudsman and they are investigating the neglect that is occurring at Orchard, not just my mom but I'm sure all residents. I've also contacted Solano County's Congressman Mike Thompson to alert him to the situation at Orchard. The owners of this facility also own Hayward Gateway Rehabilitation that was being called a "death camp" back in April from all the covid deaths. They  evidently owe millions in back taxes in the city of Vacaville. indybay.org/newsitems/20…This facility is deplorable. As soon as my mother is symptom free and tests negative I'm pulling her out of this place. I wish I had seen the reviews of this place before she was transferred there from Kaiser. I would never willingly send her or anyone here. Kaiser should not be referring anyone to this nightmare of a facility.

    If I can give zero stars that would be my rating.  This facility has provided a low level of care, medication not administered correctly, non operable HVAC, and the patient not receiving meals or snacks. This facility should be shut down. My brother was a patient there for a week & half and the place was horrible.  He was rushed to the hospital under their care after two days of complaining of chest pains, vomiting, urine brown and was given OxyCodine , Acetaminophen on a empty stomach all in a three hour period. He later had to have his gallbladder removed.The heater went out in the building for the entire weekend of 11/20-11/22, it was freezing my brother room was next to the back door that the staff continuously kept open. I was given permission from the nurse to bring in a heating blanket, since the engineer didn't return to work until Monday. That evening when I brought in the blanket, I was told he couldn't have it from the nurse on the evening shift. There was no protocol in place to have someone fix the heater on the weekend. The residents had to suffer the entire weekend in 40 degrees temps at night. He was forced to sleep in his clothes due to the coldness.The staff could never get the transportation schedule right, On two occasions he went to dialysis, and had to wait 2 and half hours for pick up.  This resulted in him not receiving lunch, and not to mention not offered a bag lunch, nor snacks to take along to dialysis.  The Nurse Frankie was horrible, when I called to ask why my brother transportation was late, he replied we were wondering where he was.  The Nurse Renee, gave wrong information regarding my brother scheduled Clonidine weekly patch, it was determined the information was incorrectly charted, and she was unable to follow up with the nurse who charted that his medication was administered.  She could of been proactive and checked the date on the patch to ease our family concerns, and the patient concerns regarding the patch on the same day. Two days later, I received a response that it was not administered due to his blood pressure. That was clearly a lie, it should have been noted in the chart.  The nurses are clearly unconcerned about the patients well being.    The door bell didn't work, I informed the staff and when I returned the next day it was removed from the building structure. The food is horrible, and no snacks are given in the pm. Thank God for DoorDash because I had to order  soup  for him to eat. When I went to retrieve my brother belongings, they had thrown in other patients items. He did not share a room, nor requested his items to be  washed by the facility. I did his weekly washing. I did point out that these were not his items, or his size of clothing. Looks as if they just threw the items in bags and boxes without referring to the property sheet. I declined the plastic hangers, barbells, and plastic tub and hygiene  supplies.  If you are picking up your love ones belongings bring gloves as I did and go through each of the items.  I also did not appreciate the soil underwear that was placed in a tied bag that wasn't even my brothers. I reported Orchard Post Acute Care to the Ombudsman Program and CCL and completed the RCFE complaint form.  Contact info: Ombudsman Program 1-800-231-4024, CDPH  send your complaint form to cdph.ca.gov  We can not allow this abuse to go on with our love ones,  if we could take care of them we would have them in our warm homes.  We are trusting the facility to care for our love ones.

    My mother has been there 2 months it started with workers laughing at her because she needed help. 2nd incident they stole her drinks. Last incident she went to hospital we went to get her things today because she will not be going back there, her tablet is missing. Be very careful with your belongings the staff are THIEFS.

    My dad has been there for ten days.  No one returns calls.  When you go down there everybody tells you a different story.  Once we got a hold of his nurse she knew nothing about his condition, or his treatment.  She kept saying we had to call someone but would not give us a name. She was rude and giving tons of attitude Unfortunately my dad can't speak well so we have not idea how he is being treated.  But if they are treating him like they are treating us it's horrible.  Please spare your family member. Don't let them stay here.

    Needs a total renovation to start. Dirty, I was on a sheet with a hole behind my back and the top sheet was like tissue paper with a holey area (I have photos) for 8 days before they were changed. The nurses and physical therapist are great, but a lot of the CNA's could care less. You better have an hour after you press the call button if you need to use the bathroom.I also have evidence of bugs. After 2 days I was in tears. I couldn't get home fast enough. If you love your family never have them sent here. The menu is another thing. Tell the dietitian your likes and dislikes and it doesn't matter.

    My mother came here yesterday - transferred from the hospital. Facility is a little bit beat up. But she is in a fairly spacious double room. The staff here are all exceptional! Incredibly helpful and responsive. The RNs and CNA's really seem to care about the patients and are happy and cheerful. The support staff is very responsive. The PT'sOT's and speech therapist all really seem to know what they're doing. They have a wonderful woman named Brenda who leads a program in the dining room. Everybody does simple exercises with her and then she leads a fascinating discussion about current and past history. I have a post grad degree and learned a lot. She has such a friendly, enthusiastic and cheerful attitude! Are nurse Kitty is awesome! The level of care she provides  is unbelievable.

    My husband has been at Orchard Care Home for the past couple weeks.  The staff has been great to him and for his care.   Since day 1 they have been responsive to his needs, helping him learn to walk, talk, dress himself, etc.   They provided a special bed mattress that helps with bed sores that he received at a different facility.   They listen to him and he is part of discussing his rehab care.   My husband was in the ICU for 14 days and then in a cardiac recovery room for an additional 7 days before coming here.   He has to learn everything all over again. It is like having a 54 yr old toddler.   Everyone at the facility checks on him, makes sure that we as his family are taken care of and keep us informed with his progress.When you walk in the facility the patients there seem happy and have life to them.   They are not all depressed and just sitting there.  They participate in activities and they love strolling the halls in their wheelchairs.I would recommend this place if you have a family member that needs that assistance care.

    My father was sent to this facility by Kaiser.  The nurse assigned to my father said that she had 20 patients to care for a shift. My father hit his call button to get help to use the restroom and no one came. Once he soiled himself it took about an hour to get him changed. He tried to get out of his bed on Thanksgiving to use the restroom and fell on his knees and the facility told us that he fell on his butt, but the roommate told us it was his knees. We were told to bring him clothes and to put his name on everything we brought so that laundry could be brought back to his room and 90% of his clothes are missing! We still have not received them.  The facility sent my father back to kaiser twice one for a fever which they gave him Tylenol before sending him so by the time he got to Kaiser he had no fever and Kaiser sent him back and I don't remember what the other time was for. I tried to get my dad transferred to another facility and Graylyn (kaisers liaison) told us that he was already doing his physical therapy and was not sure they could get him into another facility. My dad went back into kaiser for an infection in his leg and when they were getting ready to discharge him I told them he was not going back to that shit hole and they found a much nicer place in Napa. I wouldn't send my dog to Orchard! Every time I asked to have someone call me regarding different things no one returned calls

    There is no rating low enough. My friend was transferred here by Kaiser. She was put in a room with three beds, and still could hear nothing but loud voices coming down the hall on the first night.  Swearing, screaming, uncontrolled people all around her. She got extremely fearful of spending any more time than she had to, and ended up having her son come get her on that first night.  This place needs alot more help than the prior reviews intimate they already received in 2016. NO ONE SHOULD HAVE TO STAY IN THAT ENVIRONMENT , PERIOD.

    Overall most of the services here have been outstanding. They lost stars with me based on the medication piece. I came here for rehab after bilateral knee replacement . I arrived on Saturday afternoon and was placed in a room with two other noisy residents. As well my bed did not work, it wouldn't go up or down and my call button didn't work. In addition, I did not receive my first dosage of pain meds and other parts of my prescriptions til well after 10pm.... I was told they were waiting on the in-house pharmacy. That Monday I was able to speak with administration and was able to be moved to a much quieter room. They fell drastically off again when they let my pain meds completely run out BEFORE ordering. I had to send my husband to my regular HMO pharmacy the following morning to get my refill. Again I missed 3 doses of pain meds that I am prescribed every 4 hrs. On the flip side the CNA staff have been excellent. Physical therapy is a beast, but they too have been professional and polite. That's my rant, I pray your family member has a good stay.

    This place makes me ill when I hear the name. Why the state has not shut them down... I have no idea. A family member was transferred to this skilled nursing from the hospital for rehab. The family member was left in their feces and urine and neglected for over 8 hours without any nursing assistance or medication. The bed had no sheets or pillows and the family member was laying on a bare bed. The family member said, "It feels like I just have entered hell."

    Orchard Post Acute care made a lot of changes this year. They got a new administrator in January 2016. This place now clean all the time, the nurse are top notch and the CNAs great they actually care for every patient needs. They catering to the patient is they don't care what's for the meals, they will something that you like.

    This place is the worst. The nurses range between apathetic and downright abusive. The first 24 hours they left my sister in her own waste three times. This has gone on consistently since she has been there. They have repeatedly pushed her and thrown her legs about. They are not providing adequate rehabilitation. I called the State Department and lodge a complaint and they were at the facility! Someone had already called them.I'm going to call and lodge another complaint. Avoid this place at all cost. It's terrible.

    For those of you who need to get a positive attitude back on life, I was down flat on my back and came to Orchard Post Acute and now I can walk and I have a more sincere look on my life. For those of you who are ailing and sick this is the place to come. I could never ever ask for a better treatment. The staff were wonderful, the care was amazing, the facility was very entertaining and helpful. I was treated great. I went back home with a very positive attitude. Thank you so much to everyone in Orchard.

    My mother had to stay there for a 2.5 weeks . While she was there she ended up with bed sores really bad from staff not giving her care . Letting her sit in her own pee/ Etc. They have her take a bunch of pain killer even tho she has stage 4 kidney which is bad . Over all here my opinion, This place has been the worst place since I could remember I have. Been in Vacaville all my life. People don't get better there, they get worst there .. to the staff do us all a favor and quite if you don't like to help people. If you are one of the really few who like to help I feel bad for you .. maybe if you fire all the bad people and management!! it would be a better place .. find people who just isn't there for a check ... but want to HELP. PEOPLE..Ps we will never go there again and the review star are wrong trust me this place shouldn't have 1 star.The one on Nut Tree is very better but hard to get in ..

    A family member was in Orchard Post Acute Care for two weeks. During that time he received excellent care. All Shifts gave personal attention to his needs. The physical therapy department aided his strength and gave him enough help where he was able to function at some degree on his own. Whenever I visited my family member I was treated with respect and was always offered a beverage. Some even remembered my name which was a plus. The entire staff was always Pleasant and willing to help with any questions or need. I never worried about his care. I felt comfortable with him being there knowing that he was being properly cared for. I highly recommend this facility because the staff is professional and willing to help all of their patients meet the best of recovery.Mrs. Allen
