    Oak Hills Terrace is a premier Memory Care community in the Tyler, Texas area that serves all of Smith County. Our residents come from all over Texas and beyond. No matter where you’ve lived before, we’d be proud to have you call Oak Hills Terrace home.
    The area offers residents and visitors an unending list of places to go and see. For friends and family that are visiting, the Azalea Residential Historic District is a must-see. Stroll through the district and view stunning gardens and historic homes. The Discovery Science Place and Historic Aviation Memorial Museum are great places for kids. We also have award-winning wineries and delicious casual dining in the Tyler area.
    We take great pride in delivering high-quality care that is compassionate, safe, and resident-centered. Our services are designed to meet our residents’ physical, emotional and spiritual needs 24 hours a day, 7 days a week.
    Before a resident moves into the community, customized assessments are completed so individual care plans can be created. These care plans are updated biannually or earlier if needed. Care plans are overseen by the Executive Director, staff, Resident Care Coordinators, and RN’s. In addition to the Medical Director and highly trained nursing staff, our community is staffed by a Certified Activity Director, Certified Assisted Living Manager, and Certified Food Managers, and medical technicians.
