Our Staff and Training
On staff here we have RNs and LVNs - those are the nurses who provide direct care and manage the nursing staff, and CNS who assist the nurses in providing care for activities of daily living. We have occupational therapists, physical therapists, and speech therapists who provide therapy for all these disciplines. We have social services, activity staff, administrative staff, dietary staff, and housekeeping staff. As far as the training, each profession that requires a degree has such, and our staff has ongoing training monthly.
Safety Measures
The measures in place for the safety of residents are of course fully trained staff to monitor care and plans in place for any emergency that could arise.
Transportation Services
We offer transportation with our onsite van and are contracted with local transportation companies to assure residents have appropriate transportation.
Dining Experience
The dining experience is residents may choose to eat in their rooms, or eat in the dining room, which we often encourage. The idea is to feel like a home or restaurant and not like a cafeteria. We provide several options for meals 2 full meal/hot options and several al l cart options including hot and cold sandwiches, soups, salads among other things. We also offer an appetizer program, before lunch each day we serve a unique appetizer for the residents to enjoy and also to encourage communal dining for socialization. The appetizer is only served in the dining room. Some appetizer items include chips and guacamole, fruit salad, fried okra, corn dogs, and monkey pie to name a few. It is generally a big hit.
Addressing Complaints and Concerns
If a resident or family has a complaint or concern, they need addressed, any member of the management staff can take the complaint and get it to the proper department to address it. It is the goal to get the concern addressed within 72 hours as appropriate. Follow up is made with family/resident to inform of how concern has been addressed.
Costs and Payments
For private pay, a semi-private room is $218.50 per day and a private room is $310.50 per day, this covers room and board, therapy services are an additional cost.
CarePlan Meetings
Every 3 months each resident has a CarePlan meeting with the management staff and direct caregivers to assess how each resident is doing and discuss any changes/plans/goals with resident and families. Each resident also has a staff "ambassador" who checks on them daily and reaches out to family approximately once a month. Any time there is a change of medication, change of condition or incident the emergency contact person is called and notified.
Rehab and Respite Care
We offer short term rehab care, that is most likely covered by one's insurance. We offer respite care that can be covered through VA benefits, Hospice benefits, Medicaid benefits or private pay. Our private pay amount listed above is the same regardless of type of stay. Additional costs do accrue based on additional services, you may call for specific price of services.
Pet Policy
Pets are always welcome to visit their loved ones at North Park. We do not allow pets to live at North Park. Pets must be up to date on shots and must be held, caged or leashed in common areas as some of our residents may not wish to visit with certain animals.
Vaccination Program
We do provide Covid, Pneumonia, and Flu vaccines to those who are eligible.
Our Environment
Our environment provides short term and long-term care so residents are able to age in place when choosing to do so. We currently do not have a waiting list at our facility, except for our secure unit. We only have 1 male bed open on that hall at this time.

Review Summary
The following is an AI generated summary of the reviews of the community from across the web.
North Park Rehab Facility and Health and Rehabilitation Center have received numerous positive reviews from residents and their families. Many have praised the kind, caring, and hardworking staff, highlighting their attentiveness and the high level of care provided. The facility, despite its older appearance, is noted for its cleanliness and welcoming atmosphere. Therapists, nurses, aides, and administrative staff are frequently mentioned for their professionalism, compassion, and dedication to both residents and their families.
Several reviews commend the facility for its effective communication, with staff keeping families informed about their loved ones' status and care plans. The rehab services are particularly lauded for helping residents regain quality of life, with specific staff members being recognized for their contributions. The environment is described as friendly and supportive, with a focus on engaging not only the patients but their families as well.
However, one review strongly advises against the facility, citing concerns over staffing ratios, cleanliness, and overall care quality. This contrasts sharply with the otherwise positive feedback, which includes appreciation for the facility's efforts in rehabilitation, the professional and cheerful demeanor of the staff, and the quality of life improvements noted by residents and their families.
Overall, North Park is recommended by many for its exceptional care, supportive environment, and dedicated staff. While there is a mention of potential areas for improvement and one significantly negative review, the majority of feedback is overwhelmingly positive, with families expressing gratitude for the care their loved ones received.
