Community Leadership and Care Team Structure
The community is led by an executive director.
The care team is led by an RN, known as the Health and Wellness Coordinator.
Under the Health and Wellness Coordinator is the Health Service Director, a position intended for a nurse, currently filled by a nursing student with extensive community experience.
Two Resident Care Coordinators, who are Med Techs and CAN’s, report to the Health Service Director.
Below them, we have CAN’s.
All mentioned personnel are licensed.
Resident Care and Programs
We check on every Assisted Living resident in the morning, with follow-ups based on their individual care plan. Our unique Roster Review program involves daily meetings to discuss residents' conditions and adjust care as needed.
Assisted Living residents receive additional support for community integration.
All residents undergo a thorough orientation and are paired with a resident “buddy” for further guidance.
Services and Amenities
We offer transportation for various outings, including grocery shopping, doctor appointments, and leisure activities.
Our dining room operates in a restaurant style, open from 7:30 am to 6:30 pm.
Residents can file complaints through multiple channels, including directly to the Executive Director or the Ambudsman.
Communication and Policies
Communication with families varies based on the resident's living status, with ongoing contact maintained.
Respite stays are not currently offered due to space limitations.
We welcome cats, birds, and well-behaved small dogs.
Housing and Health Services
Some apartments have a waiting list, with studio apartments currently available and one and two-bedroom apartments expected soon.
Flu and COVID vaccinations are offered seasonally, alongside in-house physician and physical therapy services.
Community Goals
The overarching goal is to support residents in aging in place, recognizing that each community has its own care threshold.

Here at Cogir Mill Creek, we offer both Assisted and Independent living with a strong focus toward the care and comfort of our residents. We are a vibrant and active community with personalized care plans and an exciting activities program. Some of our amenities include transportation for shopping and appointments, concierge services, and all day restaurant style dining. We also have a great pool and fitness room.

- We are pleased to announce partnership between Cogir Senior Living and EmpowerMe Wellness a national leader in rehabilitation services. The focus of the onsite EmpowerMe program is to help residents maintain and regain independence to live a high-quality life. In collaboration with Cogir of Mill Creek, EmpowerMe will provide residents access to Physical Therapy, Occupational Therapy, and Speech/Vision Therapy services. Fitness and Wellness Services are also provided to help residents achieve their fitness, strengthening, and cognitive goals.
