    Staff Ratings

Grandview Rehabilitation and Healthcare Center, a nursing home, is unfortunately ranked among the lowest 30% of facilities concerning nursing aid staffing hours per resident per day, providing only 1.87 hours - lower than the state's average of 2.14 hours per day. However, when it comes to LPN staffing hours, Grandview excels, falling into the top 40% of facilities, supplying 1.02 hours per resident per day, which surpasses the state's average of 0.82 hours. Similarly, Grandview falls into the top half of establishments in respect of RN staffing hours, despite providing 0.58 hours per resident per day, less than the state average of 0.72 hours. With licensed staffing hours per resident per day, the center does well with 1.6 hours, putting it in the top 40% of facilities and beating the state's average of 1.55 hours. Nonetheless, it falls into the bottom 40% for physical therapist staffing hours, offering 0.03 hours, less than the state's average of 0.09 hours per day.

    Covid Data

The Grandview Rehabilitation And Healthcare Center is currently surpassing expectations in keeping its residents up to date with the latest vaccinations. According to recent statistics, the nursing home falls within the top 40% of facilities, boasting a resident vaccination rate of 76.3%, which is significantly higher than the state average of 68.5%. However, when it comes to staff vaccinations, Grandview Rehabilitation and Healthcare Center lags behind, falling into the bottom 10% of facilities. They have a staff vaccination rate of just 0.8%, which is drastically lower than the state average of 24.1%, creating an area that is in dire need of improvement.

    Quality Measures

Grandview Rehabilitation And Healthcare Center, a nursing home, has documented some concerning statistics regarding the health management of its residents. With only 73.48% of residents receiving flu vaccines, this facility falls in the bottom 10% compared to other nursing homes. This number is also substantially behind the state average of 92.71%. This trend continues in the area of weight management as 8.31% of residents have experienced substantial weight loss, exceeding the state's average of 7.02% and placing the facility in the lower 30% percentile. Also in the area of vaccinations, only 40.53% of Grandview's residents have been given the pneumococcal vaccine, compared to 83.32% statewide, placing it again in the bottom 10%. However, in relation to catheter use, Grandview enters the top 40% of facilities. Even though its rate of 0.5% is beneath the state average of 1.22%, it demonstrates that this care practice is used more sparingly than at other places.

    Emergency and Hospitalization Quality Measures

The Grandview Rehabilitation And Healthcare Center is falling behind other facilities in several key areas of patient care. It ranks in the lower 30% of nursing homes due to its higher rate of hospitalizations per 1000 days, tallying 2.16 compared to the state's lower average of 1.93. Furthermore, it falls within the bottom 10% of facilities regarding outpatient emergency department visits per 1000 days, scoring 2.61, significantly higher than the state's average of 0.96. This nursing home also has a high rate of short-stay residents who require outpatient emergency services, posting an alarming 20.98% as opposed to the state's 12.34%, positioning it in the lowest 10% of all facilities in this regard. Though it falls in the top half when comparing the percentage of short-stay residents rehospitalized after a nursing home admission – with its 22.4% figure less than the state's average 23.58% – it's evident that Grandview is facing challenges in its care standards.

Horrible service, the staff is horrible and unprofessional. There are maybe three staff members that actually show they care. The patients there are not being treated fairly and the social workers do not call back and show that they even care. I would never recommend this place.

If it wasn’t for the dedicated nurses and cnas this place would not be open. Nurses and cnas truly do care for residents. I would not advise anyone to apply to be an employee here unless you want be disrespected by Admin ,DNS and ADNS . Grandview does not value their employees. Pay is terrible. Administrator does not know how to run a building, changes a Director of nursing every year and no positive changes have been made. If you have a choice please send your loved one else where. Not enough staff to provide adequate care. Nurses and cnas are often over worked and working short. Cnas work 1:30 at times with no help. Nurses do not have support from supervisors or dns when in need of help putting their nursing license at risk.

Mom was sent here after a fall and is now a hospice patient. Advanced Medical Directive was given to them and they still asked if we wanted a hospice patient to have a flu shot. Unthinkable!! No reason to prolong Mom’s life when she is ill and bedriddenAlso billing is all screwed up. Paid for the room and board at $4750 and was not credited with paying. Now I had to find my canceled check as proofStay far away from this place. They are certainly not to be trusted with medical or paperwork / administration (billing) issuesSend your loved one someplace better. Do place them here.
