Rates: Fort Caroline Gardens as of January, 2023
Base Rates: Semi-private $3500 per month. Private $4500 per month
Base Rate includes: Medication assistance, housekeeping, regular wellbeing checks, assistance with activities of daily living, laundry, bedding, toilet paper, towels, three meals plus two snacks daily, access to all onsite activities.
Additional/optional services
- Nursing services: Additional $300 per month. Covers insulin assistance, compression stockings assistance, oxygen management.
- Respite care $175
- Transportation/supervision to outside appointments: $65
- Private telephone/line: $50 initial plus $15 per month
- Incontinence supplies: $175 per month
- Call Pendant: $50 initial plus $15 per month
- Bed rails: $75
- Bed alarm: $100 initial plus $10 per month
- Chair alarm: $70 initial plus $10 per month
- Thickener products (for swallowing difficulty) $125 per month
- Special Dietary Preparation: $30
- Nutritional drinks: 1 a day $45, $90 2 a day, $135 a day
- Community Fee: $2000 one-time fee for admission.
Please note the admission fee is required to place someone on the waiting list. Once a bed comes available, the admission fee will hold the empty bed for up to one week while paperwork is processed for an admission. If the admission is prolonged beyond 5 days, the client has the option of continuing to hold the empty bed at a rate of $75 per day. The admission fee is refundable only for medical reasons.

Fort Caroline Gardens is a 78 bed facility located in a nice and quiet residential neighborhood on Fort Caroline Road. Although we have many residents who are healthy, alert, and independent, our staff experience allows us to provide a home where clients can age in place for many years to come.
We have a team of staff experienced and trained in providing a high level of assistance with all activities of daily living and also caring for those with dementia and Alzheimer's. We have nurses on site seven days a week. We have a variety of room styles, including both private/semi private rooms, some with kitchenettes and private bathrooms.
