This nursing home is certified by Medicare and Medicaid and was first approved to provide Medicare and Medicaid services on 01/01/67 and is run by a for profit organization. It is a very large-sized nursing community with 229 nursing beds. Currently, there are 114 residents within the nursing community. Be advised these resident occupancy numbers are not always up to date and are collected from the Center for Medicare and Medicaid Services website periodically.Alerts from Centers for Medicare & Medicaid Services

The facility has also had 1 substantiated complaints reported in the last 3 years that resulted in a citation. The facility has had 1 facility incidents reported in the last 3 years that resulted in a citation.Overall Ratings
The Carolton Chronic & Conv Hospital is a nursing home that unfortunately falls into the lower segments when it comes to ratings. With a 1-star overall rating, the lowest possible, it ranks in the bottom 20% of all facilities based on overall assessment. This rating is also below the state average. The health inspection rating is relatively better, but still only at 2-stars, which is the second lowest possible grade. This places the facility in the bottom 40% of all nursing homes in terms of health inspection standing, still below the state average. Similarly, when it comes to quality rating, the nursing home is again within the bottom 40% of all facilities, and remains lower than the state's average.Staff Ratings
Carolton Chronic & Conv Hospital, a nursing home dedicated to long-term patient care, unfortunately holds a 1-star staff rating. This is the lowest possible achievement, indicating that the staff performance and patient care commitment are significantly below average. This rating raises concerns over the home's ability to provide residents with the proper care they deserve. While the hospital continues to operate, it is vitally important to address these shortfalls to improve the current condition and to ensure the welfare of its residents.Covid Data
The Carolton Chronic & Conv Hospital, a nursing home, ranks in the lowest 30% of facilities regarding the rate of residents who are up to date with the latest vaccinations. Specifically, the nursing home displays a subpar vaccination rate of 48.3%, significantly lower than the state average of 68.5%. Furthermore, the facility also falls in the bottom half for its staff vaccination rate, with only 12.0% of its employees up to date on their immunizations, compared with the state staff average of 24.1%. These figures reveal a daunting discrepancy in both resident and staff vaccination levels compared to the broader state averages, highlighting a significant area that requires urgent attention and improvement.Quality Measures
The Carolton Chronic & Conv Hospital is a nursing home that falls in the bottom 40% of facilities in two key areas of resident health. Firstly, it has a higher percentage of residents with a catheter inserted and left in their bladder (1.75%) compared to the state average (1.22%). Secondly, this nursing home has a significant percentage (7.44%) of residents who lose too much weight, a figure that again surpasses the state average (7.02%), placing it in the bottom 40% of facilities for this measure. When it comes to percentage of residents given the flu vaccine though, Carolton outperforms the state with a rate of 96.95% versus the state's 92.71%, although it still lands in the bottom half when compared with other facilities. This data reflects areas of concern and potential improvement for The Carolton Chronic & Conv Hospital.Emergency and Hospitalization Quality Measures
The Carolton Chronic & Conv Hospital, despite being within the lower 20% of facilities regarding hospitalizations per 1000 days with a rate of 2.78 (higher than the state's rate of 1.93), exhibits commendable performance in various areas. For example, it is recognized to be in the top 40% with respect to outpatient emergency department visits in 1000 days, delivering a rate of 0.69, a figure notably lower than the state average of 0.96. Significantly, the nursing home is in the top 20% when considering the percentage of short-stay residents who had outpatient emergency department visits. The statistics show a rate of 6.98, remarkably less than the state's percentage of 12.34. Nevertheless, the facility does face some challenges as it ranks in the bottom 30% of establishments in terms of the percentage of short-stay residents rehospitalized following a nursing home admission. With a rate of 26.07, slightly higher than the state's 23.58, there seems to be room for improvement in this aspect.Healthcare Nearby

Carolton will always be a part of me and my family. It’s been years since we moved to another state to be near our relatives, but in our hearts, we treasure the unconditional support and kindness the Tortoras extended to us. If they can embrace people they barely know like family, how much more the patients and residents who are entrusted to their care. Our heartfelt thank you to the Tortora family. May God bless you more.
My experience with the Carolton has been nothing but positive. I first went there for rehab after my first knee replacement in 2014. Then outpatient. And that outpatient staff is well educated, caring, and professional. I'm now going there after my second knee replacement. Wouldn't go anywhere else. And my insurance has covered with no problem.
Best rehab in the area by far that I have been to...and have been to 2 others plus Carolton. Their folks have a variety of educational rehab backgrounds and each do a variety of different rehab techniques and as a result, I was able to recover from hip replacement surgery up to 2 months earlier than my surgeon warned. Nice people who a dedicated and concerned about their patients.
On 12/13/15, I feel and fractured my hip. After a week in the hospital, I spent a month at The Carolton as a patient. After that, I went to outpatient therapy there until the end of May, 2016. I can't tell you how pleased I was with my inpatient and outpatient therapy. The staff was wonderful. Very caring and very supportive. If I ever had to have therapy again, it would definitely be at The Carolton. I made many new friends there. I can't thank them enough for all they did for me.
The Carolton helped my Dad get back on his feet after a fall. Both the inpatient and outpatient nurses, physical and occupational therapists were fantastic. My Dad couldn't have gotten better care. Once he was discharged, the outpatient services were incredible. Especially Brianne Hudak, PT extraordinaire, was instrumental in getting my father mobile again. Her work with my Dad right from the time he was admitted, was the best. Thank you Carolton!
I had a ruptured disc in my lower back years ago, and had sought treatment at various sports and traditional physical therapy practices over the years. During the past 12 months, however, my pain had been getting worse, and my usual exercises weren't cutting it. My primary care doctor referred me to the professionals at the Carolton Rehab. At the time, I thought that the Carolton Rehab only served the patients of the Carolton Hospital. I am pleased to say how wrong I was. The care I received at the Carolton Rehab has been life-changing. The therapists took a lot of time in diagnosing the specifics of my issues, gave me an in-depth understanding of the anatomical causes of my pain (because I asked for it!), and inspired me to make great strides in obtaining relief. They are highly educated, many holding advanced degrees and certificates. And, they care deeply about their patients. They have a keen intuition as to whether a gentle hug, a sense of humor, or a calm reassurance is needed, and they provide it all willingly, compassionately, and respectfully. Other staff members, including the receptionists, interns, and assistants, are also extremely welcoming, helpful and pleasant. I enjoyed every visit, and feel very confident in recommending them to family and friends.
Everyone was kind and considerate. I was very emotional when I arrived and everyone did there best to help me find the reason and work through it. At that time Q came and spoke with me and he offered to get me help and reassured me that everything was alright. The Physical therapy department worked with me as I had limitations on my therapy. The aides and housekeeping were so kind and cheerful and always ready to help. I did get to a couple of events sponsored my the Recreation Department and they were well attended and a lot of fun and really helped. I enjoyed the musical events also so well attended, There was not a day that went by that Carmen Tortora didn't pass by to make sure all was well and to say hello. I was there for 60 days which was a long time and always had good care.
If you value your loved ones' lives don't let them go here. No one is in charge and they pass the buck at every opportunity. Medicine is not delivered to patients in accordance with doctor's orders and they actually argued with us about this point for my father's medication. I complained to the owner and still haven't received a response.A nurse claimed my father's medication did not need to be administered at a particular time because "he has been taking it for so long that his body can absorb it at any time." Considering his doctor has never made that statement and continually stresses the importance of taking his medication on time and the fact that I work for a company that makes levodopa and carbidopa, I can tell you that that statement is absolutely incorrect. The efficacy of these pharmaceuticals, and the side affects, are absolutely affected by the timing of them. I find it shocking that an employee, a nurse no less, would be so misinformed on medical matters, especially when I'm sure some patients are relying on these medications to keep them alive. My father is 88 years old and every detail has the potential to affect someone of his age, hence the specific timing of medication as prescribed by his doctors. This not the first time my father has been at the Carolton, but I was very surprised that the staff were unaware of the medication he needs to take and the flippant attitude for when it is administered. I would expect an institution with the Carolton's history and prestige to be doing everything possible to remain in a positive light. Unfortunately, based on the care received there, I have to rate the Carolton very poor.
To the entire staff at the Carolton Chronic and Convalescent Hospital,We want to express our deepest gratitude for the professionalism and care you showed our beloved Sam Palmer as he transitioned. Over the years, the kindness and love you showed him did not go unnoticed. It was an absolute comfort having your team’s support even when we were not always able to physically be present. It brought us a sense of relief to know there were people who looked out for him as their own. We are forever grateful.Thank you and may God continue to bless you all. With love,The Family of Sam Palmer
First day had high Hope's for this facility to take care of my physical therapy needs. As the evening progresses I'm still without surgery pain medication waiting until8¹/²hrs later. Also on a high calorie diet heads up I'm going to bed hungry honestly very disappointed !January 1st 2021 nurse Anna who was taking care of my blood pressure on that evening and bedtime I asked where is my night time long lasting insulin she then replies we don't have it at 12:05 a.m. technically January 2nd she says doctor Miller did not put in the order just like my pain meds he never ordered it.Saturday January 9th 2021 I was told by Elaine they didn't have any of my morphine 15 mg medication from surgery they had a higher dose so if I was to take the higher dose I would be left in bed lethargic because it was just too much for me also would make me nauseous and throw up. Because of the higher dose I had to take because they didn't have the lower dose I technically could not do any physical therapy missing my appointment. I was In severe pain and discomfort 13 and a 1/2 hours later is when my medication finally came in, By then it took about 3 days to catch up on my Correct dosage to finally have pain management under control this was a huge setback for those 3 days Because it then set me back on physical therapy Again which this is why I'm here.Friday afternoon January 16th 2021 started feeling a lot of pain in my legs again started feeling like I wasn't on the right medication for my neuropathy I also realized that I had those little floatys in my right eye Now the last time this happened to me it was because I wasn't on the correct medication It's now Saturday late afternoon in my legs were in severe pain again my nurse at the time his name is Bernie I asked him Bernie am I on lyrica he went to check the nurses station to find out that I was not on lyrica I was on pregabalin So somehow my order was changed so we both went to the nurses station to find out it was actually the pharmacy who switched it over to pregabalin With out consulting with me the patient or the nurse on hand. Bernie immediately called the pharmacy to put in the correct order lyrica So I then waited again about 6 and a 1/2 to 7 hours for my lyrica to come in causing me lots of pain and great discomfort again. It seemed to be a crazy facility and chaotic every night with staff only to find out the staff was screaming in the hallway Lisa Lisa Lisa we're missing a patient did you see her after the confusion of nurses running down the Hall they finally located the patient that was missing who happen to be down the Hall in another Room confused! This place seems to have no oversight clearly in regards to all the mis medical mishaps and Fortunately finding the patient who was not in harm safely still within the facility North wing now I'm not sure how much this is on the regular in this facility but I can tell you in16 days I went for approximately a total of 4 to 5 days of severe pain and discomfort. Proper pain medication with management is essential for an amputated foot which is quite painful for anyone who unfortunately has had the surgery but hopefully Will not come to the Carlton for short term physical therapy. The physical therapist for ot and PT are phenomenal they work with you very closely and do not push you more then your body is capable off unfortunately it's the facility And the 1 particular aid Adleen who are not professional the nurses were fantastic especially Bernie and Jane and many others except for Dr Miller who did not over see or follow up on proper medications on myself and possibly many others who can't speak for themselves because they are elderly here for long term care as well. I will also say Lisa from dietary is the boss and does not follow up with trays to make sure everything is good with your meal or particular special diet with no quality check on trays that arrive some times taking to long to correct the problem or mistakes unfortunately.Personally I had to complain to the state about their blaiten disregard of medication!
CAUTION - don’t send your loved ones to this place unless you want to never be able to see them or even send them flowers!!! Our 96 year old, intermittently confused, grandmother is there and they won’t let anyone - not even one single family member - go visit because of COVID for at least two weeks (just give her a COVID rest and stop this nonsense!!!) and even then only outside in a tent, which she may not even be able to do since she is bedridden. I understand wanting to keep the other residents safe with everything going on, I’m in healthcare myself, but with Connecticut’s low numbers, not to mention the state thinks it’s safe enough to open schools and restaurants - I think it is absolutely INHUMANE to have her there with no visitors AT ALL at her age. Even ONE from our family could make a huge difference for her. Connecticut is not in crisis mode anymore and the Carolton is the most restrictive nursing home around. I tried to order flowers for her and local florists said they don’t deliver there because the Carolton insists the flowers sit outside all day before they can be brought in (which honestly makes no sense when you look at how long it stays on surfaces). This place doesn’t give an inch and we wish we had listened to warnings from friends about this place before we placed her there! Beware!!
As a vendor, we are proud to be part of The Carolton's comittment to providing their residents with the most sanitary linens possible. We observed warm and friendly staff and resident interactions both days we were there to provide service. How many places do you know where the longevity of employees is decades long? Says alot about the quality of ownership and management.
I've been through two surgeries and convalesced both times here. Very professional. The outpatient rehab, though , is REALLY EXCELLENT. An exceptional facility for outpatient physical therapy, better than any I have tried in the area, by far. Most PT places don't have the same level of staff training and competence, and other places tend to leave you sitting around waiting for them to finish with their other patients. Here you get one-on-one the whole time. Plus, they MEASURE results. I Highly recommend.
I was referred to the Carlton by my doctor who had gone to the Carlton for her rehabilitation. Although my surgical recovery was very painful, the facility staff were the best people I had ever met. I was so comfortable and secure while I was there for 10 days. I had the pleasure of meeting the owner who asked how I had been treated during my stay. I had to confess that he had very professional and pleasant staff who always had a smile on their face. The food was the best. Someone had decided that I needed a small snack every evening. (I think it was the woman who supervised the kitchen) I had the pleasure of meeting and speaking to her about my meals that were always good. Even the coffee was good. I could go on talking about the staff from maintenance, the aides, nurses and office staff. I even had my hair shampoo ed at the salon on site. Even physical therapy was a pleasant experience because of the people who worked there. In closing, the building and grounds were well maintained and very clean. They had a summer cook out for patients, families and staff. My family and I enjoyed every minute. The Carolton is a melting box of cultural diversity in its staff overall. The many ladies that took care of me felt like friends. The Carolton is a very nice facility because of the staff that work there and the owner who has a big voice, a bigger smile and nice hair, lol.
Carolton is the most wonderful rehabilitation facility! My father needed both in patient and out patient PT and OT there and the staff over delivered! From the nurses who cared for him for the 2 weeks he was there, the PT and OT therapists, the nurses and staff were all incredible. The Carolton is the ONLY place to go to if you need any kind of rehabilitation. The literally got my Dad back on his feet with kindness, smarts and compassion. Specifically Bri Hudak is the BEST PT therapist-- she cares deeply and is a total pro. I can't say enough good things about the Carolton!
