The Center provides both short-term and long-term care for patients recovering from an injury or illness. Our skilled nursing facility in San Diego is a state-of-the-art center that provides residents with the resources they need to achieve optimal health and functional independence. Our team of providers and professionals are devoted to helping each resident reach their full potential. While many patients recover from illness or injury, some require specialized care. Skilled nursing care is a type of health care provided by licensed professionals who have extensive training and experience. At Bella Vista Health Center, our skilled nursing staff provides individualized care that may include complex wound dressing, medication management, catheter care, IV therapy and other services to promote healing and comfort.

Memory Care
At Bella Vista Health Center, our Occupational Therapy programs are designed to help patients restore functional independence and improve their quality of life after an illness or injury. By providing customized treatment plans and working closely with each patient, we hope to make a lasting difference in their lives. Our skilled and compassionate Occupational Therapists also work with clients who suffer from Dementia and other types of memory loss. They help these individuals maximize their independence and quality of life.

Rehabilitation
Not too long ago, being hospitalized meant a lengthy stay with plenty of time for aftercare and recuperation. But now, modern hospital policies have shortened the duration of hospitalizations to just a few days. If you need extended care and more time to recover before going home, there are short-term rehabilitation programs at skilled nursing facilities. Skilled Nursing
A skilled nursing facility provides top-notch nursing care and rehabilitative services to individuals recovering from an illness, injury, or surgery. When patients are no longer in need of a hospital stay but not quite ready for independent living, their physician will usually refer them to a skilled nursing facility. We offer individualized programs that include physical, occupational, and/or speech therapy to help patients rehabilitate back to their personal best.

    Nursing homes and skilled facilities always give me a bad feeling. Bella vista proved me wrong. My father was there and connecting with him during his stay because of covid help reassure me that he was well taken care of. My father said the staff was amazing, I especially want to thank his day nurse shannon, every time I called and wanted updates on my father she took my call, no problem. My father raves about her saying she gave him the best care. Bella vista you have a gem amongst you. Thank you guys for being so wonderful during these trying times.

    There are not enough stars that would be deserving of this facility with the amazing care it provides. My elderly mother was in this facility for three months after a head and back injury. To say that she was cared for like family is truly not an understatement. My mother required 24/7 care in addition to physical and occupational therapy. Everyone in that facility from the front desk, to the manager, to the nurses and CNA's, therapists were invested in her progress and went out of their way to make sure that she was not only well taken care of, but that she was comfortable and happy.  During these times with Covid in addition to everything else, they go above and beyond to ensure patient and staff safety.  There are no words to say how appreciative I am for their incredible care.  A special shout out to Dolores, Dani, and Daniel who became like family and took great care of my mother's every need.  I am forever grateful for all of you!

    My name is Rob Halliburton and I have been to 3 other places do to my accident and have looked in to many places unfortunately to seek treatment. Trust me I am very skeptical of nursing care places especially with this virus going on. So I was sent here for care thinking that it was going to be like a corporate type place like all of the other ones were you are just a number, and know one cares about you. Even a slum lord place. But when I first entered the lobby I said wow this place is beautiful and I tell you the treatment and the staff just made me feel right at home it was an amazing feeling. They were caring and professional and extremely knowledgeable. And the management team they take a personnel interest in every patient. Cant forget the physical therapy department I tell you the best of the best. also the best kitchen O my God really the best food ever. I VOTE THEM THE NUMBER ONE NURSING AND HEALTH CARE FACILITY IN SAN DIEGO COUNTY.

    i was here for almost 2 months and from the first day they surprise me with a vase with flowers the nursing staff is awesome,the management is awesome when i was there ricardo, ivy and every single person in charge did even the impossible to have me as comfortable as i wanted to be, i gotta say that edy, luisa, traemon, aira, jessica, jennifer, pamela, kara, christine, francis, dora, karla, and justina are the most excellent nurses in bella vista, but also i gotta thank janett and her staff for keeping me entertain in their activities room, im specially thankful to my therapists: carmelita, philip, steve, bell and lee, thank you jeff and orlando for your help both of you rock, and last but not least raquel thank you very much for your help, to anyone that reads my review don't think twice in picking bella vista health center for your needs they will take care of you, i'm starting to walk thanks to god and bella vista if there was more stars i would give them 10.

    My mother in law just spent five weeks at this facility. She had been quite ill and weak when she got there. I am an RN so had an eagle eye out for just about everything. These were my observations: The facility is clean and well staffed. The care and personal attention she received was excellent. The Physical Therapists, Occupational Therapists, Nursing Staff and personal care attendants were professional and readily available. Administration acted quickly when there was a problem with a roommate, and they changed my mother in law's room in less than 24 hours. The food was very good, and though she has distinct lines and dislikes, she very much enjoyed the meals there. It was nice to be greeted by staff who knew us by our association with our  mom, and for those people to be so encouraging to her and to us about her progress. I would recommend this facility to anyone in need of this level of care.

    An excellent facility. The staff was extremely courteous,  professional,  caring, and understanding.  I was there for 3 weeks rehabbing my knee, the PT team was on task and supportive.  The food,  well it's hospital food. But they try hard.  The nurses are above and beyond to providing care and safety.  Thanks Bella Vista!

    I did a lot of research to get my mom into a good place for a leg break and this place had good reviews. After visiting facilities I decided on this one and I am so sorry I did. My poor mom had to manage her own health care which was extensive. They constantly ran out of her pain medication that she needed extremely bad so she was in severe pain more times than she should have been. Many things related to her healthcare and her general comfort just fell through the cracks and our family had to be heavily involved, micromanaging everything which was exhausting. The food was not good... On her first night there they gave her a nasty bologna sandwich and this was not the last time she received this for dinner. Another issue was the physical therapy dept who had her lean heavily on her "good leg" even though she kept telling them it was excruciating. She ended up doing significant damage to her knee because of what the physical therapist had her do. This has been further confirmed with her orthopedic surgeon and he said she should not have been commentating on that leg because her knee was so bad before she entered the facility.I could go on and on about all of the reasons why this facility sucked including constantly screaming patients etc. I'm so sorry I picked it for my poor mom. We finally just took her out and managed her care on our own.

    My mother was sent to Bella Vista Health Center after a hip replacement surgery, she was really exited about the fact that she was going to be having the attention she needed and wouldn't have to worry about anything herself, but the first two days in that place were unbelievable, the bed she got was broken it would not go up, she was not able to stand by herself, In order for her to be able to eat she had to be in a reclining position, but since the bed was broken, we had to find a way for her to eat while laying down, and that is just the start, the nurses show up whenever they feel like it, and not when the patient needs assistant, I understand sometimes nurses are busy taking care of other patients but I was present when my mom ask them for an extra pillow and the nurse came in 45 minutes after, and the funny part is i walked through the front desk where the nurses have their computers and they were all chit chatting eating like they were having a picnic, it's so unfair and sad that instead of patients feeling like they are in a good place for healing it feels like they are in a place asking for thing they don't deserve, I would never recommend this place to anyone.... so disappointed

    Some great nurses but my mom gets a shower  a week after being admitted. Today is her discharge day, paperwork isn't ready and no on knows she's leaving. Now they are scrambling to get their shit together while we wait. It took a week for people to bring her daily snack. Again they have some great nurses but they are extremely unorganized!!!!!

    My Dr.told me I dad to go to Bella Vista after my knee replacement surgery :(.I really didn't want to go I just wanted to go home after my surgery but I'm so glad I did go I got all the care and therapy I needed after my surgery .I was there for 3 weeks and I don't have no complains at all everyone there were so nice and helpful.

    I was a patient at Bella Vista for about 2-3 weeks. The physical therapy and occupational therapy people were very helpful in getting me to exercise my leg and knee, since I had 5 knee operations this year. The food was ok compared to the jospital food. No major problems related to my, just my care. My fiance would visit about 5 days a week to check on me snd my care. I ended up with renal failure and was apparently babbling when my fiance came to visit. None of the nurse picked up on the fact that I was running a fever and talking incoherently. My fiance had to yell at the nursing staff to get them to come check me out. I ended up at the hospital in the ICU for about a week. The only other problem I had was an accident that occurred in the physical therapy dept. when some things they had stacked against the wall fell on my leg cutting it slightly. They were quick to clean me and wrap up the wound, but nobody in charge came to me to ask what happened. I went there with glowing reviews from the surgeon, but it was really a joke.

    I would not recommend this place. They have bad skilled nurses some are delightful but the nurse my grandmother had wasn't so helpful. She would ask to go to the bathroom and the nurse would say No. When the whole point was for them to help her out. Due to her saying no my grandmother had to get up by her self when she can bearly even walk and she fell and got a big cut in her forehead and ended up in the hospital again. If I could rate it less then one I would im just letting this out so your loved ones won't get hurt or get less attention.

    I have been here at Bella vista as a patient for 4 days.   I am writing this review because the surgeon's office highly recommended it. Even with a glowing recommendation from the person who is going do surgery on you. One can never be certainWell unfortunately I feel compelled to write a review that will not be very favorable. But I feel strong enough to take time to tap it out on my cell while still there/here.  Overall the majority of the staff there is as nice as others have written about in their yelp reviews. However there is one particular team that works the 3pm to 11pm shift that turned what started as a simple smooth recovery from knee replacement surgery into a painful, stressful and a very upsetting experience.  What started for me as a simple mistake on a meal I was served I wanted corrected so I could eat and move on with my recovery and move forward turned into an issue.  As it turned out the gentleman in the bed next to me had a dinner issue as well.  Let me state that this point my issues began. Only because the lady he was speaking regarding his issue was rude and seemed to talk down to the gentleman. I believe it was because of his thick Spanish accent, then the employee, with her own thick Filipino accent, rudely responded after he advised of the issue with his food.  He asked for something else and she very curtly responded that that's what they served and after some dialogue the employee relented said she would check and left the room.  After witnessing that behavior from the employee I elected to speak to the charge nurse instead of the employees.  After 15 minutes I asked a staff member when the charge nurse was going to respond, she said she would check and hurried out of the room.  Stopped and spoke to another employee. That led me to believe that they hadn't honored my initial request. 15 minutes after that, the charge nurse showed up.  As to be expected, her posture and body language was very defensive, arms crossed facial expression, etc.  I explained my issue with my meal showed her the instruction sheet for the tray that specifically said "NO CARBS" then I showed her a photo I took and pointed out the main course that consisted of pasta and other things, a bread roll and some kind of greens.  I advised that every day since my arrival I have sent back most trays and asked for fruit or salad, yes I know most fruit has carbs, but I needed to eat.  I seized the opportunity and advised her why I had requested to talk to her instead of one of the ladies that was working in our area. I advised there were barriers I have personally experienced and/or witnessed such as communication, putting off patient requests, she only stated that she would elevate my issue to her boss, as if only to placate me and get me out of her face because she had more important things to do.  Upon noticing that I advised her that I understood her position because she's responsible for her employees but she also holds responsibility for the patients.  I also advised her that I would be speaking to the hospital administrator about the same. So based on this long review regarding this facility, specifically this team, I felt anyone considering to rehab or recover make a well informed decision.  Would I stay here again? Probably... well the other teams of caregivers have been great! Heaven help if this team is assigned to care for them. But no one is perfect.  To me that is a prime demonstration of folks becoming complacent and have forgotten that they elected their careers, and we the patients have entrusted them to properly care for us (patients).  But like many folks they've become tired with their jobs and seem to have stopped caring.  You know the saying one bad apple ruins the bushel. I think that fits perfectly for this situation.Before I checked out I also realized that these folks that I called complacent and said they seem not to care was being overwhelmed.  Like all folks they do attempt to explain doctors orders and do abide by them.  Patients even older ones throw tantrums when they don't get their way.  I'm referring to medical staff explaining dosages and times for medication, patient says he understands, at this point I knew his schedule better than my own, yet arguments at 3 am, 5 am and throughout the day would tak place DAILY!! That's my only true negative regarding my stay there!  I left the entire post on purpose and to help.

    Mom has been there for a while, some of the staff is really nice & friendly and about a third doesn't care at all, if a patient calls them there's no response, there's been so many times that one of us shows up and she hasn't been changed, she'd been calling them x over an hour to change her, they turn off the light & if we don't go get someone she would stay wet for another hour, sometimes it takes them up to 2 hrs to change her if we don't show up, she has a really bad rash due to being wet too long. She gets very upset that some of her personal belongings disappear, some of them we've been able to "find" after raising a stink & some not at all! Bathroom is not very clean and it's old, the food isn't good at all, I've seen it, frozen vegetables, white bread, fried stuff, etc. she hardly touches it, I wouldn't either! Her present physical therapist is knowledgeable & really good, some in the past have not. Facility is old, definitely understaffed and the outside needs landscaping!

    taking care of people who are sick and often elderly can be a challenge, but BELLA VISTA does a pretty good job. 1. for the most part the staff is smart, skilled and friendly. 2. the food is consistently good and the staff dietician goes over your preferences and needs to help dial it in to your specific needs. 3. they place is CLEAN and well maintained. of course because of the nature of the situation there may be times when you need to assert yourself to get what you need, but at the end of the day the staff and administration really do care for your well being and does their best to make you comfortable and content

    I came to this facility because I was hit by an automobile and needed rehab after my Hospital stay. I was there a little over a week. I am a vegetarian and at times difficult to feed LOL however after a few minor hiccups the kitchen and kitchen staff rallied and came to the rescue! The food was really good and well-balanced for proper nutrition, which was important to me and the kitchen staff had no problems with my special requests. The nursing staff was not only kind and quick to come when I rang my call Button but they were also very efficient and extremely helpful in all of my needs. My roommate has been a longtime resident for over a couple of years and seeing how they treated her with such respect , gentleness and kindness gave me even more admiration for the nursing staff. The physical therapy and occupational therapy Staff were equally as phenomenal , they never pushed harder than I wanted but challenged me in all of the ways I needed. My recovery could have easily have taken 20 days or longer but because everybody was on the same page from staff to Administration, I was able to go home in half that time! I was sent home with all of the appropriate tools I needed to succeed, including aftercare. The facility itself is also very well kept and clean they have many places for you to socialize and to relax outside of your room if you like to meander around. And they have lots of activities every single day and most evenings to keep you busy and your mind engaged. The activities staff is clever, kind and creative which ensures you will never be bored :-D I would highly recommend this place for anyone that is considering a Skilled Nursing Facility

    My sister left Balboa Hospital and needed to go to a skilled nursing facility to receive Physical and Occupational therapy.  Luckily, for her and her family, she ended up at Bella Vista!  We cannot say enough great things about the care she received at Bella Vista.  From the moment she was admitted and checked in to the day she was discharged we do not have one bad thing to say about this facility.  I want to make it clear that we are not being biased at all.  In the last 2 years my sister has been in four other skilled nursing facilities and they were all pretty much a nightmare.  We have a lot to compare against and Bella Vista truly deserves the 5 star rating they proudly display at the reception area.  The charge nurses, the CNA's, the LVN's, the housekeeping staff, the social workers, the activities director, the kitchen staff, and most importantly, Susan, the Administrator are top notch caring professionals.  The Administrator, Susan, is involved in every aspect of her residents care. She walks through the hallways and interacts with patients, guests, and staff.  She cares!  She is in tune with everything that goes on at Bella Vista.  The ratio of CNA's to residents is amazing.  My sister's stay at the other SNF's was a nightmare.  At several of the other places they would leave her in soiled diapers for up to 1 1/2 hours at times.  Every time we left her our hearts broke because we knew we were leaving her under the care of individuals who did not have her best interest at heart.  While my sister was at Bella Vista we never had the same feeling when we would leave her in the evening.  We knew she was in great hands.  She is a very complicated patient with lots of needs and she can be a handful, we acknowledge this 100%.  However, she never once complained.  She made excellent progress with her therapy and she is now home.  The day she left Bella Vista I actually cried.  We really got to know the staff and other residents and it was hard knowing we wouldn't see them again.  I have never felt the need to write a review on yelp, however, I had to take the time to write this.  I am grateful for my sisters stay at Bella Vista - it was a blessing.

    My wife & I came to visit my sister. So far, the place is clean and staff are friendly. I didn’t have a lot of interaction with a lot of the staff but overall I’m very satisfied. It’s just my sister’s second day in the facility. Thank you in advance for taking care of our sister.

    DO NOT COME HERE OR BRING YOUR LOVED ONE HERE. I can’t put into words how frustrated I am but I will try. We have had a horrible experience. I tried to be patient as I assume working and adjusting through a pandemic must be tough. But my father’s intermittent stay here was absolutely a disaster and has left the worst impression on me. Communication here is trash. No one returns calls, nurses seem either clueless or unconcerned. My father was rushed to the hospital multiple times since he was admitted here but this most recent time at some point the ER called this facility for basic information the response they received was “we dont know”. He has bedsores that have developed here, and all the progress and improvement done with a acute care facility that he was previously at, has been lost here in the span of a month. Im finally fed up and had him moved, notified the staff, left a message yesterday stating I was picking up his belongings today (no call back! as usual) I called over a hour before arriving as a reminder, get here and guess what, NOTHING HAS BEEN DONE!! Sitting here now for over 25 minutes waiting and no one knows how long this will be. But I guarantee if there was money or paperwork needed, their very rude billing or admissions assistant would definitely be calling my phone.

    Dreadful facility. Employees stole my mothers clothes an d possessions. Nothing was done. Not even an apology. She was left lying in bed with just a shirt and dirty diaper when she was sent for rehab to walk again. She left weaker than she went in. Took 12 weeks of Pt to get her back. They deliberately kept her until Medicare ran out all while not helping her and lying about it.

    i was here for almost 2 months and from the first day they surprise me with a vase with flowers the nursing staff is awesome,the management is awesome when i was there ricardo, ivy and every single person in charge did even the impossible to have me as comfortable as i wanted to be, i gotta say that edy, luisa, traemon, aira, jessica, jennifer, pamela, kara, christine, francis, dora, karla, and justina are the most excellent nurses in bella vista, but also i gotta thank janett and her staff for keeping me entertain in their activities room, im specially thankful to my therapists: carmelita, philip, steve, bell and lee, thank you jeff and orlando for your help both of you rock, and last but not least raquel thank you very much for your help, to anyone that reads my review don't think twice in picking bella vista health center for your needs they will take care of you, i'm starting to walk thanks to god and bella vista if there was more stars i would give them 10.
