Overview of Professional Staff and Training
We have a Registered Nurse that is on call 24/7. She is the Executive Director, and also the Wellness Director. We have RN delegated Med Aides, and Caregivers.
Safety and Well-being Measures
Fall Incident Tracking is one of the key measures in place to ensure the safety and well-being of our residents.
Transition Support Services
Through Home Health we offer Physical Therapy, Occupational Therapy, and Nursing. There is also Hospice Care, if needed.
Transportation Services
Yes. M-F within a 16-mile radius.
Dining Experience
We have restaurant style dining. Offering 12 different items at each meal. The Caregivers go table to table and take the orders, nothing is pre ordered.
Resident Complaints and Issues
Complaints/Concerns reported to the ED. Investigation completed by the ED with all involved parties. Help from Regional Team for resolution, if needed.
Cost of Living
I have attached a 2024 Rate Sheet for AL/MC and an IL Rate Sheet as well.
Communication with Residents' Families
We have an Internet App called Sagely. Resident’s families download Sagely to their personal device. This way they have access to resident well-being, activity participation, and all upcoming events. We also make frequent phone calls and send text or emails. We do Quarterly assessments with family updates.
Short-term or Respite Stays
We do offer Respite Care, there is a Seven Night Minimum Stay. The rate is $185 per night.
Pet-Friendly Community
Yes we are, for small dogs, and cats. The non-refundable Pet Fee is $650(paid upfront before the resident moves in) Vaccinations must be current, with a copy kept on file at the Community.
Waitlist Information
We have a waitlist for our Independent Town Homes. We have very limited availability in Assisted Living at this time.
Covid and Flu Vaccinations
Yes. We have a local pharmacy that provides Covid/Flu Clinics.
Aging in Place
Yes, and it is highly encouraged.
