Overview of Staff and Training
We have licensed nurses on staff 24 hours a day, along with nursing assistants. All nursing staff are CPR certified, and undergo monthly trainings on topics such as abuse/neglect, communication, handwashing, PPE, working with dementia clients, ADLs, elopements, and fire safety. The Executive Director (ED) has over 33 years of nursing experience, is a Certified Dementia Practitioner, and a Teepa Snow Positive Approach to Care Coach. The Health Services Director (HSD) brings 30+ years of nursing experience, with a strong background in long-term care.
Safety and Well-being Measures
We conduct frequent safety checks, allow the use of bed canes, halos, bolsters, and fall mats for mobility assistance and to mitigate fall risks for our residents.
Transitioning into the Community
New residents are given time to adjust, an orientation to familiarize them with the community, and are introduced to fellow residents who help them integrate smoothly.
Transportation Services
Transportation is available for residents, requiring appointments to be scheduled at least 2 weeks in advance. There is a charge for trips over 10 miles.
Dining Experience
We offer set mealtimes with a variety of breakfast options, a main entrée and soup for lunch and dinner, along with alternate meal choices, sandwiches, salads, and hamburgers daily.
Addressing Complaints and Issues
The ED’s office is always open for residents to voice concerns. We also hold monthly resident council meetings and maintain a concern/issue box for after-hours communications. Family members are provided with the ED and HSD’s contact information for direct communication.
Cost of Living
Room and board fees range from $3355 to $4550 per month, with additional care fees based on individual resident needs, ranging from $810 to $4100 per month.
Communication with Residents' Families
Families are notified of all changes and incidents if a resident has a POA or requests family contact. Families can also reach out directly to the HSD or ED with questions.
Short-term or Respite Stays
We offer respite stays subject to apartment availability, with costs varying based on the room and care needs. Respite stays are for 30 days or more.
Pet-Friendly Community
We welcome small dogs and cats, with a one-time, non-refundable pet fee and additional charges for staff-provided pet care services.
Covid and Flu Vaccinations
A local pharmacy provides Covid and flu vaccinations for our residents.
Aging in Place
Residents can age in place with support from outside therapy and hospice companies, barring any regulatory issues. We have limited Medicaid Waiver rooms available.

With a full-time licensed nurse available 24 hours a day to provide clinical oversight and coordination of care, our pet friendly community strives to maintain each resident's independence through engaging activities like, Gardening, Bingo, Library Chats, Prayer Groups and Trivia.
Our Apartments
Urbana Place is a small homey community of 37 apartments. We have 1 bedroom and studio apartments that have a small kitchenette, microwave and refrigerator. The current apartment we have open has a nice walk-in closet as well. We have a beauty shop on site as well as a Spa room with a whirlpool tub.

We like to keep our resident active. You can see all the activities the residents do by visiting Trustwell Living at Urbana Place on Facebook. Many of our residents are from locally around the Urbana and St. Paris areas.
