All beds are Medicare and Medicaid certified, and all rooms are equipped with new electric high/low beds and cable TV. Private phones are provided for those staying in the short-term rehab units. Additionally, Sunnyvale Post-Acute Center is the only provider of peritoneal dialysis in Northern California, and a physician is on-site three to four times per week. A full in-house rehab unit provides physical, occupational, and speech therapy, and admissions can be processed 24/7.

    Hi guys. Here is such a nice and quiet environment and source of positive energy. Specially administrator Daniel Jenkins is a nice and polite guy. His is very loyal, positive and humble! Their staffs and all nurses are so kind and positive thinking. They really care about patients and do their best. Service is the best. It has a magnificent management! They are problem solver, authentic, ethical, smart, dynamic, energized and committed, inventive, compassionate, expert and loyal chiefly administrator! I had a great experience with them and highly recommend this center as the best Health Care Center in the U.S. !

    My brother has been in this facility for almost 2 weeks and what he tells me about the care is atrocious. He is supposed to be getting physical therapy. If you call it therapy he is recovering from surgery on his foot. I wouldn't want my dog to get care there. His name is Richard Bsharahand is in room 40 A, if any manager is reading this you need to talk to my brother. Also I called to talk to a nurse last week and was told they were handing out meds. And was told I would get called back but was never called back that was Monday and today is Sunday. I just hope he can get away from this place.

    This space is Absolutely terrible rehab center and it all exposed with COVID-19 my father is there they did not allow to anybody go and visit starting March and all of the sudden started calling people last week and they said two of our staff member has positive COVID-19 and 10 residence but nobody allowed to go there from outside it's only from their staff the owner and the administrator and the people are working there for staff and nurses that are not responsible for their job they being sick and they make those elderly people sick and they did not announce it anywhere today they called and they said my father is positive as well somebody needs to be responsible for that and answer all those questions that family have Elderly people that have severe Alzheimer's and very sick heart problem and there and he was system is not good who is going to be responsible for that many elderly people it is really heartbroken They really broke my heart I just want to somebody be responsible for that please

    Be careful with patients in the dementia unit.  A few months ago a resident called Peter wheelchaired himself into my mom (who got a stroke and has been paralyzed) so I tried to stop him.  As soon as I walked by his wheelchair he hit my upper arm twice that I still have pictures to prove it.  There were many times he went into my mom's room uninvited.  Since I filed a complaint with the nurses, he seemed to behave himself.  I heard he touched women inappropriately before.  Not only was he scary, his daughter was very rude too.  Tonight I went to the kitchen to wash my hands. As soon as she saw me, she jumped to me and screamed at me to go away so she could get something from the cabinet.  That was another unpleasant encounter from the family.  So be careful dealing with other patients and their family too!

    I want to thank every staff members in this facility for taking care of my  grandma. This nursing home is one of the clean facility I've been to and was greeted by every staff I bump into. My grandma had a problem not able to walk for too long and somehow after weeks of being in their care. She felt better and was able to walk, her attitude towards us was more friendly and more energy after leaving that place. The nursing staff understood our concerns and always kept us in the loop with our grandma while she was in there. Social service was helpful with resources and was able to meet upon request.  I highly recommend this place to my family and friends. Thank you Sunnyvale post acute center!

    Visited Sunnyvale Post Acute today. Before entering the facility I saw their beautifully planted flowers. I was immediately greeted by the front desk staff. I was surprised to see their taco fundraiser.  I saw their patients  and staff enjoying tacos. Everyone genuinely seemed happy. It's no surprise I hear wonderful things about this nursing facility.  The tacos were delicious.

    I was a patient for 1 year & 8 months, i require special care due to my condition & they went out of there way to meet my needs! My time there was very comfortable with the friendliest staff & I was happy there. The staff is very knowledgable & work very hard to provide you safe & comfortable environment in a home like setting. Lilyana was hands down thee best cna to work with, she really makes sure you are very well taken care of & works very hard to make sure everyone of her patients receive the best possible care! I would recommend Sunnyvale Post Acute to anyone that wants to trust their family to safe, comfortable & reliable care. Thank you for providing me such a positive experience!

    warning:  to all families that are looking for a s.n.f (skilled nursing facility) in the santa clara county...please be forewarned & do your thorough research on snf's you are considering entrusting your loved ones care & well being!!! please let our unfortunate predicament & many, many, many other family's horrendous experiences help you decide where to send your loved ones needing rehabilitative care, but most critical " not make a grave mistake of admitting your beloved family member to sunnyvale post-acute center" in the santa clara county!!!. this post is only a warning post to immediately get the word out that "sunnyvale post acute center's business name is clearly a misnomer!!!; speaking from experience in the snf within that last month till (thank god) 2 days ago. their dba (doing business as) should actually be in reality "sunnyvale adult day care center." !!!!!case in point, my 89 y.o mother, with multiple cardiac diagnoses & having had two separate episodes of having a heart attack was admitted, by majority census of our family members, due to the few yelp reviews that were posted. extreme mistake!!! . she was admitted 3/22/19 & luckily had doctors appointments with her cardiologist & podiatrist doctor for a follow-up visit. thank god, a tragedy turned out to be a blessing that my mother had to be admitted right there & then to the er for an amputation of her r-big toe that developed sepsis due to inadequate or more realistically...no continuing treatment at all at the snf. thank god we requested her whole case files at the snf & did notate her s/p l-r-big toe matrxectomy, solving our unforseen fortunate discharge from that "adult day care." more specific documentation will be posted a little later, but this is a specific warning to help families not make the same mistake many many many earlier families had to go through.thank you for reading my post,in god we trust,chito z.

    My Great Grandma was a resident here. She spent the final year of her life at this facility. The staff took really good care of her. There are great activities at this place, and a ton of things to keep you busy and active! The staff here is awesome! Thank you for taking such good care of my Great Grandma! You guys rock!

    Pros: * Close to 85 and Fremont* Rante and Roy are excellentCons: * Short staff resulting in long 45 to 90+ minute waits* The staff sometimes yells at patients.  Josephine, CNA, has a bad bad bedside manner.  The way she talks to my friend is horribly unprofessional.* The food is, at best, ok.* Posted in the hall is a sign requesting you to "be quite" (sic), yet the PA system frequently blasts announcements in the halls. Use walkie talkies, folks!

    My grandmother had to stay here twice, this one specifically because Kaiser only partnered with them and it's located closest to my home out of the other providers I selected. 1. Parking sucks2. The aids are not friendly and they weren't adhering to the schedule that the nurses told me would happen. I was suppressed at the plan of treatment- it seemed great, however they simply didn't have the man power to do all they promised. 3- the doctors and PT were great! probably because they are Kaiser doctors. But they weren't on site 24/7.4. Billing- this is one of my biggest complaints. On my grandmas first stay they tried to bill us $50 a day for the first 10 days but as my insurance was explained to me- I should have had no bills.  Obviously I didn't pay it- and received a second notice. So I took it to their office and they acknowledged the mistake. A year later she had to stay again and the same thing happend. I told them again and they told me to disregard the bill. She left the facility in July to come home on hospice and passed in August. NOW DECEMBER 1st- 4 MONTHS LATER, I received a bill for $2,300 for the stay for the month of November!!!??? How is that? I called them to try and handle the situation because I want them to be aware of their mistake, the lady on the phone took down my name and number and told me their manager will call me back. Be careful if you or your loved one stay here. They will bill you incorrectly. I'm sure they do it on purpose to try and find "suckers" that actually actually pay. It's sad.

    1.  The place smell bad2. The place is dirty and the building is old. 3.  Some of the staff need to go back to school again and learn how to give patient their medication.  The nurses are like robots, they just work no thinking.  When asked, did you read the patient chart?  Their answer, too busy, just scanned through it.  4.  Some of the staff are unprofessional and lazy. 5.  Some of the staff need training on how to handle patient with care, compassion, and kindness.   6.  The food is disgusting7.  Some of the staff are ignorant and do not care what's going on around them.  They are just there doing the worst job they can do and go home with no conscious. 8.  Sunnyvale acute center should be put under investigation for their poor care and negligence to patience safety.  9.   See this web site for many complaints filed against this facilityhfcis.cdph.ca.gov10.  I do not recommend Sunnyvale Post Acute center.

    This review is a month or so overdue.. but I just have to say this is the Worst place ever. It smells so bad once you enter. The lobby may look nice in the pictures, but of course they don't put a picture of the "garden room" where activities happen, and mind you, there's not even one plant in there and looks like it hasn't been upgraded since the 1920s..it's disgusting!  No one ever answers the phone. And never any management around, what's up with that? Understood that they are off on weekend but it's a weekday before noon and no one here? All the staff said they haven't seen anyone either. Strange. Goodbye, I'm glad I put my family member somewhere where they deserve.

    My mother was admitted to Sunnyvale sub-acute to recover from hip surgery.  Our family spends a lot of time on site advocating her treatment as this place is horribly understaffed especially on weekends.  Patients wait for very long periods of time to get help.  The staff is constantly trying to decide who needs help first.  Urinary catheter bags are not emptied and urine overflows onto the floor.  Surgery wound dressings are not checked every shift to determine if they need to be changed.  We were told this is because the 'treatment nurse' called in sick 2 days in a row.  It takes multiple requests from a family member and many hours of wait time to get someone to come out and change a dressing.  Patients are left in their night clothes without a sponge bath as CNAs are short staffed. This environment is not conducive to recovery.

    Sunnyvale Post Acute Center provides a warm and safe environment for personal rehabilitation. The staff there is highly competent, professional, caring, and they see to every need of the patients. There is opportunity for the patients to socialize with one another, and the facility provides activities to keep the patients occupied and optimistic during their stay. Three balanced meals are provided during the day, along with snacks. You will be in good hands during your stay at this facility.

    This place is a shop of horrors. My mom was suffering. They didn't give her blood pressure meds and they kept her sedated. She was begging to be rescued. Screams at night and smell of urine all the time. The entire time we were there, not a single staff member checked on her. She was there one week and they never bathed her. She ended up getting pneumonia and went back to the hospital. I've never seen her so terrified.  When we took her out, we had to lift her into the wheel chair ourselves. We rolled out the door ourselves and we got into the car ourselves. A liability nightmare. Not even a glance from staff to help. Five stars? Yeah, from the no-care staff trying to save their jobs.

    My dad was brought here after a fall. It was a terrible experience for him and those visiting him. Listening to our patients constantly begging for help but aides seemed to never be around to help.  The same visit we experienced a patient trying to leave facility. Place is not cleaned regularly as I had put a piece of paper in the lobby table as a test and it was as still there as he next day.  As a family we decided that it was best for my father to o come home.   We discovered the medication that he was discharged with was not his!  It was another patients!   I can't explain how frustrated I am to know that if this happened what makes It not east for them to give wrong meds to the patients.  Now I am wondering what they gave my dad?   Unfortunately we were not given a choice about taking him here.  We were told this was the only available place.  If we would have had a choice. We would never have brought him there.

    My wife stayed here for 3 weeks and the place is the best nursing home I've ever seen. Every staff member (maintenance to administration to the caregivers) demonstrates a genuine concern for the patients. Everyone treated my wife like a family member. The meals were varied and quite delicious. I am so impressed with this place. I would recommend it to anyone who needs their services. If I had to be in a home, I would want it to be at the Sunnyvale Post-Acute Center.

    I have been here for an entire month now and I have to say that this place really surprised me with their level of concern for each of their residents here and their attentiveness and attention to detail on a patient by patient basis is outstanding. I wouldn’t say any of this if I hadn’t witnessed it first hand.There was a time where I had a problem with the transportation to my doctors appointment and Sandra from Admissions took complete charge of the situation and stepped up tot fix the problem herself and got it all figured out right away. That was very impressive to me how quickly she made things happen for me and I will always be grateful for her and her efforts! Thank you Sandra I hope you read this some day!I am going home from here this Friday and of course I am beyond happy to be going home but, I must admit it’s a tad bitter sweet. I will miss some of the staff here that I have grown close to in my short time being here, and Sandra is definitely one of them! She is awesome! And hilarious!!! The nurses are kind, the CNA’s are timely, and the PT & OT are truly amazing!

    Sean Barksdale. Is a Rock star at his job.I transferred from Sunnyvale back to VMCHospital 1/2 my belongings made it w/me.1/2 didn't. Then i got the other 1/2 of my things...yay! But no purse or my personal walker ( which has my name written on it in like 5 different places on the Walker Michelle Roach)BUT SEAN. RECOVERED MY PURSE!!INCLUDING: my birth certificate my bank card my current ID my driver's license my everything. Are the very difficult to recover all these items and there was no cash missing. So a big hug and a big thank you and God bless you toSEAN BARKSDALE. worked relentlessly AND KEPT HIS WORD.. recovering my purse I'm so grateful

    My dad entered the facility 2 months ago and it was not a good experience. They are under staffed, no communication, and don't check on patients with urgency. Things that belonged to him always went missing and no where to be found. They do not follow instructions that are given. My dad went down hill rapidly and passed away. I have a hard time to not think that if he was cared for better he may still have had some more time.

    Listen folks if the place has any bad reviews when it comes to your health or the well being of a family member. I had the worst experience in this placeFirst off I was lied to at the hospital about what my freedom and boundaries were while staying there.I got there late at night and was roaming the halls and asked a nurse a simple question and was answered with go bother someone else I'm busy.Second the next morning I was chased down when I went out front like I was a prison inmate .Not all the staff are bad but are you going to chance it to the odds of one of your loved ones to get the disrespect of someone who they're looking for comfort and your giving your trust in someone to take care of your loved one while your not there.A place is only as good as there worse employee when it comes to your family.The extent of ones insurance should not matter on the level of treatment and compassion when your too I'll or weak to look after yourself or the ones you hold dearest in your heartI hope that this is helpful in making the right decision

    Selective treatment of patients.While I don’t doubt the reviews below, it is my experience that the staff at this facility are selective on the treatment of their patients. They will either help or leave you for dead.My dad was recklessly transferred here by another facility with missing information that put his life at risk, along with both unclear and vague dietary information/medication schedules. Because of this confusion, the staff chose to do nothing until they figured out what to do. Meanwhile, the doctor, instead of taking responsibility for his patient, decided it was better to try to convince my mom it would be better to let my dad go. Because he isn’t going to recover and he can help make that happen if we were okay with it... It was a combination of this confusion and attitude which lead to my dad being rushed to the emergency room a week after initial admission to Sunnyvale Post Acute...Primum non nocere, “first do no harm”. That is the Hippocratic oath that all doctors must take. I know this and was personally there to witness an entire graduating class of new doctors take this oath at my brother’s graduation. Doctor Chi must have forgotten his oath or if he hasn’t, he should have his license revoked. I fully understand my dad’s condition is a serious one, my brother is a physician after all. But how can someone, sworn to care and do no harm, offer to starve someone to death after only seeing them once and barely any evaluation? How can a patient make any progress in recovery if that patient has never been provided sufficient care to be able to recover, especially after such a serious injury?The admitting doctor from the E.R. At ElCamino Hospital was able to admit his initial prognosis was wrong since my dad showed improvement during his stay there. Why can’t Doctor Chi? Arrogance? Lack of compassion? Does he even deserve his license?Despite the objections from myself and my entire family, my dad was forced to be transferred back to Sunnyvale Post Acute. I came in to visit my dad today to see broken a light above his head with a broken housing and shattered fluorescent bulb inside of it. Someone had apparently attempted to change this light and failed miserably. A chunk of the housing was still on the floor behind the bed, behind my dad’s head... This was also right above a running oxygen concentrator, putting both my dad, the other patient in the room, and everyone in the facility at a serious safety risk!

    Megan, the Dietary supervisor is a top star, she always make sure that what I need for meals is what I get. If there are any problems, she will make sure to correct them. In my opinion, she is one of the best employee the facility have. I would recommend this facility as far as food goes, food is okay.

    I have stayed here at the facility. I'm very satisfied with how the staff caters to all patients needs. They have the appropriate people to care for me as well as others. The social services department set up my transportation that i could not do by myself, they got me to my appointments on time and came back on time as well, it was a stress free feeling that i really appreciated. Sometimes alone like i didn't have anybody, but the staff has felt like a family and i will always remember that feeling. The activities department held a great amount of activities, i personally enjoyed their movie nights on Wednesdays, they made the environment feel like a movie theater. Football Sundays were also another personal favorite, i enjoyed being able to gather with others in the social room and watch and talk about football, i really enjoyed that. The kitchen staff is also very accommodating, if you don't like whats on the menu the kitchen staff will try their best to give you something that you will like. The wound care is good, but they could probably use an extra person for assistance. I have stayed here almost 4 months preparing to go to my operation and my overall experience here has been a great pleasure, and a 5 star experience.
