This place is absolutely beyond terrible.  Thank goodness my friend is lucid enough to know what is going on.  She has been given the wrong medications several times.  Her wound change is inconsistent, different times of day, different person different procedure.  Practices are unsanitary causing infections. The nursing book being used for her care had a different patients name in it ! I have seen several mice running in room. If you love your loved ones stay as far away from this place as possible .

My mother received subpar care at this facility. They ignored her wishes, and her Hospice team's requests. They put a patient with a contagious disease in her room without telling me. My mother passed away last Friday. And no one, NO ONE from this facility has bothered to contact me. Shameful. Our Elders are not Cattle.

This place is horrible, the evening and night staff do absolutely nothing, they gave my client the wrong meds several times, left a pill pop out sheet of Oxy in his room, and did not even go to look for it for several hours, I saw a staff member on more than one occasion just get up and stop feeding one of the residents because they didn't want them asking questions or talking to them, they hardly check on their residence, they ran over my client's glasses in a wheelchair and never replaced them. I would not put your loved one here unless you want their health to get worse or for them to die, worse place I've seen since I did my clinicals 11 years ago! Numerous complaints were made by my client and myself, I had to question staff members more than once why my client is not being taken care of, why he had not been helped after calling for it two hours prior (this was not during a busy time this was at night when they were doing nothing). All they do is sit around and collect their paychecks from what I have seen!

Do not put your loved one in this horrible place! I cannot say one good thing about it. My father was there for a very short time. We moved him out as soon as we could possibly arrange it because of the poor care.

If I could give this a negative star I would! This is ONE of the worst centers out there! Please DO NOT bring your loved one here especially if they do not speak good english. My aunt had hip replacement surgery and was moved to this center. She does not speak good english. They never got an interpreter for her, even when she requested. She was not checked up on regularly, the bed she got was not fit for her recovery. One male brought her to the bathroom and never came back. When her friend came and saw the neglect of care from this center they decided to move her from there- when they were discharging her a caretaker came in asking her to sign papers my aunt told her she does not understand what she would sign and needs an interpreter. The care taker came back later and asked her to sign one piece of paper, my aunt did sign it. They discharged her with out any information on how to take care of herself. When her friend came to transport her, on the way to another facility my aunt told her that she needed one more injection to prevent thrombosis ( she could have died of a blood clot)- her friend contacted the hospital and told them all that happened. They were asked to come to ER where nurses and social worker met my aunt took care of her/gave her rx for injections she can do at home (that the facility was SUPPOSED to give her), assigned a physical therapist to come out to her home and etc. Later to find out my aunt had signed a paper saying that she will pay for all the care she received from the facility WHILE it already HAS been payed by her insurance. I know this is a lot but PLEASE this place completely screwed her over and took advantage of her. She said that after hip surgery she is supposed to be rotated to allow for proper circulation and muscle contraction so they do not atrophy but it was not done for her for 4 days. And the people did not even know how to move her- they verbalized this themselves. What is this place? I am so relieved to know she is out of that place. Don't bring your father, mother or whoever here. They will be neglected of attention and basic care. Look into other place, do random check ins, place a hidden camera- they do not deserve the money they get.

My sister was in this place and the care was horrible. They didn't get her up and out of bed, they didn't roll her over or move her, they did nothing. She ending up with a sore that got infected and was ignored for so long that she was septic and passed away. I was there when they were supposed to get her up and take her down for dinner. The aid did everything she could to convince to just stay in bed and eat. She came in with a tray and threw it on the table out of reach and left. My sister was too weak by that time to feed herself but the aid didn't care. I tried finding a nurse to check on her medications and no one would help. Do not go here!!

My wife, Phyllis Johnson is writing this post.I have been home now for more than 5 weeks. I still miss my nurses and some special nurses aids. I received excellent care and have no major complaints. The food is really not bad and offers a well balanced diet including frequent fresh fruit and desserts every day. When I put my call light on, aids or nurses were usually quick to respond. It's pretty quiet here at home, and sometimes I miss being at St. Anthony. I'm not saying everything was perfect, but they were very good to me and really seemed to care and wanted to make me comfortable. The Therapy department and the laundry service was also very good.Thanks to all for your hands on care and pleasant personality. I was also very impressed with the Chaplain who came to visit me.Sincerely Grateful, Phyllis Johnson.

The Doctor and Nurses at the facility are friendly and are competent. The Physical Therapy Department al the facility is very good. Unfortunately, there are many negatives associated with the facility. The food is terrible, the Aids do not appear well trained, and I have experienced some rudenesss from two of the Aids. I had a lot of trouble initially getting the Aids to understand how to get me up and even now have some trouble with this. The rooms in Rehab are not well lighted, are small, the beds as a standard do not have rails and the dressers are located close to the bed which could be a problem if the patient rolled at night. The rooms should have reclining lift chairs as standard both from the stand point of relaxing and so legs can be elevated for patients who have a tendency to have swollen legs or if necessary sleep in the recliner. The facility is dated and needs renovation. Aids are not always prompt in responding to call lights.The system for serving food is confusing and lots of mistakes occur. The system for handling food service should be computerized. Activities for Rehab patients are almost nonexistent and the activities that occur are based on the second floor with The Rehab patients for the most part not invited. Overall I did not feel comfortable at the facility and I would have to rate it a one out of five. If things were changed, it could be a very good facility.

My mother had a horrible experience here. She slipped out of her wheelchair and they put her back in the chair and then left her. They didn’t do an X-ray until a week later at our insistence and found she had broken her hip! The only thing they gave her for a week with a broken hip was Tylenol. Their doctor wanted to just keep her in bed and give her morphine. When we told them we wanted to have them transferred to Mercy hospital they said they would have that taken care of. I waited at the hospital for her to be brought there for over an hour and when I called to see where she was they said that they thought I was going to pick her up and take her. How was I going to take a 98 year old woman with a broken hip to the hospital in my car. We filed a complaint with the state of MN and they were found to be negligent in her treatment. Stay away from this place!!
