Southgate is a locally owned, family-run facility that offers a continuum of care from independent living to skilled nursing and rehabilitation services.
We have been caring for Southern Illinois and Western Kentucky seniors for more than 60 years.
We’ve grown from humble beginnings. Our nursing & rehabilitation center was founded in the early 1960’s. It began as a small operation, with two Metropolis homes that were converted to serve as Geriatric care facilities.
Over the years, we have grown to meet the needs of the Metropolis and Paducah area’s aging population.
But we haven’t forgotten our roots, and our connection to the community that we serve.
We have a well-earned reputation of providing exceptional care, and a compassionate and competent team of health care providers and support staff that keep that reputation in good standing.
