Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
Each case is reviewed for the need for a Secure Unit, falls, wound care, etc., prior to Admission.
Do you offer any resources or support services to aid residents' transition into the community?
If when in Rehab, the Discharge Planner at the SNH, will help get additional service in the Home if needed.
Do you offer transportation services for residents?
Transportation is only covered if the Patient has Medicaid. It is not included in the daily rate. If a Patient does not have Medicaid, this will be a personal cost.
Could you tell me a little about your dining experience?
Each Patient is assessed if they can feed themselves, or will need assistance, there is a scheduled Meal time for each floor. The dietitian will meet with each Patient to go over restrictions, and preferences.
What is the process that takes place if a resident has a complaint or an issue they want addressed?
If the Complaint is Medical, they or the Family need to speak to the Nurses Manager on the Floor, if not Medical, they can make the complaint to the Discharge Planner, then is sent to the DON, and or Director of Social Services, investigated, and will get back to you with the plan of correction.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
New York States directs that if the Patient is Alert and oriented, we are to discuss their care with them first, and they will relay any communication to the Family. When first Admitted we have a Team Meeting two weeks after Admission, and the 1st contact will be sent a letter. If the Patient has dementia, we will call the 1st contact.
Do you all offer short-term or respite stays? How much does this cost?
We offer Short Term Rehab and Long Term Care in Skilled Nursing. If coming from the Hospital, there is a chance that the Medicare Plan may cover. If coming from Home, that will be either private pay or Medicaid. Price list attached. Respite is at least seven days, and is private pay only.
Are you all a pet-friendly community?
Pets may come to visit, but must be groomed first, have a shot record on file, and meets in Patients room.
Are residents able to get Covid vaccination and flu vaccinations in the community?
Vaccination can be given at the Skilled Nursing Facility. If in Assisted Living, the Patient can receive vaccines at their Primary care Office.
Are residents able to age in place?
Ageing in Place is started at Assisted Living, if they then need Skilled Nursing, we will transition to Summit Skilled Nursing. This will depend on which facility, as in Skilled Nursing it changes daily.
