Do you guys currently have a waitlist?
Yes, we do currently have a waitlist. We do not have a set time on when the next room will be available as there are many contributing factors. Let us know if you are wanting semi-private or private.
Could you share pricing and any other general information?
$350/day semi-private room, $400/day private room. These costs are for private pay/no insurance. If you have insurance we can discuss costs based on your plan. We can provide you with an admission packet via e-mail or when you tour.
Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?
All of our staff undergo monthly training provided by Healthcare Academy- topics include dementia, HIV, customer service, infection prevention, etc. We conduct quarterly skills fairs which are mandatory and allow all staff to have hands on training- ie: catheter care, perri care. We have safety and ethics meetings monthly. We have resident and family Quality Assurance Performance Improvement (QAPI) meetings the first Wednesday of every month. Our QA meeting with all department heads and Medical Director are monthly. We have weekly risk meetings which we focus on QAPI and current Performance Improvement Plans that are in place to ensure we are meeting or working towards our goals- these meetings include DON, ADON/IP, ED, Unit managers, CDM, and Risk Manager.
Our department heads include: Director of Nursing- RN (DON), Assistant Director of Nursing who is also our certified Infection Preventionist- RN (ADON), Staff Educator- RN, Risk Manager- RN, Two Minimum Data Set Nurses who focus on quality metrics- one RN and one LPN (MDS), Unit Managers oversee floor staff on every shift (2 on 7-3, 1 on 3-11 and 11-7), Licensed Social Servies Director (SSD), Certified Dietary Manager (CDM), Housekeeping Supervisor, Maintenance Director, Activities Director/Life Enrichment, Director of Therapy-OT (DOR), Executive Director (CDM, NHA)
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
We are a restraint and alarm free facility. It has been proven that alarms do not prevent falls but more so alert staff when one has already occurred. Restraints are not humane. We strongly believe in treating the person as a whole and every individual has a different background. Our interventions to protect resident safety are individualized. For example, we have a falling star program for residents that are at high risk for falls or who have frequent falls- a star is placed on the outside of resident room to notify staff to frequently check on resident when in room. However, these residents are strongly encouraged to attend activities throughout the day or be in common areas. We have developed a “bumble bee program” to include high fall risks and those who like to wonder which goes on until 7pm 3x week. We look at each individual resident for why they may be falling- for past examples- some residents stand when they have to use the restroom but are non verbal (we place them on a toileting program), some do very well with music therapy, some require a snack program. We learn to read residents needs to include interviewing families for any past habits/preferences.
Our weekly risk/QAPI meetings are to improve well-being through continuous quality improvement. Which we can explain further in detail when you tour or attend a family/resident council meeting.
Do you offer any resources or support services to aid residents' transition into the community?
Yes, we have many resources and our SSD will help every step of the way.
Do you offer transportation services for residents?
Yes, we have our own van and also have multiple transport companies available- our medical records clerk will follow and set-up appts.
Could you tell me a little about your dining experience?
To continue with our focus on quality of care we have a seasonal special menu with a full restaurant menu offered at all meals- 52 choices total. Resident or family can fill out tickets (just like restaurant) for an entire week at a time. We won 1st place in the Florida Pioneer Networks Quality Initiative for our dining process.
What is the process that takes place if a resident has a complaint or an issue they want addressed?
We feel that all issues or concerns should be completely addressed immediately or maximum 2 hours depending on the concern. We have suggestion, grievance, and concern forms located at each nurses station, SSD office and admin suite. These are responded to immediately once received and come straight to ED. However, you will have the cell phone number of the DON for if you ever may need it. Our unit managers are also always on the floor and looking to help with whatever is needed. You will have an initial care plan meeting within 48 hours and then again within 14 days and quarterly thereafter. All department heads doors are open at all times- you can always just ask the receptionist to get someone.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
We can provide you with a copy of this but the first contact is contacted regarding any new orders or changes in condition. Care plan meetings as noted above. You are always welcome to call any of us or the nurse on floor.
Do you all offer short-term or respite stays? How much does this cost?
Yes we do. Same cost as mentioned above.
Are you all a pet-friendly community?
We do allow pets to visit as long as they are up to date with there shots and supervised by family/friend.
Are residents able to get Covid vaccination and flu vaccinations in the community?
Yes we do provide these, to include PNA and RSV.
Are residents able to age in place?
This is our goal to provide them the highest quality of life and care throughout their stages. However, do note that we do not have memory care.
