Employee Reviews for Ensign Services
2.97 out of 5 stars. Based on 442 reviews
Employees of Ensign Services report a great environment, benefits, and friendly staff. However, they also mention a fast pace, location issues, lack of competitive pay, poor management, limited resources for help, low pay for the work they do, rude residents, lack of structure and non-defined responsibilities, lack of transparency, nepotism, and a lack of appreciation for employees. Advice to management includes being more empathetic and listening to employees, putting patients first, not profit, hiring for talent, not just bodies, and creating more lower level office staff to help ease the burden off administration and nursing.Information from Centers for Medicare & Medicaid Services

Spotting in the top half of nursing homes for nursing aid staff hours with 2.25 hours, which, albeit less than the state's average of 2.53 hours, is a notable achievement. The facility also shines in terms of Licensed Practical Nurse (LPN) staffing. With an impressive average of 0.88 hours per resident per day, it surpasses the state average of 0.82 hours. It's also in the top 30% for Registered Nurse (RN) staffing hours and licensed staffing hours, despite falling slightly short of the state averages at 0.77 and 1.65 hours respectively. Though alarmingly below the state average for resident vaccinations, the rate within this nursing home is currently at a low 37.9%; considerably lower than the state's average of 61.0%. The staff's low vaccine uptake has also caused concern, sitting at a paltry 4.4%, a far cry from the state's average staff vaccination rate of 28.6%. Therefore, not only are less than half of the residents at the Mountain View Rehabilitation and Care Center up to date on vaccinations, but the staff member vaccination rate is also staggeringly low.Quality Measures
Mountain View Rehabilitation And Care Center, a nursing home, has shown a mixed level of care in terms of vaccinations and resident safety. Additionally, Mountain View Rehabilitation And Care Center performs well in controlling the use of antipsychotic medication, falling in the top 30% of facilities. Despite the relatively high usage rating, the nursing home's use of antipsychotic medication stands at 8.84%, still well below the state average of 13.94%.Emergency and Hospitalization Quality Measures
Mountain View Rehabilitation and Care Center, a nursing home facility, ranks among the bottom 20% of facilities regarding the percentage of short-stay residents that had outpatient emergency department visits, with a standing percentage of 17.1. This rate is markedly higher than the state metric, which sits at 16.8 percent, indicating room for potential improvements in patient care and discharge planning.Healthcare Nearby

Do not leave a loved one at this facility, it is extremely poor. Overcrowded, three to a room. They do not change the clothes on their patients regularly. Unwatched for long periods of time. Falls go unreported. After three days we had our loved one returned to the hospital by ambulance after having a fall leaving a huge knot on her head. The food resembled Dog Chow.If you are considering leaving a loved one here quickly change your mind.
I'm devastated at how my mother was taken care of! If you care at all about your loved one don't leave them there!! Personal care was missing , my mom was told to urinate in her brief instead of helping her to toilet, medications were lied about documentations were in accurate! Bed sores started in less than a week! I will say it it's a clean facility and a few gems of employees for sure maybe 2-3 .. I truly felt if she would stayed in there for much longer she would of died but not from her cancer but from them taken care of her , BE AN ADVOCATE for your loved one!!
The VA sent my dad here for rehabilitation for just under a month. The facility has been remodeled and is beautiful. Management, CNAs nursing, and PT are all amazing. In the last year my dad's been to 3 facilities and this by far is my favorite. I can't say enough nice things about this place and trust me I'm hard to please. I rarely came into a soiled brief. My dad put this place through hell. He pretty much required a one on one sitter. He was learning to walk again and has dementia. So he couldn't stand up and walk alone. He would get up every five minutes setting off his bed alarm and the CNAs would have to run in and go for a walk with him, because he would refuse to sit back down. The facility accommodated it and did so with a smile on their face. It did put a huge stress on their staffing and they ended up having to have an extra person for that shift just to babysit him when he would get up. I can't comment on the activities because we never participated in any. It's been a year since we were there and I forgot to post the review. I can't recall what the food was like. Now I have only been to two other places rainier Vista in Puyallup which is a bit older but my second favorite and kindred Northgate run away fast.
My wife went in there for a broken leg and wrist. This place is more of a nursing home than rehabilitation center. I got her out of there ASAP. Actually some of the questions the asked her were more of someone with mental issues instead of being concerned about her injuries. They didn’t want her getting out of bed by herself to go to the restroom but didn’t come when she turned the help light on. After waiting 20 minutes she did it herself. Medication also was an issue. Couldn’t get that right. We both knew this wasn’t the place for someone who had physical problems. We started asking the first night to start her discharge papers ready. After they started them they came in again and started asking her the questions again on her mental state of mind. I could go on and on about this place in the 2 days she was there but I think you get the idea. No place needs this kind or review but honestly this place was horrible
It was one of the most humiliating times in my life. I was left on the camode unattended for 45 min. The call button to my room did not work for 3 days. Was left in a soiled bed for hours. I will never recommend this place to any one. A few of the employees were great but generally they acted like I was to much bother.
Extremely poor facility. Overcrowded. Food like dog chow. Patient left in same clothes day after day. Patient left unattended for long periods of time in bed, not even comfortable chair for patient to sit in. Three days never brushed teeth.Third day there the patient fell on floor, she was found family. No idea how long on floor. Huge knot on head.Removed her by ambulance back to hospital to check head injury.Wish we had never seen this place.
I am always amazed how great of a place Mountain View Rehab is. The nursing staff really cares about the patients and makes sure it feels safe. The therapy team is amazing and very professional. The building is clean and I always feel welcomed when visiting. I highly recommend bringing family here.
