Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?
Nurses and Resident Assistants. We are an Ebenezer community and have proper training.
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?

Do you offer any resources or support services to aid residents' transition into the community?
Staff support is available
Do you offer transportation services for residents?
Yes - a once a week shopping trip to a pre-determined destination on Mondays
Could you tell me a little about your dining experience?
All meals are provided in the dining room
What is the process that takes place if a resident has a complaint or an issue they want addressed?
They would talk with our Executive Director or Nurse Manager who office in our building.
What is the cost of living in your community?
We take EW primarily. We can work with private pay up to 6 months - see attached price sheets for private pay pricing.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
Monthly newsletter to all and regularly scheduled care conferences for each
Do you all offer short-term or respite stays? How much does this cost?
No
Are you all a pet-friendly community?
We only allow pets that are certified companion animals with doctors orders.
Are residents able to get Covid vaccination and flu vaccinations in the community?
Yes, we have an annual clinic in the fall for this on-site
Are residents able to age in place?
There are limits at every assisted living facility and reasons that sometimes a person can no longer be cared for in assisted living. We also have a nursing home on our campus where folks sometimes will transition to if needed. Yes
Eligibility Criteria
Please note we only work with residents who are spending down their last $40,000 or less OR those already on Medical Assistance/Elderly Waiver (out of money and on public assistance programs). We are working with the State of Minnesota to serve this population.
