LEVELS OF CARE
There are five levels of care available, allowing each resident to receive, and pay for, only the assistance needed. Your monthly care level will be based on an individual assessment and added to the price of your selected apartment. Assessments occur prior to admission, after 30 days, and then every 6 months thereafter or at change of service.
- Level One: Base rent plus $825
- Level Two: Base rent plus $1,550
- Level Three: Base rent plus $2,575
- Level Four: Base rent plus $3,475
- Custom Level: Base rent plus $3900+
Level of care will be reduced to Level 1 on the 15th consecutive absent day from the community.
MISCELLANEOUS FEES
- Second Occupancy Fee: $800 / month + level of care
- Waitlist Deposit: $750 (applied to community fee)
- One Time Community Fee: $6,000

We are conveniently located on 12 picturesque acres, adjacent to the 26-acre Brookhaven Nature Preserve and less than one mile from Crabtree Valley Mall.

We offer 20 Cottages, 140 Independent Living apartments with an option of supportive services provided by our on-site partner Interim Healthcare.

Additionally, we have 48 Licensed Assisted Living apartments with 24/7 Magnolia Glen care associates.
