Horizons Description
Horizons is a 76-bed Assisted Living Facility designed to meet the needs of advancing age. Our residence is situated in Canandaigua, New York in the beautiful Finger Lakes region, 25 miles southeast of Rochester. Horizons is the only facility in our area also licensed to participate in the Assisted Living Program (ALP) which provides a higher level of care as an alternative to placement in a nursing home. Medicaid is accepted for this program.
