RATES & FEES
Apartment Style Monthly Fee
Studio $5,450
One Bedroom $6,010
Double Occupancy $7,050
Your monthly fee includes:
Three meals daily plus snacks, most utilities, weekly housekeeping and laundry, personal assistance with activities of daily living, medication reminders and monitoring. On-going nursing assessments and health monitoring, 24-hour emergency response system and a variety of activity and recreational programs, including outings.
Technology:

Community Fee: $2,000
The Community Fee is used to help cover the costs associated with the move in process including processing paperwork, completing resident assessments, orientation of the resident to Heritage Woods, and covers other costs associated with operation of the community including the beautification of your apartment and community common areas. Prior to your move into Heritage Woods, the Community Fee is entirely refundable.

Overview of Professional Staff and Training
We have an Executive Director and Assistant Executive Director with many years of experience in both Skilled settings and supportive living.
Director of Nursing is an RN
Assistant Director of Nursing is an RN
Two LPN’s on Staff
Certified Nursing Assistants/CNA’s
ALL staff is required to complete both Semi Annual training and also monthly training on RELIAS
Safety and Well-being Measures
We have 24/7 staffing of CNA’s and our DON is on call 24/7. Both our ED and Assistant ED are also on call 24/7.
All residents are given keys to their apartments and the outside door. The front doors are open daily but locked at 8pm.
Residents are allowed to come and go as they please, we ask that they sign in and out at the front desk for obvious reasons.
Resources and Support Services for Residents
We have several residents who serve as “Welcome Ambassadors” and help show the new resident around and take them to their first few meals or activities. Our Resident Services Director will also try to “match” new residents up with folks with similar interests. Of course, if the new resident refuses any of this we cannot force them to do anything.
Transportation Services
We do have a Community bus which takes residents on outings however we cannot transport individuals to their appointments. With 150 apartments this would be impossible. We can assist them with finding transportation as some insurance companies will reimburse for that.
Dining Experience
The dining room is open 7-9am for breakfast, 11am-1pm for lunch and 4-6pm for dinner. Meals are served restaurant style and there are two specials for lunch and dinner and the “always available” menu if they do not like either special. We also have snacks available at the front desk.
Resident Complaints and Issues
Both our Executive Director and Assistant ED have “open door” policies and residents are encourage to bring up issues with them. There is also a grievance process and forms are provided to residents in their move in binder or at the front desk. There is also a number posted for the local Ombudsman.
Cost of Living
We are supportive living which means we do accept Medicaid (we will file the application when needed. Our current private pay rates are $5450 for a studio and $6010 for a 1BR. This includes all the care. Extra fees apply for internet ($19), Cable ($33) and land line phone ($26 plus 8 cents a minute to dial out). If you move in private pay there is also a onetime community fee of $2000.
Communication with Resident's Families
Our DON will communicate with the residents POA for Healthcare on clinical issues; however, the resident has the right to ask us to not share that information. There are quarterly reviews of the Care Plan unless the family brings up a concern and it needs to be addressed immediately. Our Business Office and AR Specialist usually work with the Financial POA.
Short-term or Respite Stays
We DO NOT offer respite stays.
Pet-Friendly Community
We recently became pet-friendly however, part of the resident’s assessment would determine their ability to take care of the animal as our staff does not. There is also a one time non-refundable Pet Fee of $250 collected at move in. There is also an addendum to the lease requiring someone to take responsibility for the animal if the resident needs to go to the hospital or if it is determined they can no longer care for the animal. It is a very new policy so there are many additional details that we can discuss if and when the times comes.

We currently have studio apartments available and there is a wait list for 1BR apartments.
Heritage Woods of South Elgin
Heritage Woods of South Elgin is a Medicaid-approved Supportive Living community for seniors 65 plus, with a combination of 150 studio and 1-bedroom apartments. Our residents receive 3 meals daily plus snacks, laundry and housekeeping services, activities, planned outings, 24/7 access to care, emergency call devices, kitchenette with sink, mini refrigerator, and microwave. On site beauty salon and barber services.

White Oaks at Spring Street is our private pay memory care community and currently has Apartment openings.
White Oaks of South Elgin
Our additional memory care is White Oaks of South Elgin and is located next to Heritage Woods of South Elgin and this is a supportive living community which accepts Medicaid. It is currently at capacity with an extensive waiting list. The supportive memory care is prioritized with our White Oaks at Spring Street and Heritage Woods of South Elgin residents getting top priority.
