    HealthBridge Post-Acute Rehabilitation is a new joint venture with Mission Point Healthcare Services, who have been providing comprehensive health services to the Greater Grand Rapids and West Michigan communities for over 40 years. HealthBridge is proud to introduce our Rehabilitation to Home program, designed to get you living a healthy lifestyle back at home as quickly as possible.

    HealthBridge is a short-term skilled nursing and rehabilitation center that has been designed to help you recover after an illness, injury, or surgery. Our team of physical, occupational, and speech therapists, as well as nurses and physicians will work with you on an individualized care plan to help you get back to your home. Our goal is to help you maximize your independence and mobility and to provide you with the tools you need to help manage your health care. HealthBridge provides 65-private rooms, lounge and dining areas, and a 3,000 square foot rehabilitation gym. Our experienced staff, quality services, and friendly staff make the road to recovery closer than you think.
