Grace Community Description

Grace Community is a secure community that includes 14 studio apartments, which are part of the larger Mother of Perpetual Help. This facility is designed to provide care and assistance for those suffering from Alzheimer's disease or other forms of dementia. The Mother of Perpetual Help offers a home where you will be cared for and supported. This community encourages independence and wellness to ensure the fullest life possible. We focus on keeping our older adults engaged both physically and mentally. Our activity coordinator plans a wide array of outings for shopping, dining, theater, and sporting events.

Assisted Living

In our communities, we treat older adults with the same care, support, and kindness that we would show to any member of our own families. We believe that independent living is important and should be advocated for, while also providing the appropriate amount of assistance. After all, we're one big family. Our services help your loved one live independently while focusing on their individual empowerment to live their best life.

Memory Care

An estimated 6.2 million Americans age 65 and older are currently living with Alzheimer’s dementia, a number that could grow to 13.8 million by 2060 unless we develop a medical breakthrough to prevent, slow or cure the disease. We will continue to provide memory care to your loved ones at our assisted living and skilled nursing communities located at our main Shrewsbury campus until a discovery is made.

Grace Community provides person-centered care with access to amenities such as three healthy daily meals, chapel services, a beauty salon/barbershop with spa services, a fitness center, private living and dining rooms, and an updated memory care home for those who need specialized care.

Assisted Living Services and Amenities

Grace Community offers the finest assisted living services and amenities. Residents can enjoy three nutritious and delicious daily meals prepared by the in-house culinary team. They are also supported by 24-hour care provided by a highly trained and dedicated staff. Grace Community is a lively community, hosting fun events and outings as well as providing mass pastoral care and spiritual activities for residents. The common areas are beautifully designed for gathering with friends and family, while personal care assistance (such as bathing, getting dressed etc.) is also available.

Memory Care Services and Amenities

Grace Community provides a safe and secure environment for their memory care services, with open living rooms, dedicated dining rooms, and activity spaces. They also have safety features such as wander guards, key-coded main entrances, and secure courtyards. There are supervised and safe outdoor areas as well as three healthy meals served each day. Additionally, Grace Community offers individualized personal care assistance and lively community events and social activities to its residents.
