This nursing home is certified by Medicare and Medicaid and was first approved to provide Medicare and Medicaid services on 10/10/94 and is run by a government organization. It is a large-sized nursing community with 86 nursing beds. Currently, there are 35 residents within the nursing community. Be advised these resident occupancy numbers are not always up to date and are collected from the Center for Medicare and Medicaid Services website periodically.
Overall RatingsThe Golden Years Nursing and Rehabilitation Center is a much-recommended nursing home that has earned a notable 4-star overall rating, placing it within the top 30% of all nursing home establishments. This rating surpasses the state average, demonstrating the facility's commitment to providing superior care. Notably, it boasts a 5-star health inspection rating, the highest achievable score, placing the center in the upper echelon of the top 10% of all facilities when it comes to health inspection scores. This rating is significantly higher than the state average, reflecting the organization's stringent adherence to health and safety standards. However, the site's commitment to quality seems to falter a bit with its 2-star quality rating. This score situates it in the lower 20% of facilities in terms of quality, underlining a potential area of improvement as its score unfortunately trails behind the state average.
Staff RatingsGolden Years Nursing and Rehabilitation Center, despite its promising name, holds a concerning 1-star staff rating, placing it at the lower end of the scale. It ranks among the worst 10% of facilities for nursing aid staff hours, providing only 1.55 hours of aid per resident per day, notably less than the state average of 1.92 hours. Similarly, its LPN staffing hours per resident per day fall short of the state average (0.81 hours as against the average of 1.03), thereby putting the Center in the bottom half of all facilities in this regard. For RN staffing hours, it surprisingly surpasses the state average offering 0.53 hours per resident daily compared to the state's 0.37 hours, though it still remains in the bottom half of facilities. The Center also ranks in the bottom 40% in terms of licensed staffing hours, providing 1.34 hours per resident a day, below the 1.4 hours state average. On a more positive note, Golden Years excels in physical therapist staffing hours, ranking in the top 10% of facilities. Providing 0.16 hours per resident per day, it notably exceeds the state's average of 0.07 hours.
Covid DataThe Golden Years Nursing and Rehabilitation Center has been performing poorly in keeping both its residents and staff up to date with the latest vaccinations. Shockingly, just 2.9% of the residents are inoculated, placing the center in the deplorable bottom 10% when compared to other nursing facilities. This vaccination rate is significantly lower than the state average of 50.1%. Furthermore, the management’s apparent failure in ensuring that their staff receives the most recent vaccinations is evident, as only 3.6% are vaccinated, further dragging down the center’s performance to the bottom 20% among similar facilities. This rate is way below the state average of 20.9%. Therefore, the center's ability to safeguard the health of its residents and employees against infectious diseases is called into question due to these low vaccination rates.
Quality MeasuresThe Golden Years Nursing and Rehabilitation Center, despite offering a solid commitment towards maintaining optimal health rates for its residences, has marked a 2-star quality measure, indicating room for potential improvements. However, in context of immunization, it tops the charts being among the top 20% of facilities, providing a full 100% pneumococcal vaccination rate, far exceeding the state's average of 95.73%. In terms of bladder health, it falls into the lower half of facilities, with 1.12% of residents having a catheter inserted and left in their bladder, though this is still less than the state's average of 1.37%. As it pertains to mental health, the facility is within the top 10% of facilities, having only 1.98% of its residents on antipsychotic medication, far less than the state's 9.7%. Lastly, when reviewing nutrition, the nursing home is in the top 40% of facilities, with only 4.3% of residents experiencing excessive weight loss, less than the state's average of 4.75%.
Emergency and Hospitalization Quality MeasuresThe Golden Years Nursing And Rehabilitation Center, a nursing home, has a mixed record when it comes to hospitalizations and outpatient emergency department visits. Unfortunately, the home ranks within the lowest 10% of facilities in relation to hospitalizations per 1000 days, reporting a rate of 3.62, which is notably higher than the state average of 1.98. On a more positive note, the center performs comparatively better in terms of outpatient emergency department visits. With a rate of 0.92 visits per 1000 days, it sits significantly in the top half of similar facilities. However, it's noteworthy that this figure is still less than the state's average outpatient emergency department visits per 1000 days, which is 1.24.

My father in law had a recent stay here, the staff truly cares about the residents. Our family is very impressed with the staff, ALL the staff, from house keeping to nursing, they are friendly and efficient. The therapy department is awesome! He was able to recover enough function to return home. My FIL said he expected to hate it but was pleasantly surprised that his stay was good, he said the food was MUCH better than the hospital food and he would definitely come back if/when needed. Thank you for all you do!
I transfered my mother toGolden Years two years ago. I can honestly say I am very impressed with the care she has received atthis facility. The staff members truly care about each resident and do their best to make them feel at home there. I appreciate the kindness and dedication from every staff member that cares for my mother.
I was at Golden Years for rehab from my fourth back surgery. Everyone from the office staff to the cleaning staff were terrific. Being that I am only in my 40s I thought I was going to feel very uncomfortable having a male staff member help me shower. On the contrary, the gentleman not only helped me shower but taught me techniques for showering on my own when I got home. The people in the physical therapy department are absolutely wonderful. They helped me become more mobile and taught me ways of doing every day things in easier ways. I really recommend Golden Years to anyone who might be needing this kind of facility.
