Staff at Founders Plaza
Our team comprises Registered Nurses (RNs), Licensed Vocational Nurses (LVNs), Certified Nurse Assistants (CNAs), as well as Physical, Occupational, and Speech therapists. Supporting resident care are professionals such as a Social Worker, Activity Director, and Dietary Manager. Each team member undergoes certification or degree programs, with frequent in-house training supervised by our administrative team. Education is continuous…
Environment
At Founders Plaza, we prioritize providing a comfortable, safe, and joyful environment. Our life enrichment program includes daily activities, respecting residents' choices in engagement. Safety measures include routine in-services, security protocols, fall prevention programs, medication management, and our wander guard system for mobile residents at risk of wandering. For dementia patients exhibiting exit-seeking behavior, please note that we are not a secure unit and would not be a suitable option.
Support Services
We have resources in place, with our social worker acting as a bridge for residents transitioning from the rehab program back to the community. Numerous resources are available to meet various needs.
Transportation
Our facility provides transportation to assist residents to and from scheduled physical appointments that align with their care at Founders Plaza.
Dining & Culinary Services
We offer in-house dining with three meals and snacks daily, catering to residents' dietary needs outlined by their doctor. Our culinary team, overseen by the Dietary Manager and nutritionist, ensures personalized meal service.
Concerns or Complaint
While striving to be a home for our residents, we acknowledge the importance of certain amenities. We provide a guardian angel for each resident, and administrative staff with personal relationship with resident, acting as a one-on-one contact and a lifeline for their family. Administrative staff, including a Director of Nursing and/or Administrator, are available for concerns. A hotline is also in place for reporting concerns, as well as our Accushield system collects feedback during monthly guest check-ins.
Cost
As previously emailed, our private pay rates are $225 per day for a private room and $175 per day for a semi-private room. Latricia in our admissions office to explore benefits or clarify levels of care. We accept MCR and most commercial plans for skilled services as well as PVT pay and Medicaid for the residents who are qualified. Depending on your loved ones level of care could depend on which payer source is accepted.
Communication
Communication varies based on family expectations. Direct communication is available with the responsible party. Routine care plans address adjustments and expectations. Additional updates can be discussed with our social worker. Due to HIPPA specific guidelines must be followed with who and how we share patient information.
Short-Term
We offer both short-term rehabilitation and long-term care. In addition we do offer respite visits. Short-term rehab which typically follows a post-hospitalization or surgery, typically lasts 20-30 days. This is called a skilled stay and is often covered by Medicare or commercial insurance. Pet Friendly
Pet visits must be prearranged with administration, meeting a specific criteria. All shot records must be provided for consideration. Pets are allowed only in resident rooms, and approval depends on roommate considerations. While we welcome visits from four legged loved ones, pets cannot live or stay at Founders Plaza.
COVID Vaccine
We make vaccines available, including those for COVID and flu. Contact our social worker for more information or arrangements.
Age In Place
Founders Plaza supports residents through aging in place. We work with many services like hospice to provide a wide array of services needed to our aging population. To explore our offerings, please coordinate with Steven or Latricia for an in-house tour to learn more on Founders Plaza.

At Founders Plaza, we provide short-term rehabilitation with the goal of returning patients home as well as long-term skilled nursing care for situations where home is no longer a safe and viable option. The payment options may vary based on the required level of care.
We will happily assess any benefits you or your loved one may have and confirm benefit eligibility. In certain cases, patients admitted to us under skilled care may transition back to the community or choose Founders Plaza as their permanent residence based on their progress.
