    Fayette Long-Term Care is a 122-bed skilled nursing facility that offers both Short Term Rehab and Long Term Care for the people of Fayette County and the surrounding area.

    Our Short Term Rehab Program is designed to offer individualized recovery services for those who are not quite ready to go home after a hospitalization for surgery, acute illness or other medical diagnosis. We offer a personalized approach for those who need therapy or skilled nursing services to get back into the community and home utilizing Medicare Part A or other insurance.

    In cooperation with Champion Partners in Rehab, rehabilitation services include: physical, occupational and speech therapy. Our therapists work alongside our trained nursing staff and social services to design an individualized plan of care with goals that work toward returning the individual to their prior level of functional independence and gain strength, endurance, functionality, balance and range of motion. Our team meets daily to make sure you have the support you need to meet your individual goals.

    Our program also provides a myriad of medical services including congestive heart failure/cardiac recovery, stroke rehabilitation and wound care management to name a few.

    For those individuals who need continued support, our Long Term Care program offers 24 hour nursing care provided by trained licensed and professional staff that work hand in hand with our physicians to provide individualized care that meets the physical, mental, psychosocial and spiritual needs of the resident. Our program promotes a homelike environment while providing medical care specific to your needs and wishes.

    Our support services include: beauty shop, dental evaluations, podiatry visits, nutritional assessments/support, individualized activity programs and laundry services.
