    About Our Skilled Nursing Facility

    Our facility is a dedicated skilled nursing community, providing high-level care for those who require medical or nursing care or rehabilitation services. We do not offer assisted living or independent living accommodations such as studios or one-bedroom apartments.

    Availability

    We currently do not have a waitlist, allowing for immediate availability for those in need of skilled nursing care.

    Short-Term Stays

    Our facility includes a rehabilitation center that offers short-term stays for patients who require rehabilitation services.

    Dining Services

    We provide full dining services to our residents, including breakfast, lunch, and dinner, ensuring their nutritional needs are met.

    Staffing

    Our staff-to-resident ratio is approximately one nurse to twelve residents. We employ both Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide comprehensive care to our residents.

    Medical Staff

    Doctors are available and make regular rounds at our facility to ensure the medical needs of our residents are addressed promptly.

    Safety Measures

    Each patient has access to a call light by their bed for immediate assistance. Additionally, we have implemented a WanderGuard system to ensure the safety of residents who may require additional monitoring to prevent wandering.

    Resident Grievances

    We have a formal grievance process in place. If a resident has a complaint, we fill out a form, investigate the issue, and work towards a resolution. Once resolved, we close out the grievance.

    Family Communication

    Communication with families is a priority. We utilize various channels including our website, email, text messaging, and in-person meetings. We also conduct care conferences every three months to keep families informed and involved in the care of their loved ones.
