The Smiley family would like to take this opportunity to thank all the LPNs, CNAs and other staff members that looked out for dad during his stay. I know dad was stubborn, hear headed and sometimes a little mean, but he love each one of the staff. A lot he felt as if they were family. We will miss each of you and remember you in our hearts.

The Lawson family would like to thank all of mom's nurses, CNA.s and everyone for loving mom and taking care of her. I know it started out kinda ruff but we caught on, and mom made "Best Friends". She called all of her CNA's her kids and she loved them all. We know she was hard on ya sometimes but that was her dementia and not her.Thank you for giving mom life for the past year. We will miss you all but remember mom loved you and we did too. Thank you from, Becky, Vicky and Terri

I have only been here for two months now, I'm a long term resident because of the severity of my disability. I have been treated with the utmost respect since arriving, the staff, nurses, and CNA's have gone above and beyond to make my transition as stress free as possible! Thank you for your kindness, compassion, and professionalism! I look forward to our future together.

I must be on a different planet than all the other reviewers! Please tell me how it's possible for my father to be there for rehab and no one seems to either know his rehab plan or have one. And more importantly, please explain how it's possible for my father to have a catheter and be wearing an adult diaper and when I go to see him he is sitting in a urine soaked bed. His room is dirty with liquids in puddles on the floor and sticky drips down the bedside table. I am beyond upset and disgusted with the care and attention of the staff at this facility. I would highly discourage anyone from having their loved ones there.

My grandma is here and I have been so impressed with every single one of her nurses and the communication from the administration. They are always updating me on her progress. They make a huge effort to help her facetime me or do porch visits with us. They take good care of her and its a huge relief.

I didn't have family at this facility however I did have a friend. Her family was told they couldn't even visit her from a window,my friend wasn't in quarantine wither said, that's not correct and told the family they needed to speak to her attorney and start to plan to move her. And at that point this place decided they would let her family see her from a window. They had to be made to let someone visit their own family member through a d glass window from outside the building. SMH!

I do not know where to begin with this place. First let me start by saying that my father was placed here after 10 neuro strokes and congestive heart failure for Rehab by the VA Hospital. I really didn't want him here but I was practically forced to put him here by the VA. They made me sign an arbitrary form if I'm not mistaken upon sign in. The first thing I noticed was them giving my father numerous sugary drinks when he has diabetes so bad that he has had Comas and has had a leg amputated on the right side. I asked a black female in the dining room to give him tea with Sweet and Low instead of lemonade and fruit punch. She must have handed him for drinks the time I was there visiting him for lunch. She begin to argue with me saying she cannot deny him what he wants saying it was against the law. I'm like but I'm his power of attorney and he doesn't know what he is saying because he has dementia he says things over and over again. I begged and pleaded with her to not give my father those drinks. He began to have sugar attacks where they will call me in the middle of the night saying that he fell out of the bed or the wheelchair. One nurse called me around 10 at night and said I'm going to put your father on the floor because he keeps getting out of the bed. Then they would leave him up for 12 to 14 hours a day without rest from breakfast 2 around 8:30 to 10:30 at night saying he wouldn't let them lay him down. His leg that was amputated begin to swell four times the size. And his left leg begin to swell. I asked them to speak with their DON who caught an attitude with me and also refused to correct her staff. She said it was illegal to refuse him food or drinks. He gained 18 to 20 pounds in 2 weeks and mostly in his legs according to documents. They overfed him like a pig because they refuse to follow protocol and staff their employees on the fact that he was diabetic and dementia and incompetent. Because of the stroke he doesnt know what he is saying. It got worst as the days went by. We had a meeting because they put him in a chair where his feet was dragging the floor every day and they refused to put him in a Geri Chair saying that it would be a restraint and that also would be against the law. The very day after the meeting where they agreed to try to fix the chair situation he was taken to a eye doctor appointment where they dragged him scraping his two furthest left toes on the left leg opposite of the amputated leg which is on the right. This caused him to be hospitalized and he caught an infection in his toes causing it to be amputated. Heavenly father. I prayed for my daddy every time I would visit, and read the bible to him thank God I did! Or it could have been worse. We said the lords prayer together. Then the administrator and the DON laughed at me when I complained. Told me to come get his clothes out of the nursing home because it was taking up space. These two woman is extremely prejudice. Then they had their black staff laughing at me in FACEBOOK reviews while at work. One of their cnas, Brenda called me the B word. Because this neglect came up substaintied by the OOLTC. I found out several of the black people are family and so are the other staff. This nursing home is nothing short of a cult. And again I cannot tell you how devastated I am, how hurt I am how much mental anguish I have suffered and how much I cried. How can people who are suppose to be taking care of an elderly person be so very cruel and heartless. These people donot see color.

My grandmother stayed at this facility for 5 days of respit care. The first day she was there she fell within 5 hours of being dropped off because she was left unattended. As a fall risk and a memory care patient this was unacceptable. When picking her up and packing up her personal belongings, her clothes were filthy like she had been dressed in dirty clothes and her dirty clothes were thrown in the floor smelling of mildew from being wet. Her room was not appropriately clean. The whole facility just seems dirty when you walk in. There was no communication between the facility's staff and the hospice staff that regularly works with her. There was no Neuro documentation of her fall. The most off putting thing about her stay here, was when I visited at 9am they had all of the memory care patients sitting in a dark room watching a movie. Why would you take patients that are already confused from thier own afflictions and set them in a dark room shortly after waking up? The only redeeming part to her stay, was her Nurse Emily, was pleasant and tried to provide what information she could at the time of the fall.
