This score is notably lower than the average rating of facilities in the state. The center also received the lowest possible score, a 1-star, in health inspection rating, which puts it in the distressing bottom 10% of facilities when it comes to health inspections. Furthermore, this health inspection score is lower than the state average. Clearwater Nursing & Rehabilitation Center struggles with its quality rating too, falling in the bottom 40% of facilities, a rating that is slightly below the state average.Staff Ratings
Clearwater Nursing & Rehabilitation Center, unfortunately, falls significantly short in comparison to other state nursing facilities, specifically concerning staff ratings and staffing hours. Its staff is assigned a poor 1-star rating, which is the lowest possible. Its nursing aid staffing hours per resident per day measure at 1.89, falling distinctly below the state's standard of 2.53, placing the home in the dismal bottom 30% of facilities. Moreover, Clearwater holds a place within the bottom 10% of facilities regarding LPN staffing hours per resident per day, as it only offers 0.26 hours compared to the state's average of 0.66. Clearwater's record doesn't fare much better regarding RN staffing hours, functioning at 0.24 hours per resident per day in comparison to the state's 0.71, putting it in the unfavorable bottom 10%. Licensed staffing hours are similarly inadequate, as it provides only 0.51 hours per resident per day compared to the state's 1.38. Lastly, in terms of physical therapist staffing hours, Clearwater again lands in the undesirable bottom 20% of facilities with just 0.01 hours compared to the states average of 0.05. Similarly, the staff's adherence to staying up to date with the latest vaccines falls into the top 30%, reflecting a significant 34.0% rate and surpassing the state average of 20.4%. Therefore, the facility is profoundly dedicated to the well-being and safety of its residents and staff by ensuring a higher vaccination rate than the state averages.Quality Measures
The Clearwater Nursing & Rehabilitation Center has a mixed reputation when it comes to patient care. Despite the fact that it falls in the bottom half of facilities with respect to the percentage of residents given the flu vaccine, it surpasses the state's average with a rate of 96.51% compared to 94.62%. On the other hand, the center has a higher proportion of residents with ulcers (8.47%) as opposed to the state’s average (6.99%), which also places it in the bottom half of facilities. However, the Center excels regarding residents with catheters left in their bladder, with a percentage of 0.0%, significantly lower than the state's average of 2.35%, ranking them in the top 20%. The center's record is also commendable regarding patients with depressive symptoms, with none reported, outperforming the state's average of 5.89%. Notwithstanding, the facility does face challenges, as it falls in the bottom 20% in terms of residents' weight loss, with a higher rate of 9.18% compared to the state's 5.68%.Emergency and Hospitalization Quality Measures
Clearwater Nursing & Rehabilitation Center, a nursing home facility, unfortunately ranks in the lower quartile, specifically the bottom 20%, when it comes to hospitalizations per 1000 days, recording a higher rate (2.55) than the state average (1.85). Yet, the center excels in terms of the percentage of short-stay residents who were rehospitalized after a nursing home admission, as it falls within the top 10% of facilities. They seem to be taking good precautions with letting people in. Asked many times if we are doing okay and if we needed anything. It's just sad that our new normal has had to come to this. We need to pray for better days ahead of us. It takes special people to do what you all do. You all have hearts of gold and they'll be a special place for each and everyone of you. Thank you to all the Caregivers and God Bless You and all that you do!
Excellent facility! My mom was blessed to have found this facility! The care was phenomenal. Everyone was wonderful from the administrative staff to the nurses to the care givers to the dining room staff to the entertainment staff. Excellent food and a variety of entertainment. Mom really loved her new home and my family is so grateful for all that they did during her stay.
We were very fortunate to have found this Facility for Grandpa! We really feel at ease knowing that she is part of this great and friendly family, A real blessing for all of us! Especially during COVID-19 they handled the situation extremely efficient & extreme caution! Keep up your great work!
