    Chariton Park Health Care Center is located in beautiful Salisbury, Missouri. Here, we strive to provide individuals with amazing levels of care and rehabilitation services all within a warm and comforting environment. Our team works diligently to provide personalized medical care, skilled nursing, and rehabilitation, social and psychiatric services to help both short-term and long-term residents achieve their goals and maximize independence. We offer three specialized units to best meet an individual’s care needs, addressing complex medical and rehab, dementia/cognitive disorders and various psychiatric disorders.

    Our Medicare Unit offers a variety of complex medical and skilled services including physical, occupational, IV and speech therapies are available as well as wound care and hospice. Since all our beds are dual certified to include both Medicaid and Medicare we are staffed with licensed professionals 24 hours a day.

    The Safe Haven Unit is designed to meet the specific needs of those with dementia and other cognitive disorders. Our 24-hour staff is trained to meet the ever-changing needs of the individuals care needs. While still providing a home like environment the unit is alarmed and secured for their safety.

    The New Beginnings Unit allows residents with various psychiatric and mental disorders to have a fresh start with caring staff who understand. The facility has a great activity program that has daily groups and outings to make each day fun and exciting for the residents. We also offer an enclosed courtyard, which allows time to relax outside or to exercise!

    At Chariton Park Health Care Center, we know that care should be both personal and personalized to every individual. It should be carried out the same way you would care for a loved one at home, always meeting their expectations without compromise. Those that choose to stay, as a long term resident will experience a tranquil place and continuous loving care where they can live the fullest life possible.
