If I could write 0 stars I would. DO NOT SEND YOUR LOVED ONES HERE. Wouldn't let family or the case manager talk to the resident that was ill. Resident claims they took her cell phone away, left her on the floor for two hours, she crawled across the floor and had to dial 911 by herself. They withheld her expensive life needed medication. Do your research about this place... they were fined 20 grand because of complaints and abuse allegations. This place is a death trap. Information can be verified on Medicare.gov
Zero stars, please hear me out, these are CNAs that are there to do get their hourly pay, NOT care for your loved ones. Trust me when I say no one deserves to be there. Zero compassion in how they talk to people, as if they were in a mental ward. Everyone’s deserves compassion. It’s rated the lowest nursing home in your surrounding area for a reason. Do a walk through first and you will see what I mean. Absolutely horrid. Patients dirty, not bathed, no routine checks to see if they are wet therefore someone with dementia unless they say something will sit in their stool. THIS PLACE IS ON DHS LIST. Please for the love of god, your family does not deserve to be here and I am working closely with a social worker to get them to close. As for the VA, what a disappointment in partnering with this place to begin with. Your veterans, are under this care due to your low contracts because you won’t pay for better care.
This place deserves 0 stars. We walked in, got checked in, temp checked, but no hand sanitizer was given. The receptionist couldn’t figure out what room my dad was in. The nurses stations are unmanned most of the time.I asked for help with getting my dads diaper changed - he had been sitting in his own stool for an hour & a half before we got there because he said the call button didn’t work. The nurse at the desk flagged the ONLY NURSE ON THE HALL down and had her change him. She’s the ONLY decent one we encountered, so for her we are grateful. We had to ask they change his soiled linen and still didn’t changed his pillowcase.The doors are left open - the patients don’t have privacy or dignity while being changed.We brought my dad food & I asked a nurse if I could have a plate to make sure his food was warm - she said “it’s in the kitchen. go down this hall and find it.” - I opened the kitchen door & people were in there, told me wrong room. Pointed me to the dining hall where I had to reach my hand into the plastic plates. (Yes, I washed my hands when I arrived to my dads room - as a nursing student, as I know the importance of keeping him & people around him here COVID free.) The counters weren’t clean or wiped down & there was still juice/coffee/crumbs/napkins on tables in dining hall.The floors weren’t swept, & there was a remaining dorito on his room floor and a random stain that looks like blood or pee.I’m not sure why the VA partners with this nursing home. It’s Labor Day weekend so we can’t request my dad be transferred until Tuesday, or else I would have gotten him transferred in a heartbeat.
Have my god father in there and he can't sleep well with his roommate blaring his TV. He doesn't get washed up twice a day or a shower half the time. Seriously I did for 26 years and I made sure they were clean and made sure they got a shower at least once or twice a week. Sad
This place is a joke residents are neglected, cussed out, ignored, and made fun of. They are not handled with the gentleness and care they deserve... The staff are rude, miserable, and have no class or common sense. it's nothing to see a resident with a black eye or head injury.... Not to mention they have RATS ROACHES and the horrible plague of BED BUGS, ITS NOTHING TO SEE A BED BUD CHILLEN ON A COUCH ARM don't sit down no where.... Definitely don't bring your loved one anything of value staff and other residents steal
Sharon in the business office needs to be fired along with Cathy Wilson the Director...Kim in nursing needs to just quit..she is worthless. Melodie is the only upper staff member that should be kept. This place cares more about the money than anything...more than patients for sure. Just close.
This nursing home should not be in business. They cared for my grandmother and throughout her stay she was abused by another resident, over medicated, and most recently had gangrene set up in her foot due to insufficient care; this resulted in her death. I hope that this review helps a person that has a loved one know that your loved one may not survive due to the lack of care.
My mother was transferred to this facility on the recommend from a director from another facility an hour away from Tulsa because of my Mom's dementia issues. In speaking with the management initially it seemed like the facility would be a good place for her needs. There was mix-up right out of the chute regarding her being ambulatory and not being able to be in the locked memory care wing of the facility. They managed to rectify the situation and her care seemed to be working ok. The nursing staff seemed to be on the ball, caring and matter-of-fact on the phone regarding Mom's care especially in light of the fact that I live 1.500 miles away. Trying to get ahold of this facility after hours M-F was very difficult. You had to leave your number on an answering machine and maybe the nurse on duty , if she had time, would get back to you. I never got a call back and eventually got cell phone numbers from specific persons to get to my mom to see how she was. After two months in the facility my mom had been having some swallowing problems and had tests done and protocols followed. Within three days, the nursing staff went in to wake her and she was gone. I'm thankful that she did not suffer with choking to death while eating or getting pneumonia from congested lungs. She was 90 years old. The following protocols were unacceptable by any professional/ medical/senior facility. I was called at 5:10 a.m. PT in. The morning nurse had gone in to wake my mom and she had passed in her sleep. That was the only phone call I ever received from the facility regarding my mom dying. No one from the Administration office ever called me about her death and what happens next. The first call I got the next day was from the cremation society in Tulsa. They had my mom's body and went through the process with me on the phone. Very professional organization. The only other phone call I received,more than once, was from the nursing staff on what to do with my Mom's clothing. I instructed them to keep the clothing and send all other personal belongings to my older sister in Illinois. Those belongings have still not been sent.Excuses have been given multiple times and no clarification was ever given on their part as to me paying for the shipping or any other problems with that event. To this day no one from Administration had ever called to say sorry for your loss. The Management at this facility in my opinion, needs some training in working with intake and care from their position so that we can get answers without frustration almost every time we call to check on our family member. I feel for the nursing and care-taking staff to be working under these difficult life experiences and not have administration that helps with efficient protocols to run this facility smoothly.
