Do you guys currently have a waitlist?

Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?
Our staff consists of caregivers, STNAs, LPNs, and an RN. They undergo training specific to memory care through a program called Hope Blooms. They also receive first aid training and comply with state regulations.
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
Our facility is a specialty facility, and it is devoted to memory care. All of our residents have some form of Dementia/Alzheimer's. The layout of the building suits those who like to wander, every door is secured, and the entire building is enclosed with a fence that has a code and lock. Safety is a great priority for us!
Do you offer any resources or support services to aid residents' transition into the community?
Again, with us being a specialty facility, all of our resources and support are geared toward memory care.
Do you offer transportation services for residents?
We have contracted with the ancillary services that would typically require transportation (Eye Doctor, Ear, Foot, Dentist, and Psych) as a result we don't provide medical transportation. The only appointments our residents maintain are specialists (ex. cardiologist). Those specialist appointments would have to be provided by family members or you can pay for a transportation service. We have a few to recommend. However, we do take our residents on field trips, and that transportation is provided by us!
Could you tell me a little about your dining experience?
The food is prepared by a classically trained chef. All 3 meals are served in the dining room and are given restaurant style. We operate under something called the Mind Diet. I have attached more information!
What is the process that takes place if a resident has a complaint or an issue they want addressed?
Because of Cognitive deficits, our residents are not able to make formal complaints. The very few that can are addressed by our grievance process. There is also a number posted that families can call as well.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
We create a care plan upon admission with the help of the family and we operate under something called person-centered care. See attached. We discuss the care plan quarterly with a meeting. We also will call with immediate changes as well.
Do you all offer short-term or respite stays? How much does this cost?
Yes, no minimum stay at a rate of $250 per day.
Are you all a pet-friendly community?
We allow dogs to visit only, and you must provide shot records. We also have therapy dogs that visit.
Are residents able to get Covid vaccination and flu vaccinations in the community?
Yes
Are residents able to age in place?
Yes, they are! We provide more care than most assisted living. We also allow hospice as well! No Waitlist
Our Rates and Services
Our rates are based on suite style. Each of our suites starts at $5800/mo. Our care is inclusive with caregiving, medication management/nursing, programs, meals and snacks, housekeeping and laundry, room, and utilities. The level of care reflects the amount of time and attention your loved one needs from our team. Obtaining the level of care requires “no strings attached" and our nursing director will meet with your family and conduct an in-person assessment if necessary.
Care Level Determination
There is not one specific care need that changes a level; it is an overall view of how much support is required to manage all activities of daily living.
Suite Amenities
Suites include a full-sized bed, towels, nightstand, table lamp, and built-in closet. We have individualized heater/AC units and bathrooms in all suites.
