Do We Currently Have a Waitlist?
We do not currently have a waitlist. We have open beds for SNF LTC, and Memory care.
Could You Share Pricing and Other General Information?
We accept most insurance including Medicare and Medicaid. Our private rates are: $315 for Semi-Private and $400 for a private room. We are a 170-bed facility with an in-house dialysis center and Spa Cognitive Unit.
Could You Provide an Overview of the Types of Professionals on Our Staff and the Training They Undergo at Your Facility?
We have RN/LPN and state-licensed nurse's aides. All our employees are licensed by the state of Ohio and must complete educational training each year to maintain their license with the state of Ohio. We contract with a company that provides us PT/OT and Speech therapists, as well as an NP in our building daily and on call.
Could You Tell Me About the Measures You Have in Place to Ensure the Safety and Well-being of Our Residents?
We have a secure unit for our dementia population. We have a wander guard system in place for those that do not need the secure unit but do like to wander.
Do You Offer Any Resources or Support Services to Aid Residents' Transition in the Community?
Social Service department works with residents that may want to transition back into the community with home care, meals on wheels, follow-up appointments. They will also help patients secure placement if they are appropriate for ALS.
Do We Offer Transportation Services for Residents?
We do offer medical transportation to appointments, we can help secure transport with private companies or Lake Tran for personal appointments (banking, hairdresser).
Can You Tell Me a Little About Your Dining Experience?
Currently, our dining room is closed due to renovations on the floors. We hope to have it re-open in the next 1-2 months. Each table is served once all the residents are at that table. Tables are set prior to the dining room opening, drinks are served prior to meals being served to them from the kitchen.
What is the Process That Takes Place if a Resident Has a Complaint or an Issue That Wants Addressed?
Residents are all assigned a guardian angel (staff) that visits them weekly, and have information on how to reach them in their room hanging. They can speak to them. They can also voice any complaints or concerns directly to the Unit Manager on each floor or the Social Services department.
What is Our Policy on Communication with Residents' Families? How Do You Keep Them Updated About Their Loved Ones' Well-being?
Social services will set a care conference upon admission within the first 72 hours. After that, each resident has a Care conference each quarter (every 3 months). Any information that needs to be communicated to the family will be done by the Social Service office or nurse manager (hospital needs, new meds, labs, any behavior changes), unless the patient is their own person and they request that no one be notified.
Do We Offer Short Term or Respite Stay?
We do offer both short-term and respite stay. The price is the same as for LTC. If a patient has insurance, we can see if they will authorize payment or any therapy that would be needed. Most insurance requires a hospital stay prior to nursing home stay for skilled care.
Are You a Pet-Friendly Community?
Pets can visit if they are on a leash and you have records of current shots.
Are Residents Able to Get COVID Vaccination and Flu Vaccinations in the Community?
Yes, they are, but we also offer these in-house by our staff.
Are Residents Able to Age in Place?
Yes, we are a LTC facility. We do not currently have a waitlist, we have open beds for SNF LTC, and Memory care.
