Overview of Professionals and Training
We have CNAs, RNs, LPNs on staff at our building. We contract with On Call physician services, a primary MD who rounds weekly, Optum certified NPs, licensed therapies (SLP, PT, OT), Dieticians, Wound Care Specialists, all of whom undergo standard onboarding within their disciplines. We also provide as-needed consulting services for Optometry, Podiatry, Psychiatry.
Safety and Well-being Measures
Nursing staff coupled with administrative teams work diligently to put appropriate measures in place based on each resident’s functional mobility, limitations, and care planning needs.
Resources for Transitioning Residents
Our clinical, therapy, social services, and activity teams are all trained in providing support to residents transitioning from home/hospital to short-term rehab and LTC services.
Transportation Services
We do not have transport services available. Transport is either arranged via Medicaid services when applicable or arranged privately with a vendor of choice/appropriateness in coordination with the resident/family.
Dining Experience
We have communal dining areas on each unit. Preferences and dietary restrictions are discussed with the resident/family on admission to assure meals are delivered according to dietary needs/restrictions if any.
Addressing Complaints and Issues
Grievances can be addressed with nursing, social services, and/or administration when necessary.
Cost of Living
The private pay cost is $576/day. For those entering private pay, we require one month (30 days) plus a security deposit (equivalent to 1 month) up front. This is coordinated with our Business Office Director.
Communication with Residents' Families
We conduct care plan meetings wherein families/residents are to be involved with their discharge planning needs/etc. Nursing and social services are accessible daily for updates/changes to residents’ care plan needs.
Short-term or Respite Stays
We do offer Respite on a case-by-case basis—minimum requirement for private pay costs is 2 weeks. See above for respite private pay costs.
Pet-Friendly Community
We are pet-friendly, so long as pets are toilet trained, in good behavioral control, and owners can provide proof of up-to-date vaccinations. Visitation only, we do not allow pets to live on premise.
Covid and Flu Vaccinations
Yes, residents are able to get Covid vaccination and flu vaccinations in the community.
Aging in Place
We do not offer IL, AL, Enhanced AL. We offer Short Term Rehab, Respite, and Long Term Care services. We do not have a formal waitlist.
