Vineyard Post Acute is home to a team of individuals who are committed and dedicated to ensuring they are providing residents with the best care possible. They provide a comfortable “home-like” environment for their residents, and go above and beyond to ensure resident satisfaction.
My father, aged 95 and bed bound, was transferred by ambulance from the hospital to Vineyard Post-Acute for physical and occupational therapy following a two-and-a-half-week hospital stay.1. Upon discharge from Vineyard Post-Acute, my father was examined and found to have a bedsore on his right heel and several more that were developing on his back. Vineyard Post-Acute has no record of these skin conditions.2. Upon arrival, my father’s personal effects were inventoried by Vineyard Post-Acute but only some of his personal items were included on the list. They neglected to include his glasses, hearing aids and his walker.The next day when my sister visited him, she noticed he was not wearing glasses or hearing aids. She asked the aide who then referred to the inventory list and responded in a disinterested manner that they were not sent with him. Only after her insistence that they look for them were they located in the bag of his personal effects that was sent with him to the facility. Not having access to his glasses or hearing aids severely limited his sight and hearing and for an elderly man with dementia left him frightened and more confused.I was promised by the case worker at Vineyard Post-Acute that an aide would call me at the time of my father’s discharge so that I could ensure all personal items were sent with him. No one ever called me. The significance was that I attempted to ensure that my father left with all of his personal items but of course because no double check was done, several items were not transferred. His glasses and hearing aids were reported as lost. Upon our repeated insistence over a couple of days, they searched for and located them and turned them over to family. His walker was never located even though his name was written all over it. The facility agreed to a cash payment as compensation.3. When discharged and transferred by Wheelchair Express, a medial transfer service, Vineyard Post-Acute did not provide them with paperwork to send with him. No 602 or medication list, no PT or doctor notes. No POLST. Not a single document. Fortunately, there were no incidents during transfer nor after arrival but if there had been, there would have been no medical history to refer.4. My brother was notified one evening that my father had an elevated temperature and that they were going to test his urine for a possible urinary tract infection. No one ever called back to tell us if he tested positive or negative, what medications were given if any, how long he took to heal if in fact he even had a UTI.Most times when I called the main telephone number, no one answered the phone. Not only did no one answer the phone but there is no answering machine to leave a message, no answering service, no emergency number referenced in a recording. Fortunately, we never had an emergency where we needed to speak to someone urgently but it was irritating that it was challenging to call in to the facility.5. Vineyard Post-Acute changed his medications upon admission without consultation of or advice to the family. We asked several times what the medication level was and no one was able to tell us.6. We were notified once that he was found on the floor during bed checks. Fortunately, he suffered no visible injuries other than bruising. While my brother was later notified of the incident, with the limited communication we are not sure what other incidents may have occurred.It is readily evident that the facility is indifferent to the patients’ needs for whom they are charged or the family’s concerns. The poor care, neglect, lack of proper controls and record keeping are unacceptable.
All of the positive reviews are troubling. Vineyard forever changed my husband's and my life when we had to have his 90-year-old mother admitted there because she fell and injured her pelvic bone. Other than that, she was healthy upon entering. She was there for two and a half weeks and came back to live with us. I noticed immediately that she had a chronic cough. All of us also developed COVID symptoms. Before we could obtain an appt to get her tested for COVID, she suddenly died while sitting in her chair. We had to go through the testing process having just lost her. All 3 of us tested positive. I have read nothing but bad COVID statistics about this facility and am appalled. They had up to 30 workers having the virus (while probably going into work) from August onward, and almost double that of patients with the virus on any given month. Did Vineyard Post Acute kill my mother-in-law? Absolutely. It's nauseating.
I cannot thank the staff at Vineyard, most especially Wangi, for the loving care given to my sister Lois, during her time there, and for the consideration shown to me after she passed. I am overwhelmed. The hospice nurses and Guardians office working with Vineyard were wonderful as well. Thank you all.
THIS PLACE SHOULD NOT BE USED AS A REHAB CENTER I thought Kaiser sent me to a rehab center after my foot and leg surgery on july 24 2021 but I ended up in hell. How can they call this place a rehab center when it is simple a poorly run nursing home for Alzheimer / dementia patient. if only I had read the reviews I would have never set a foot in this place.But I must agree with Kandis the only redeeming thing about this place are some of the people that work there.A few of them were very sweet and kind especially the young one as for the oldest worker they were obviously burned out and could not wait to finish their shift and couldn't care less about the patients needs.The next day after my arrival I asked to be moved into a different room because my bed was so bad, I was in a cubicle with no light , the women next to me cried all night complaining she was in a lot of pain, and the other one was telling me over and over that someone had stolen her pillow and her rings. it is only when I was moved into the new room , yes I had more space and more light , that I realized i was not in a rehab center but a nursing home for Alzheimer people. This place is filthy , the smell is so bad that I did not want to get out of my room .In two days the bathroom we have to share with the room next to us did not get clean , no one bother to check on me during the night, and could not get anyone to help me to go to the bathroom . It is only in the morning that a young nurse changed my bed when she found me in tears on the side of it as for the food I feel sorry for the patient that have it everyday. I fell so sick , stress and unsafe that I demanded to be release immediately. I have a hard time believing that some people gave this place 5 stars . Thank god that most of those people don't even realized where they are.
My Sister had to do some rehab there from a bad fall, with a broken shoulder and other injuries. They were very accommodating and took very good care of her. Their staff consists of RN's and CNA's, which are professional and very kind and helpful, going the extra mile to care for elderly, sick patients. The food was fresh everyday, and they try to accommodate requests. The facilities are clean, lots of hand sanitizers and masks, and they really take good care of their clients. They set up meetings with families to discuss progress and help set up after release in home care.As a family member, one has to be proactive and get involved with the care. If you need to utilize one of these type facilities, I do highly recommend this place. Depending on the insurance, Kaiser will pay 100% for care up to 21 days, which was a blessing for our family.
In a word (minus the food) spectacular. The staff at Vineyard Post Acute took amazing care of my rehabilitating 88 year old father. My dad is not a primary English speaker and is hard of hearing. They did everything to accommodate him and helped him in every way. There are a couple knocks - 1, the food. I get it, they are a rehab hospital not a restaurant, but when your an old guy and incapacitated some of the only joy you look forward to each day is lunch and dinner. Next - the AC and heating system are part of an old building. So you are either hot or cold. We almost brought my dad a fan to help cool him off because the heat was almost too much for him. The kind CNA and nurses tried but this was really more of a limitations of an old building. Aside from that, his stay and especially the PT helped him walk better than before his accident. Awesome people. Thank you.
I spent 2 weeks in this hell hole and was glad to leave when I did.The only redeeming thing about this shabby place are some of the CNA's that work there.CNA's are notoriously underpaid, undervalued and overworked. But a few of these folks are wonderful. Thank you to those of you who worked with me and got me cleaned up and fed.The office staff is bad. They don't listen, don't seem to care and are quite condescending at times.The kitchen staff seemed like slackasses to me and they don't really pay attention to dietary needs much at all.The place needs a good cleaning because there are splashes of fluids on walls and they don't seem to wipe everything down at regular intervals. They lost several articles of my clothing and never found them or replaced them.I went many times without eating because they couldn't get my food correct, EVER. Plus it wasn't good and I even had to order in at one point.Spent most of my time there in the most uncomfortable bed I've ever slept on and that's because the rehab portion of my stay was inconsistent and inadequate. Though I must say there are some cool therapists there. But they can get ify too.Some CNA's didn't clean others or myself well at all and when I left there there was an outbreak of an infectious disease. I had to be put in quarantine when I left here and went back to the hospital. There was even many issues with my medicine. Dosages being not given on time, wrong meds at the wrong time and even missing meds! They told me that I was out of one in particular, I should not have been out of it because I didn't take that much of it at all. How do I know? I documented everything that happened to me while I was there.This place stressed me out so badly during my stay that they re-agrivated my condition and made it worse!I even tried calling an ombudsman while I was there because I felt so mistreated. Unfortunately the office was closed during the holidays so I just had to deal.It was literally a nightmare for me the entire time I was there. I'm about 20yrs younger than the average client age and this was my life for 2 weeks. I heard things from residents and staff that I can't unhear. I can't imagine what the residents have to deal with day in and day out for years...If you love your family, don't place them here. They will hate it.There's much more I'm leaving out because I don't want this review to be the longest ever, but I hope you'll reconsider based on reading this.Just know we picked this place because of the glowing reviews online and on Yelp, but it is every bit the opposite. BEWARE.My stay was in Dec-Jan. 2019 Absolutely horrible.
It took three days to get her name correctly. They continued to bring Apple juice when she & I asked them not to. Seems like every time I speak with her, she's been unhappy with the help. Left in the chair too long, not changed for an hour after request, pain meds not given regularly, so I have to call and request for the charge nurse to give her meds. She's 90 with a broken hip. Worst of all, my Aunt had a rash on her bum which wasn't treated for 5 days after I brought the medication her Doctor previously prescribed, because they had to first obtain Doctors approval. Now they say she's going to get a bed sore like its her fault somehow. 2-3 star with a new coat of paint.
Vineyard Post Acute in Petaluma took on 29 evacuees from Santa Rosa Skilled Nursing Facilities who were evacuated from the fires. These residents were triaged to our dining rooms and activities rooms. Our administrator, director of nursing and staff stepped up to care for these patients and residents. Staff from our sister facility assisted in working all shifts to cover the floors. Staff members who have been displaced because of the fires were encouraged to bring their children to work and our administrator sheltered and fed them. It was an amazing feeling of community and family to see how the staff and management at Vineyard Post Acute stepped up and out of their comfort zones during this disaster and great time of need. I am proud to work for such an establishment.
My husband has Alzheimer's disease and is a full-time resident there. After poor care at Our House he's gaining his weight back, has had PT and is still having deep heat treatments to his knees and shoulders. The smiles and caring attitude is wonderful at Vineyard as well as superior care! Music programs every day and a sunny / shady patio make life interesting and pleasant for those who want to take advantage of them. After being evaluated, my husband was found to need blended food, and is helped to eat each meal. I'm very pleased to have found such a good place with staff so caring!
At 37 years old I had a major heart attack and have been in recovery for over a month. The first facility I was sent to after being released from the hospital was Vinyard Post Acute. It has been a fabulous experience, the staff is exceptional, working with nurses, PT, and OT has been nothing but a pleasure. The staff is very knowledgeable and willing to help. Being the youngest patient by many years I have felt like a VIP with my needs quickly being taken care of. My wound care nurse is a Godsend dressing and redressing my wounds which has helped to speed the healing time. The food is probably the best "hospital" food I've had so far in this crazy trip that has had me in 8 different facilities. The support staff is as amazing the nursing and therepy staff continually checking on me and updating me with what's going on. I would recommend this facility to anyone who is in need of rehabilitation therepy or going into a nursing home situation. I will be sad to leave all of the great people I've met here.
THE CNA'S HERE ARE REALLY GREAT! AND THEY HAVE A GREAT CLEANING STAFF. I HAVE A VIEW OF THE COURTYARD THAT I ENJOY. The activities department keeps us busy. I LIKE THE NEW ADMINISTRATOR, HE really puts attention to keeping things GREAT and is always looking for things he can improve.
