Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?
Executive Director, Dir. Of Nursing, Director of Activities, 24-hour Maintenance Director, Memory Care Director/Coordinator, Sales Director, RN, LPNs, Caregivers and CNAs. All staff must attend our monthly mandatory meetings which includes state regulated trainings and in services. All nursing staff have monthly In-Services and trainings regulated by the state. We conduct fire drills, inclement weather drills, full evacuation drills, etc.
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
All residents must wear a 24-hr. pendant in which they will page the caregiver for assistance. Our doors are locked every day at 7pm and unlock at 7am. Times are adjusted based on daylight savings time. Caregivers make rounds every 2 hrs. checking on residents. Depending on their needs they may need more supervision so the caregivers may make rounds every 30mins. to an hour. Our Memory Care residents have secured doors where access is granted using a code that only the staff has access. There’s a secured back patio where they can be outside if the weather permits. The Assisted Living side may come and go as they please with caregivers making rounds every two hours or depending on their needs.
Do you offer any resources or support services to aid residents' transition into the community?
Yes.
Do you offer transportation services for residents?
Yes. Once a week. Sometimes twice depending on the situation.
Could you tell me a little about your dining experience?
Residents have 3 meals a day with two options at every meal. Also there’s a mini breakfast. There’s always dessert at every meal. Drink options – tea (sweet & unsweetened), coffee, lemonade, water. We ask the residents some of their favorites where it can be included in the meals. Please see meal times below:
- Cold Breakfast: 7:30 (fruit, yogurt, cereal, coffee, tea, juice)
- Hot Breakfast: 8:00 (eggs, sausage, bacon, biscuits, oats) Sometimes waffles and pancakes.
- Lunch 12pm – Ex. Pot roast, mashed potatoes, green beans, roll, dessert.
- Dinner 5:00pm – Ex. Meat loaf, cabbage, roasted potatoes, roll, dessert).
What is the process that takes place if a resident has a complaint or an issue they want addressed?
The resident can report it to any staff member. The staff member reports it to their supervisor and the ED. All issues/concerns are addressed at the time it is reported.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
If there’s any issue, concern or change in health status the nurse/Dir. Of Nursing will call you. All residents are evaluated every 6 months and a Care Plan Meeting is scheduled with the family. If there's a decline or notice of change the Dir. Of Nursing/Nurse will call you asap. All financials statements are mailed. Any gatherings such as cook outs or events the family will receive an invitation by mail.
